Fall from Platform at Lehigh Cement Facility. On 10/10/14 a truck driver/loader employed by FT Silfies Inc. was in the process of pre-loading a trailer on miner property. At Aprox. 11:18AM, the driver was standing in side the safety loading rack on top of a trailer when he fell between the lower railing of the rack and the trailer, aprox ten (10) feet to the ground.

Use of sledgehammer to remove wear plates inside a mill. Employee missed the wedge, hit wear plate that a small metal piece broke off and punctured employee in right upper leg area.
Minor back strain from lifting old wear plates that were being removed from a miller lining.

Loader operator drove to Central Control for a pre-shift safety meeting. While exiting the payloader, the operator missed stepped on the last rung and fell backward, landing on the left hip.
The employee was tightening bolts on a compressor using a wrench when EE felt pain in EE's right shoulder.
The employee was removing a wall filter down off of the all-thread and a section of the wire mesh that wraps the outside of the filter caught the left hand of the employee. This caused a laceration to the employees left thumb pad. The employee was not wearing gloves at the time of the incident.
The employee was poking lumps out of a discharge chute with a bar. As employee was poling the material the bar broke through the material, the employee pinched right middle finger between the bar and the edge of the chute.
The employee walked into the pressure washer to retrieve the pressure washer wand and hose and when EE walked back out of the doorway EE slipped on ice. Falling backwards and reaching out with right hand to break the fall, resulting in a sprained right wrist. Located next to the door was a bucket of ice melt, ice scraper, and a shovel.

The employee was walking in the packhouse to check bag inventory for a customer when EE walked around a pallet of bags and found what EE was looking for EE turned around and did not realize EE's proximity to the pallet and caught right foot on the corner of the pallet causing EE to fall, striking knee on the ground. The employee sustained a fractured kneecap.
The employee was working on getting a feed pipe choke out when EE was attempting to remove an air lance from the feed pipe of finish mill 1. The employee was pulling on the air lance and was having difficulty as it was stuck. When the air lance came out it caused the employee's momentum to carry EE backwards. Which caused the employee to roll and sprain EE's right ankle
The employee was tightening the flip on the Finish Mill 3 transport line when the tool EE was using slipped out of the knuckle of the flip causing EE's left finger to get pinched between the tool and transport line.
The employee picked up a strong back for a raw mill door (approx. 110 lbs.) off of the ground to place it onto a pallet and felt a pain in their left shoulder. The employee sustained a sprained shoulder.

A maintenance employee was replacing the drive chain of a discharge elevator with another employee. They were joining a section of chain when the other employee struck the pin of the link and a small fragment of metal broke off and struck the maintenance employee in the forearm about 3-4 inches above the cuff of the glove embedding itself in the forearm.
The employee was working on rebuilding a kiln feed pump bearing housing in the mechanical shop and was swinging a 12 pound dead blow sledgehammer to seat the bearing in the housing and felt a pull in their stomach.
Taking down (rigging) material using chokers. One Kevlar choker gave way & material dropped to ground. Employee went feet first over top of piece and landed on other side of piece of material, landing on their back.

The mill operator was working on a feed pipe choke and was descending down the access ladder. Material then started to fill and overflow the hopper allowing the fines to spill out. This startled the mill operator and the mill operator lost grip off left hand causing the mill operator to fall approx. 3 feet to ground level, falling onto left wrist causing a strain/sprain injury.
The employee was exiting the clinker crane cab area to an above catwalk when a hand hold gave way and caused EE to slip/fall 6 inches into the cab causing a contusion to the lower back and pelvis.

The employee was walking across the parking lot after punching in to report to their area of location. Snow had melted from the night before and then froze over night. It was snowing which covered up the ice that was underneath. The employee slipped and fell causing a sprain/strain to right shoulder.
While climbing down the ladder of John Deere pay loader the employee slipped on the second to last step on the bottom and slipped and fell. The employee hit EE's back on a wheel chock that was on the ground.

