Throwing dust collector bags into the rear of the suck truck the employee felt a pop in right shoulder. Employee started treatment with our online Teladoc with no change in pain or discomfort. Employee was advised to seek medical attention with a clinic. Right shoulder strain.
Employee was cleaning material off 117 conveyor belt when they felt a pull in is abdomen/groin they reported this to supervisor and denied care at this time. The employee reported to safety on the 27May2025 they were still experiencing pain and we called care and EE was given a home care plan with no relief they were scheduled a clinic visit for further treatment.
Employee injured right shoulder while install a piece of screw conveyor during the second shift on 1/27/25. EE reported it to the supervisor on 1/28/2025 at 10:30 am. EE called Teladoc and was referred to see a clinical doctor.
Employee got dust in is eye on 1-17-2025 and over the weekend the employee had medical treatment which changed the treatment process to a medical to a lost time incident.

Employee was removing a new fine screw from the rear of a pickup truck into the freight elevator in plant 3 group 2 so they could replace the broken screw, in the process the employee pinched A thumb between the flight of the screw and the frame of the elevator resulting in the right thumb fingernail catching being torn off. the employee was treated at the urgent care.

Employee was stepping down off the haul truck and foot hit the ground sooner than expected and felt a sharp pain in left lower back, also had numbness in left lower shin and foot.
Employee was moving a tower mounted hoist when employee felt pain in right heel. The employee stopped right away and reported the injury to supervisor, they contacted work care for the initial treatment. Employee was not feeling any better and was advised to have a clinical visit.
Employee was tossing a rake into a pond to clean algae which resulted in a muscle strain or possible tear. The employee is still awaiting MRI and results to have confirmation of type of injury. Employee has not been prescribed any medications but does have restrictions.

An employee became trapped in the Finish Mill elevator at approx. 03:00 am while going to the 3rd floor. The elevator stalled between the 1st and 2nd floor. Otis elevator was contacted and assisted in removing the uninjured employee. The third floor door contact was the issue per Otis.
At approx. 730 a contractor became trapped in a elevator. Otis was called and assisted removing the uninjured contractor from the elevator. the Otis tech advised us the first floor door contact was not making good contact with the door sensor causing the elevator to shutdown.
While cutting gaskets for a heat exchanger, the utility knife blade slipped off the gasket, slicing through the glove and causing a laceration across the employee's left ring finger.
Two outside contractors were using the elevator to access the top of the 314 bucket elevator. At 4:45 PM they were taking the ""Silver Bullet"" elevator down to get some tools and it stopped between floors. Otis repair was called at that time. Otis arrived on site at 6:22 PM and the two men were released at approximately 6:45 PM.
1) The area that the employee was working in was dusty. PPE does not help that. 2) A cover on the back of a small electrical motor was removed, exposing a shaft. That is what caught the employee's pant leg.
The employee was cleaning up spillage from a conveyor. Some of that material penetrated through pants, socks and then onto the skin of EE's ankles, thereby causing irritation.
The incident happened on 6/14/21 and turned into a reportable incident on 6/17/21. The employee was using a torch and cutting above shoulder height above EE when a spark entered EE's right ear.
The employee was assisting on getting the grinding aid pumps flowing and was turning around to walk back to the control room when EE tripped over a short stool that was left in the walkway falling and catching EE's right leg between the stool and the frame to the grinding aid pumps.
The employee attempted to move an empty 55 gallon drum from inside a building to outside by picking it up and carrying it. As employee was carrying it employee felt a pain in their back.

A come-a-long was being used to attach a bucket to the structure to prevent movement when it shifted and the hook came off striking the employee in the hand. The impact caused a laceration requiring 5 stitches between the pinky and ring finger on the employee's right hand.

The employee was removing a silencer off of a dust collector fan with a coworker and as they were moving it out of the work area the employee took 2 steps backwards to the left and hit left leg on the adjacent pipe that was connected to the dust collector motor. The incident took place at 11:30 AM on Monday and was only reported on Tuesday morning to the employee's supervisor.
Employee was removing a drag lid and pinched thumb resulting in stitches. Employee continues to work.

Employee came into contact with epoxy resin being used to repair cracks in concrete silo walls. Over a period of time employees' neck developed a rash and eyes became puffy/swollen.

A choke occurred in a kiln discharge chute. The injured employee was seen walking unexpectedly into the area housing the discharge end of the pipe. The employee later observed the injured employee was covered in dust material. Further evaluation revealed the injured employee had burns on hands, feet, torso and legs. The injured employee was transported to the hospital.
Employee was exiting the finish mill building went through dust curtain, started walking downstairs lost footing and landed on ankle spraining it.
EE was welding in the reject bin when a piece of slag fell behind EE's safety glasses and landed on the inner corner of EE's left eye resulting in a first degree burn. Antibiotics and eye drops were prescribed. No lost time.

