Employee's right thumb struck the edge of an 8 inch grinding disk while in motion. The employee released the trigger and set the grinder upside down and reached passed the grinding wheel and struck his right thumb on the wheel. Laceration to right thumb requirded 8 stiches and modified work duty and follow up medical care over the next two weeks.

Struck against stationary object

Employee was using a box end combination wrench to tighten the mounting bolts on a rotary feed chute. The wrench slipped off the nut, and caused The back of his left hand to strike the end of a protruding bolt. This casued a small abrasion. The emplyee self treated the injury on 02-10-09. On 02-13-09 the employee was sent to Concentra Medical for an infection/oral antibiotics

Struck against stationary object

Employee was working on a piece of motor when his lower left leg hit a piece of metal on the motor causing a laceration to his leg. The employee self treated the injury and did not report the injury until 07-28-08. The doctor's report was delayed in the mail system and was not found by the ALPM until 10-24-08.

Struck against a moving object

EE was filling drill shots with a Kawasaki loader. The ground was frozen and rutted, one tire slipped into a rut, jerking the loader violently. EE was wearing his seatbelt, but the action strained his lower back.

Struck by flying object

Employee was approaching secondary crusher to fix a spray bar. The crusher suddenly ejected a rock which struck him on the top of his hardhat. He had on all his required PPE.

Employee was walking from the scale house back to company truck & tripped over his feet falling to the ground. Whe he hit the ground the wind was knocked from him. Getting up he felt soreness in his chest. He was sent the local hospital to be checked.

Employee was placing a sign post into the ground. Due to under lying rock, when he hit post it moved. On the second hit, he hit his right index finger with hand-held sledge.

Employee felt sick and had chest pains. Initially thought to be a possible heart attack, he was taken to hospital via ambulance. Turned out to be other non-heart attack related medical conditions - blockage in blood vessel and other complications.

Employee exited tri-axel dump truck at customer site, turned to close door of truck, bumped elbow on edge of door.

Employee was hauling rock from top level of quarry to dump. While employee was exiting ramp, he stopped halfway up, approximately 100 feet, attempted to back down and turned left, came in contact with loose rock on left shoulder upsetting truck onto its right side.

EMPLOYEE WAS USING TRACKHOE BUCKET TO REMOVE ROCK IN FRONT OF A SHOT, PREPARING IT FOR LOADING OUT. THE BUCKET TEETH SLIPPED FROM A ROCK THAT THE EMPLOYEE WAS MOVING, CAUSING THE MACHINE TO ROCK BACK & FORTH (PINCHED NERVE NECK/SHOULDER).

TWO MAINTENANCE WORKERS WERE REMOVING A FLYWHEEL OFF WASH SCREEN TO REPLACE BEARING BEHIND IT. FLYWHEEL JAMMED AND WORKER USED COME-A-LONG AND HAMMERS TO FREE FLYWHEEL. INJURED MAN HIT SIDE OF FLYWHEEL AND INSTEAD OF MOVING A SMALL AMOUNT THE FLYWHEEL FLEW OFF THE SHAFT. INJURED MAN JUMPED BACK, SLIPPED, FALLING TO HAND & KNEES AND FLYWHEEL LANDED ON RIGHT HAND, THUMB & FOREFINGER.

EMPLOYEE WAS USING A 24 INCH PIPE WRENCH TO FREEA STUCK SHAFT. WRENCH SLIPPED THEN CAUGHT AGAIN. HIS HAND WAS BETWEEN THE PIPE WRENCH & HIS BODY & HE HEARD SOMETHING SNAP. HE WORKED THE NEXT 2 DAYS THEN DECIDED TO TELL HIS SUPERVISOR & WAS SENT FOR X-RAYS. IT WAS FOUND HE BROKE A BONE ABOVE HIS LEFT HAND LITTLE FINGER.

HAUL TRUCK DRIVER LEFT BED UP AFTER UNLOADING ATSURGE CONSOLIDATED QUARRY. WHILE CROSSING SR 182HE WEDGED THE TRUCK UNDER AN OVERPASS. THE CAB WAS SUSPENDED 10'OFF THE GROUND. OPERATOR WAS WEARRING SEAT BELT.

FUELING CAT 980 FRON END LOADER, THE FUEL HOSE SLIPPED FROM HIS HANDS. THE EMPLOYEE THEN JUMPED FROM THE LOADER (APPROX. 4') TO THE GROUND INJURING HIS LEFT ANKLE.