The employee went to remove an aluminum basket out of the back of a truck and felt pain in left arm.
The employee was measuring silo's and was stepping around a pipe and felt pain in the left knee.
This incident took place on 11/18/2019 but the individual did not seek medical treatment until 12/2/19. The employee was moving a portable extension ladder and while carrying it EE went to turn catching the ladder. When the ladder caught it pulled on EE's shoulder.
The employee was stepping up from a walkway onto the top of a working surface (the top of a separator). When EE went to push off with left leg EE's knee gave out.
The individual re-aggravated a lower back strain from last year when EE and another employee were moving a valve during a maintenance operation.
Removing hatch from mill. Hatch sprung free, pinching employee's right finger.
The employee was trying to adjust the ""T"" handle on a dial indicator when EE felt a pop and pain on top of EE's right middle finger knuckle.

The repairman was assisting a welder and was holding a piece of steel. The repairman was in a crouched position and when they stood up they felt a pop in the left knee.
The employee was in a awkward body position lying down to place a wrench on a bolt of a piston shaft and when EE got up EE felt a twinge in EE's back.

The employee was walking towards the elevator and was winding a tape while walking. EE did not have eyes on path when EE stepped from one step level to another thinking it was the same level and sprained right knee.
The employee was using a scraper to clean a chute when EE struck something hard suddenly stopping EE's movement and causing pain to EE's shoulder.
The employee was using a rivot buster to clean up around a tail pulley when the employees left finger came into contact with the shaft as it was recoiling causing it to get pinched between the shaft and the collar.
The employee was working off of a 6' tall stepladder when the employee reached to the side with both feet planted and the employee heard a pop in knee.
The employee bruised their knee after slipping on the ground, landing on their knees.
A driver activated the wrong truck access system and activated the one that the injured employee was working on allowing to lower down striking the side of employee's hard hat in a downward motion. The injured employee only asked for treatment and was given on 7/24/17.
The affected employee was jackhammering when they started to heavily sweat and became sick and dry-heaving two hours into their shift. Employee was treated and released after receiving IV fluids.
After ascending and descending a ladder that was to a platform approx. 6 1/2 - 7 feet in height the injured employee felt pain in employee's left shoulder.
Left eye was hit by a piece of material, EE was wearing safety glasses.

Employee was replacing Coal Stock Feeder belt. Employees were pulling the side plate off of the head pulley. Instead of the plate just coming off the head pulley also slide out. When this occurred employee's left middle finger got pinched between the pulley housing and the plate
Two maintenance employees were sent to check for high emissions on the finish mill 2 sweep dust collector. One employee pried open the compartment lid with a pinch bar. The other employee reached for the lid when the pinch bar slipped and that caused the lid to fall. This pinched and cut the employees left middle finger.

Employee was lowering a piece of scrap material with a 5-ton chain fall. Pull chain got tight, so employee ran other direction for a little while, then pulled again. Chain got tight, he gave tug on chain down, felt pain in left shoulder. He continued to work the balance of the day.
Employee was trying to remove a drain plug from the dust collector speed reducer and strained a muscle by his ribs.
Employee was welding, working off of step ladder. Foam insulation underneath work area started on fire & employee tried to put it out. Employee became short of breath.

Employee did not properly lock out and tag out the rotary feeder from feed dust collector number 8. The employee reached inside and cut off the tip of his left middle finger. Employee was trying to unplug the vent line.
Volvo Front End loader began to shake side to side under normal working condition causing the operator to get whiplash.
Employee was removing the short section of iron ore screw when his arm got pinched between the short section and long section of screw. Both sections of the screw were suspended in the air by chain blocks. Employee went to push the long section out of the way and his right arm got pinched when the short section swung further into the long section.
Employee was checking an air operated gate on top of storage silo 2 gate 7. Employee drained moisture bowl and noticed a smell of ammonia. Employee was exposed to ammonia that got into the compressed air lines from the ammonia injection system which had been started earlier in the day. The ammonia injection system was turned off at the time of the exposure.
Employee was air lancing coal bin due to material being hung up in the bin. Employee strained lower back.