EE was walking along the 603 feeder landing while performing an inspection. He was looking at the skirting and not the walkway. There was an open hole (15""x 15"") in the grating that he stepped through. EE hit his knee on the side of the grating as he fell as well as his elbow.
Employee was hanging up a 3/4"" diameter wire rope choker on to hooks in the truck shop. As he was extending his arms to place the loops over the pegs, he felt a tearing sensation in his RT shoulder.
Employee was cleaning up a spill by the 104 conveyor when a gust of wind kicked up dust into his eyes. Eyes were flushed onsite, but material was still present. EE was transported to clinic for treatment. EE was wearing all required PPE
The employee was walking up the stairs outside the Roll Press building and apparently caught the toe of his boot under one of the steps causing him to fall forward. The employee fell forward landing on his RT elbow.
Employee reported that the Doosan forklift used in the warehouse had a defective horn. They drove the forklift to the tuck-shop and tag it out of service. While walking back to the warehouse, the employee reported that they slipped on the ice near the iron ore ramp.

Employee was working in the clinker basement hooking up several ladders to the hook of the Model 308 Grove Crane. As he was securing the ladders with a rope, he noticed that the rope was tangled in some other equipment. The spotter misinterpreted his movements with his hand as the signal to raise the load, which cause his RT hand to be pinched under the cable.
The employee was unplugging one of the feeders in 117 tunnel using a jackhammer. The employee was positioned on the feeder platform, and was placing the jackhammer bit above the vibrating feeder to break up frozen chunks of rock when he felt a pop in his lower back.
Employee was in the process of climbing up a ladder, when he lost his balance, and fell approximately 6 to 8 feet to the floor level. Employee lost his footing and center of gravity, when climbing up the ladder, possibly due to sharp knee pain experienced, prior to the fall.
Employee was walking on the south side of the cooler building near the IKN fans when his right foot slipped on the snow. The employee twisted his left knee. The employee has a sprained left knee and has been placed on restricted duty.
Employee was directing a tractor-trailer flat bed truck of drainage pipe in the A-Frame building. While directing the truck to move forward, the truck drove over a bump. Apparently there was a loose 4 foot long 2 x 4 on top of a section of the piping which bounced off of the load striking the employee in the left thumb. The employee suffered a broken LT thumb as a result.
Employee was replacing diaphragms on 260 bag house. Employee was kneeling on his left knee using a pipe wrench to loosen diaphragm housing. While loosening one of the diaphragms, he felt a sharp pain on the outside of his RT knee. The diaphragms are located approximately 3 feet from the walking surface.

Employee was working on scale calibration for the 317 and 318 raw feeders. He was carrying 20KG weights and bending under low structures to reach the hooks to hang the weights. The employee complained of back pain after completing his task.
Employee was working on the south shipping load out sock. The employee who was in the basket of a manlift (Model JLG 860SJ) had completed the task and was maneuvering the basket downward. He apparently caught the basket on the load out causing the basket to move upward. He was caught in a pinch point.
Employee was inside the kiln inspecting feed end. While walking, the employee apparently stepped in an area with exposed metal refractory anchors which were welded to the shell. The employee felt something pierce his boot into his foot. Employee was able to walk back to his office and noticed some blood on his sock.

Injured felt a pop in his left shoulder when he caught a pin press that shifted off of the plank on which it was positioned for work on a bucket elevator.
Three employees were pulling the oil pipe out of the burner pipe. The injured employee was standing on a ladder at the burner pipe, the other 2 employees were pulling out the oil pipe. When the end of the pipe came free of the burner pipe, the injured employee could not hold on to his end and the overexertion caused a torn muscle in his forearm.
Affected employee was involved installing a scaffold base layout on 3 1/3 floor of a preheater. Suddenly without any warning, part of the refractory wall above the work area failed and pieces of the wall began falling. The refractory pieces fell about thirty feet striking employee causing injuries.

While working on a piping system for the delivery of a 19% aqua ammonia to the pre-heater some of the solution unexpectedly squirted from an open pipe and entered the injured's right eye causing chemical burns to his eye that were treated with prescription medication.

The truck was equipped with a utility body which had keyless remote locks. An acetylene cylinder (approx 20 lb) had a handle which could be turned by hand instead of needing a wrench. To the best of our ability, we have determined that the acetylene bottle was leaking and the electronic locks ignited the gas.