One employee was picking up the old crusher hammers from the ground and handing those hammers to EE. EE was then placing the old hammers on a pallet. The co-worker turned to hand another crusher hammer to EE when he noticed EE was on the ground in pain.
Employee raising safety rack. The chain slipped and employee went to grab it and twisted back.
Employee was changing conveyor belt on Finish Mill 1 feeder. Employee pulled on belt and felt sharp pain in shoulder.
A large piece of coating came down and hit the bar that the miner was holding. The bar acted as a lever and this drove the miners hand into the opening of the riser. This resulted in a laceration to miners right hand.

Employee was getting a piece of sheet steel out of the steel rack. The employee needed a 1/8"" x 4 x 8 it was in the middle of 3/8 x 4 x 8. They leaned the 3/8"" sheets to access the 1/8"". Employee went to retrieve 1/8"" sheet the 3/8"" sheets fell back upon the 1/8"" striking employee's right thumb.
Miner was unclogging kiln#1 dust transport line, there was a 3 1/2"" mill ball that was sucked up into the 4"" gap vax hose. The miner had his hand on the high reach basket. The gap vax hose jumped when the ball was picked up by the suction line. Pinching miners hand requiring 2 sutures.
Employee was removing what he thought was an oil fill cap to add oil to an air compressor. The air compressor was shut down before employee removed the cap. The cap was in actuality a cap to a check valve. When employee removed the cap the cap struck the employee on the nose bridge of his safety glasses. The safety glasses cut the bridge of his nose.
The Grad-All was being used to lower the hose connections and pipe splitter from the elevated spray tower platform. The load was secured with a nylon sling and got hung up on a fixed structure beam. The employee slid the load away from the beam. The load then swung back and pinched employees finger against the beam.
Employee was using a hammer and punch. The employee missed the punch and the follow through of the swing caused the employee's swinging hand to come in contact with the object and cut his left index finger.

Employee was grinding steel overhead. Debris was falling from overhead conveyor above his work area. Debris entered right eye, causing eye irritation.
Employee was using a chain block to pull chain drive guard off of #2 limestone belt. Employee felt pain in right forearm and did not think much of the incident at the time. Individual went home and had significant pain and the pain was still there the next day.
While lifting a 40lb. drive coupling off a vertical shaft with a puller the employee felt a pain in his lower back area.
Employee was removing spray lances from conditioning tower when he felt a sharp pain in his right wrist. He went to the Dr. and got a brace for his wrist and continued for full duty. On 9/23/2011 the Dr. put him on restrictions because of no improvement and he still was having pain in his wrist
While loading a used conveyor belt on to the loader bucket the employee felt a pain in his lower left side of his back just above the hip area.
Guys were cleaning in an area with buildup After cleaning a significant amount of the material, there remained a fairly large chunk that was attached to the wall. EE was using a pistol grip jackhammer with an approx. 3 foot long bit to knock down the buildup. The entire chunk came down and either directly or indirectly made contact with EE's instep(above the steel toe).
While unchoking a tire choke on the tire feed conveyor the employee was using a tire fork to remove a tire he felt a pain in his lower back from pulling the tire free with the fork.
While walking down a steep incline catwalk along #4 Coal belt the employee's foot slipped out from under him causing him to feel a slight pain in his lower right back area when he caught himself.

While changing a babbit bearing in #1 Finish Mill Motor, the employee was slipping the sling around the bearing, when the bearing slipped and caught the right middle finger between the bearing and the thrust ring.
While walking backwards carrying a airslide chamber. The employee stumbled and tried to catch himself with his right hand. He shifted the weight of the air slide case to his left arm and felt a pain in the bicep area of the left arm.
While unchoking a plugged feed chute to the #2 Kiln, hot dust flushed out and burnt the employees hands and back of the his neck area.
Cutting open a tube of Eboxywith a box knifer when the knife slipped and cut his right middle finger. It took two stitches. He was ent back to work same day with no restrictions
While shoveling a pile of clinker out of a walkway, the employee lifted the shovel full of clinker, he twisted his body when emptying the shovel into a chute 4 foot from the floor.
While pushing some conveyor belt gravity rollers around on a catwalk, employee felt a sharp pain in his left shoulder.
While lifting a lid off a gearbox with another employee he felt a sharp pain in his chest area. The lid was round and weighed approx. 35 to 70 pounds. The lid had fresh sealant holding it on the box lip at the time.

While walking up a incline steps to work on #2 pan conveyor, employee stumbled and hand fell on pan conveyor rail, jamming his left hand thumb which caused the thumb tip to be broken.
While working under a truck, EE used his arms and hands to push himself from under the vehicle. At that time, he felt a pain in his upper right arm.
While lifting and removing a dust collector lid because of the necessary standing to remove the lid. The employee felt a sharp pain in the lower back because the lid was sealed tight and wasn't free to remove, while he lifted the lid.

While layng down on his back, employee was reaching under a walk behind skid loader to loosen a bolt when he felt a pain in his shoulder area. He tried to work through it and give it time to go away. He was check by the doctor and maintained his work staus.. Until 11/19/09. When he was put on restrictive duty by the doctor.
He was flushing the SNCR return cooling line with sulfuric acid when the pimp dead headed. The line was pressurized. He attempted to depressurize the line by removing the hose and loosened up the hose clamps and started to remove the hose when the pressure from the line pushed through the rubber gloves causing it to spray on his arm and shirt.
Over a period of time carpal tunnel set in on his hand and wrist. He went to the DR. and got special gloves and did his normal job until 12/29/2009 then he is having surgery on both hands one at a time.
EE trying to unchoke #19 cement silo, banging on the side of the cone bottom. Reached down to turn off the air to the silo and opened the wrong valve, which caused a pressurized silo to blow cement powder dust in both eyes. Force knocked off glasses and hard hat from EE. Prescription medication given to him for eyes in the hospital.
Replacing FM#3 mill liners. While installing liner, had to release tension on cable supporting liner to remove cable and was struck through the glove by frayed off cable, causing puncture. Sutures on right hand.
Injury and other employee were lifting chute on the Kawaski when he felt sharp pain in lower back. He did not have body square and twisted when lifting on vehicle.
Injury and other ee were lifting shell liners into finish mill #2 together when he felt a pull in his back. He continued to put the liner in the mill. Went to dr 6/30/09
EE was changing bags in #2 baghouse, when his wrists started to hurt. Went to the doctor on 4/27/09.
Employee was picking up dropped tires underneath the kiln #1 area while tire feeding was going on. One tire dropped and accidentally hit his left neck and shoulder. After one hour of work he was complaining of pain on his shoulder and was taken to hospital for check up.
Employee was attempting to remove a masonry pallet from the palletizer at the pallet pusher in the process of changing from masonry to a portland packing scenario. Lifting the pallet over the pallet guide rail to remove it from the line, she believes the pallet edge caught the safety lanyard which made one corner drop striking left knee and pulling lower back.
Employee was crawling in # 2 under grate compartment of # 1 cooler when he felt a pop in his left knee and a sharp pain. He crawled back out and could not put any weight on that leg.
Employee was pushing large cable in an upward motion all day and strained shoulder.
Mounting 20"" flexible duct hose on portable dust collector, felt pull in lower back. Due to large size of duct hose, weight, and trying to wrestle in position to mount.
Employee was removing heat shield from kiln #2 pier 3 south east trunion. He was guiding the shield down when it got away from him. He tried to stop it when he felt something pull in his left shoulder. Driving home after work had trouble steering his truck.
Installing new tail ring in #2 kiln. Hand was crushed between the kiln feed shelf and piece of kiln tail ring. (Stainless steel plate 48""x30""x1""). Surgery on hand June 12, 2009.
Employee was working on a tire delivery system when shirt got caught causing roller to pull right arm between the rollers. Pain to whole right arm and right wrist the worst. He was found concious, yelling in pain. Called 911 and he was transported to reading hospital.
Doing inspection at Coal Belt # 4 discharge. Afterward went through the inclined catwalk then to the 3-step stairway. As he walked down the steps, he happened to split his legs. On 1/14/2009 at 3:30PM he complained about pain on his left leg.