EMPLOYEE WAS SENT TO CLEAN 149 BELT WALKWAY, WALKWAY HAD COAL GYPSUM, CLINKER STONES STUCK IN GRATING. EMPLOYEE WAS ON HIS KNEE PICKING STONE OUT OF GRATING WHERE HE WENT TO STAND UP TO MOVE, HE TWISTED HIS KNEE.
EE WAS ASSISTING ANOTHER EE IN REMOVING A DUST COLLECTOR BAG FROM 510 DC ON THE 3RD FLOOR FROM EAST TO WEST. AS EE WAS PULLING TO REMOVE THE INNER SCREEN, HIS RIGHT HAND SLIPPED CAUSING HIS RI GHT ELBOW TO STRIKE THE INNER DOOR LATCH.
EE WAS HITTING A BRICK SHIM BETWEEN 2 BRICKS ANDHIT HIS THUMB WITH THE HAMMER.
EMPLOYEE WAS UNCHOKING CLINKER SCALE BELT. AS HE WALKED DOWN STEPS FROM BELT, HE WAS HIT ON THE SHOULDER WITH CLINKER FALLING FROM HOLE IN CLINKER ELEVATOR. HE LOOKED UP TO SEE WHERE IT FELL A ND SLIPPED ON CLINKER AT THE BOTTOM OF THE STEPS HURTING HIS BACK.
EMPLOYEE WAS PULLING UP SUPER SUCKER HOSE BETWEEN THE LANDING FROM 3RD FLOOR TO OTHER FLOORS AND FELT A PAIN IN HIS LOWER BACK.
TRYING TO HELP FLIP OVER A PIECE OF STEEL EE GOTTIP OF LEFT RING FINGER CAUGHT BETWEEN THE STEELAND THE FRAME.
EMPLOYEE WAS INSTALLING A WATER FILTER ON 109-110 TAIL BELT LINE WATER SPRAY PIPE. EE WAS KNEELING PULLING ON A 14"" PIPE WRENCH TO TIGHTEN A 2""X6"" NIPPLE, WHEN HE FELT A PAIN IN HIS LOWER BACK . THE NIPPLE WAS APPROX 20"" IN HEIGHT ALONE.
EE WAS WORKING ON A GUARD IN THE SHOP. HE WAS TRIMMING A PIECE OF ONE END. WHEN ADJUSTING HIS GRIP HE SLID HIS HAND ACROSS THE SHARP END SLICINGHIS FINGER.

EE WAS SENT TO 109 BELT TO CLEAN OUT UNDERNEATH.FROM HEAVY RAIN ON SATURDAY MUD HAD BUILT UP UNDER THE RETURN FEEDERS. MONDAY THE MUD BECAME FROZEN & A JACKHAMMER WAS REQUIRED FOR CLEANUP. MAT ERIAL WAS HEAVY AND EE HAD TO SHOVEL IT.
EE PULLED ARM & SHOULDER WHILE LIFTING A LINER WHICH WEIGHED ABOUT 100 POUNDS.
EE WAS GETTING OFF LOADER AND SLIPPED OR ROCK AND LIFT KNEE.
EMPLOYEE LIFTED A 480 LB DRUM OF OIL WITH THE AID OF A LIFTING BAR ON THE CATWALK ON THE DISCHARGE PILE OF THE MILL.
EE WAS WALKING UP TO TAKE CARE OF PROBLEM AT 108 FEEDER BELT. HE STEPPED IN HOLE OR RAILROAD TRACKS, NOT SURE. TWISTED LEFT KNEE.
IN THE PROCESS OF CHANGING FAN BLADES AND HUB ON 409-1 EMPLOYEE WAS LOOSING HUB BOLT WHEN IT BROKE AND EMPLOYEE FELL BACK KNOCKING HIS HELMET OFF HIS HEAD AND THEN HIS HEAD TOUCHED A HOT TUBE.
EMPLOYEE WAS REMOVING OLD PIECE OF CABLE COILED UP AND THREW IT OVER HIS SHOULDER. THIS CAUSED EMPLOYEE TO TEIST HIS BACK. EMPLOYEE UNDERESTIMATED WEIGHT OF CABLE.
EMPLOYEE WENT TO THE IRON TANK AREA TO SEE WHY DUST WAS LEAKING TO THE GROUND. HE NOTICED A HOLEIN THE SCREW HOUSING WHERE A PIPE HAD FALLEN OFF. SCREW GRABBED AND TURNED THE BELT. EMPLOYEE'S THUMB GOT PINCHED BETWEEN THE GUARD AND SCREW COVER.
EMPLOYEE WAS ATTEMPTING TO ADD OIL TO THE HYDRAULIC RESERVOIR FOR THE COOLER. EMPLOYEE MOVED A FULL 55 GALLON DRUM OF DT25 AWAY FROM RESERVOIR BY LIFTING DRUM ON END AND ROTATING DRUM.
EE WAS MOVING 55 GAL. OF OIL INTO POSITION TO BE PUMPED INTO MILL GEAR HOUSING RES. MIDDLE OF BACK AT THE BASE OF NECK BEGAN TO HURT.