EMPLOYEE WAS REBAGGING FM 2 DUST COLLECTOR ALL DAY. CAME IN NEXT MORNING TO CONTINUE WHERE HE LEFT OFF AND HE HAD PAIN IN HIS LEFT HAND.
While working on iron ore screw, bent down to do some measurements, felt a ""pop"" in his left knee. No pain at first, later felt pain and knee filling with fluid. Went to doctor 10/10/08.
While walking down ramp from clinker silos to shop for parts, slipped on clinker and reacted not to fall, twisted right knee.
EE was walking up an incline when he felt discomfort behind his right knee. Also felt fatigued and a twitch in his back.
Plunging mill cutter into shaft to machine key. Felt pinch on right side of neck. Few minutes later while turning head felt a pop at same area. Pain progressing - went to doctor.
Clearing kiln feed, fish scales located at the feed enf of kiln 2.Cable wire is wrapped around the kiln to an attached pulley system with a weight to keep the fish scales firmly tight against the kiln shell.EE lifted cable wire with weight to allow the fish scales to open to allow material to clear out when he lost his grip. tried to catch wire and felt discomfort in back.
While riding down on the finish mill passenger elevator, the elevator stopped prematurely. The care free fell a few ft and stopped abruptly. This caused EE to fall down on his left ankle which had been operated on 8yrs ago. ankle has steel pins in and the fall may have aggravated the old injury, the ankle did swell and became painful after the fall.
Flying debris from istalling window pane got into his left eye. Went to doctor on 7/31/08 and was treated with Tobradex drops.
Employee was involved with emptying kiln vent dust collector hoppers. He was at the ""spare"" screw jogging it to attempt to control flow of material. All access doors/panels were open dumping to ground. Hot vent dust hit employee in face. Went to doctor 5/1/08.
Working on #2 cooler when dirt fell from build up above and fell in right eye. Foreign body removal.
While removing the doors on the bottom of #3 finish mill material cooler, the door began to fall and when EE reacted, it struck the inside of his right forearm and wrist area. He kept working until he realized the pain and aching wasn't going away. Went to the doctor on 7/18/08.
Performing normal job duties and developed carpal tunnel in both wrists and fingers. Numbness in fingers.
While working on the solenoid for the ejector arm, came up under the piston striking the top of his helmet.
EE slipped on damp steps ond fell down them. EE's left wrist became swollen and bruised, medical diag. was a sprain.
Employee standing watch in dusty area at Main Screw Vents. Employee got foreign body in left eye.
Employee was drilling a steel bar in the machine shop. He was brushing away a metal shaving from the drill bit when the shaving hung up on the glove. Metal sliced into his finger causing a cut. Two sutures in finger.
Employee cutting tack welds with cutting wheel and grinder. Grinder kicked back and caused laceration of employee's right index finger knuckle.
Employee was hit by grinder sparks in left eye from worker next to him, causing foreign body abrasion of left eye.
While mounting 8 gauge gun to shoot in the cooler. He lifted gun up on his shoulder to line up belt to hold gun to the mount. He felt something pop in the middle of his back. After done shooting went to another job when central control called. Finished shift went home.
Employee was trying to break the lumps at cooler #1 using the cooler shot gun (8 guage) and one of his shots back fired leaving him sore on the eyes.

Employee complained that his left wrist and hand hurt. Asked him to go to his family doctor who diagnosed possible carpal tunnel syndrome of the left wrist and hand. Next sent employee to our company doctor to be examined.
Finish mill #2, silo #5 ramsey belt: employee was cutting belt away from body with utility knife. Braced hand on angle and blade slipped cutting left index finger.
Cleaning out chute from collecting belt to feed belt. Material very hard and lump fell down and got stuck on discharge hood on feed belt. He went down to feed belt and tried to dislodge lump and the lump rolled and caught hand between lump and discharge hood.
He was working with a 6x6x20 ft., 100 lb. pc. of steel sliding it out, and when sliding it back in with a coworker (it was on a 3 ft. rack) it fell off the rack striking his shin and ankle.
Employee grinding black steel angle iron and tearing down scaffold planks. Debris entered right eye during work day.
Employee was burning bolts off of paddles in conveyor and received burn under right arm.
Employee was walking to discharge of #2 cooler to assit co-worker in removing metal from drag conv. Enviroment was very dusty & EE was using flashlight to see where he was stepping & struck head against scaffolding. Fell to knees & soon complained of headache, nausea & possible neck strain.
He was to fast vac lime blimp out. He entered the blimp to assist in job set up. The lime was very deep and above his knee. Material eventaully worked its way into his boot and sock causing burn.
Breaking hard cement on exit catwalk from bulk silos with a digging bar when pain occurred in lower back & down right leg into right foot.
EE was scraping gasket material off bearing housing. 2 other EE's were on north side of #3 finish mill motor jacking motor shaft. Inj Ee saw rotor turn & he was laying on his back before he knew what happened, rotor turned & kicked out jack & wood cribbing. Cribbing struck EE above left knee.
EE and another EE were taking vent collector/dust collector bags offf cages in dust collector. Repetitive work was done on and off for over two to three months.
While removing a tire coupling from dust pump #1 in an attempt to keep coupling from falling, another EE and injured EE both had a hold of the coupling. While EE was slipping coupling off of shaft, other EE let go, injured EE did not. Coupling fell on his hand and smashed his thumb between coupling and a hammer head.
Cement dust in eyes.
While trying to clear a tire choke at kiln #1 tire chute, he felt a strain between his right shoulder and neck area. Went to doctor on 3/5/07.
While employee was removing center bar after remiving kiln # 2 dust pump starter bucket, a ground wire that was not fully taped touched the buss bar behind the cabinet. The touching of the wire caused a flash which caused the burning to the hand and forearm.
Employee poking lump at cement rock quarry silo # 5. He was prying a lump which dropped causing bar to pinch finger against hand railing. finger tip is swollen and discolored. He went to a Doctor 2/27/07.

Trying to open tailgate on company truck to haul a % gal. oil part to fix leak on backhoe. Tailgate stuck and when he pulled harder with his left hand he hurt his left shoulder.
Employee was poking up the ramsey feeder belt from the tail pulley to clear a lump. He caught the edge of the feeder cover with his right hand ripping the thumb nail loose.
Employee was pulling wires from a piece of pipe which was stuck and had a slight pain at the time. He went to the dr on 7/27/06.
Poking lumps in rock silo #6, lump fell and struck the bar. Tried to hold bar back and felt shooting pain in forearm. Went to doctor 6/6/06.
Pulling trunion and bearing unit from casing at finish mill #1 tailing elevator, dirt falling from everywhere entered over the top of his safety glasses. Eye Consultants of PA took object from eye.
Cleaning a flask in the lab and heating diluted acid in the flask to clean it, she picked up a boiling flask with a towel and it bubbled over and hot liquid burned her hand.
Kneeling on catwalk & leaning inside drag thru at access door to tighten bolt for the wearback. While crawling out after tightening bolt he felt shooting pain from above his knee up his left leg.
Pulling bar grating up to elevated level. Workers failed to have guideline on piece, and rope became frayed and broke. This caused grating to fall to ground. The piece bounced off ground and hit employee in calf. A cut requiring 7 stitches occurred. He was off from work from 5/11/06-05/28/06. Returned to work on 5/29/06.
Entering raw mill #2 to repair center discharge screen, hit lower back on throat fins.

WHILE CAD WELDING A GROUND WIRE ONTO A GROUND ROD, THE CAD WELDING CHARGE BLEW UP & HOT CAD MATERIAL LODGED UNDER THE GLOVE AND BURNED THE HAND. HE HAD 2ND & 3RD DEG BURNS. PARTIAL 3RD DEG BURNS.
TRYING TO TURN A STUCK WATER VALVE AND STRAINED LOWER BACK DUE TO THE TIGHTNESS OF THE VALVE.

Employee and a co-worker were aligning door for closure. Employee was aligning and the co-worker was closing and employee's hand got caught between door and conduct and locking bar. Went to dr. 10-19-05.
Employee was tightening up a bolt on a compressor, bolt broke, and he slipped back, fell, and broke his right ankle.
Trying to remove airslide from inside of silo using two pinch bars. Applying pressure to bar, bar slipped airslide dropped and finger was caught between bar and airslide.

Employee #1 was working in the clinker cooler, when employee #2's foot hung up in a beam. Employee #2 fell and knocked employee #1 down and landed on top of him.
EE was cleaning things out to be able to install liners. He moved a piece of plywood and there was a liner jammed above the plywood. Whe he pulled the plywood the liner came down striking his big toe on left foot.
EE was entering room to raw mill compressors. Door slammed shut on EE's middle finger resulting in laceration & fracture. Stitches in finger-removed 3/24/05.
EE was walking towards locker room & slipped & fell on snow covered roadway. Put on restricted duty 3/2/05. Suffered blood clot in right upper leg and strain to lower back.

Replacing seal plates on kiln #1 feed end seal with a coworker. Employee was working out of a man basket on a forklift. lowing the forklift, the forks stuck and then dropped 10 feet all at once causing employee to fall forward in basket. safety glasses hit basket and cut his left eye brow.
Finish mill #3 feed pipe was choked. using pinch bar to get choke out bar got caught by mill and spun bar between bar and feed pipe which cut left hand finger tip off---called and amputated first joint of ring finger.
GETTING OUT OF VAC TRUCK, HIS LEFT FOOT ROLLED UNDER HIM. HE DID NOT FALL AS HE CAUGHT HIMSELF.

EE was sledge hammering on chute from silo 11 to the feed belt when his right foot slipped on loose clinker on the catwalk and he twisted his right knee.
Tightening tee-bolts on the right side #2 clinker cooler when wrench slipped off causing his elbow to hit the steel cooler wall. Went to Dr 3-25-04.
HELPING TO INSTALL A LINER PLATE IN FM #2. WHILE PUSHING THE LINER IN PLACE, HE SET HIS FEET ON THE SHELL LINING TO GET LEVERAGE. ANOTHER EMPLOYEE USED HIS RIGHT FOOT AS LEVERAGE TO PUSH LINER IN PLACE. PRESSURE ON FOOT CAUSED PAIN.
EMPLOYEE, GETTING OUT OF REPAIR TRUCK HE HAD PARKED BETWEEN STOREHOUSE AND WASH HOUSE, SLIPPED ON THE ICE. HE HIT HIS BACK ON THE GROUND AND HIT HIS HEAD ON THE TRUCK DOOR FRAME.

MAKING AN ADJUSTMENT ON THE BAG FEEDER WITH HIS RIGHT HAND. LEFT HAND APPARENTLY WAS RESTING ALONG SIDE THE LIFTING TABLE. THE FEEDER SENSOR WHICH IS OPERATED BY AIR SENSES THERE WAS NO BAG PR ESENT AND TABLE CAME DOWN AND CAUGHT HIS LEFT INDEX FINGER.

PICKED UP A 3 1/2"" GRINDING BALL OFF THE GROUND TO PUT IN BARRELL. AS BALL WAS PUT IN THE BARRELL, ANOTHER BALL CAME OUT OF THE BALL SORTER HITTING FINGER BETWEEN GRINDING BALLS SMASHING FINGER.
PULLING STARTER FOR KF BLOWER #6 AND FELT A POP IN LEFT ELBOW. EMPLOYEE WAS NO LONGER ABLE TO USE LEFT ARM EFFECTIVELY. TRYING TO STRAIGHTEN ARM, FELT TINGLING IN LEFT ARM.
CLEARING A CHOKE IN THE DISCHARGE CHUTE OF LIMESTONE BELT #1 STANDING ON A PILE OF BUILD UP OF LIMESTONE AND DUST FROM THE CHOCKED DISCHARGE. A LARGE PIECE OF MATERIAL CAME OUT THE DOOR, HIT E MPLOYEE ON LEFT SHOULDER. LOST HER BALANCE AND FELL ON HER BACK.
