Employee was repairing part of stretch hood at palletizer for packaged cement production line. Knife blade used to cut bags over top of pallets cut into employee's hand.

Fall down stairs

Employee was descending staircase and tripped and fell down three subsequent steps landing on their knee. Knee sustained a sprain.

Over-exertion in lifting objects

Employee was carrying large clinker ball removed from clinker discharge. When they stepped off of clinker drag they felt a sharp pain in lower back and pain shooting down both legs following with numb feet. Employee sought medical attention and incurred lost time as of 8/20/19

Over-exertion in lifting objects

Employee was returning excess sampled cement back into finish mill via 5 gallon bucket. Employee lifted bucket full of cement up to reclaim screw and strained shoulder in process. Employee sought medical care and had physical therapy. Recent MRI indicated tear in shoulder and employee was referred to surgeon. Employee underwent surgery 10/9/19 and began losing time from work.

Absorption of radiations, caustics, toxic and noxious substances

Employee got loader stuck in kiln dust and when EE exited the cab stepped into a mixture of kiln dust and water which entered both boots causing an alkali burn to the top of the feet and the ankle.

Accident type, without injuries

Maintenance mechanic was coming down the passenger elevator from the top of the 56 silos. The passenger elevator stopped during the flight to ground level. An electrician climbed the ladder way to the top of the silos, activated the elevator key to open and close the door and the car was recalled to the top.

Struck by falling object

Employee was working on the Kiln performing re-bricking. When stepped down to retrieve a hammer a refractory brick came loose and struck ee in the lower back. Incident became reportable when the employee received an epidural shot to lower back on 2/22/17

Struck by flying object

Employee was working in the raw mill, hammering out mill liner bolts. Employee was driving out bolts with a sledge hammer and bolt ricocheted back and struck employee in the mouth. Laceration to lower lip required two stitches. Chip on upper left incisor tooth.

Employee was cutting a 3-inch pipe line and when cut was almost complete, the blade of the bandsaw being used crimped in the cut. At the same time the pipe started to fall and the employee accidently triggered the saw and cut into fingers of right hand, requiring stitch

Employee was operating an air impact wrench. The wrench extension was modified with a bolt tore serve the socket to the extension. When the wrench was accidentally activated. The bolt tore into the employee's left hand between the under and middle finger.

Fire had started in the coke mill fining system blower while using rubber fluff fuel. fire resulted in smoky conditions and employee suffered smoke inhalation respiratory distress during kiln shut down and firefighting procedures.

Employee had arrived for work and was leaving the parking lot to come in and slipped on the snow and ice injuring his left knee.

Employee was applying belt dressing to the one-inch stone head pulley when his right hand and wrist was caught in the head pulley. Employee had lacerations and contusions to the right hand and arm, and sustained a hairline fracture to the right wrist.

EMPLOYEE WAS INSTALLING A TANK INTO A PIT USING A FORKLIFT WITH A CHAIN PLACED ACROSS THE FORKLIFT FORKS. WHILE LOWERING THE TANK INTO THE PIT, CHAIN SLIPPED OFF THE FORKS, CAUGHT THE EMPLOYEE , AND PULLED HIM INTO THE PIT ON TOP OF THE METAL BOX.

EE WAS POKING THE BACK END OF THE KILN WITH A BAR AND APPARENTLY CAUSED A METAL PARTICLE TO BE EMBEDDED IN HIS RIGHT EYE. EE WENT TO DOCTOR THE NEXT DAY AND IT WAS REMOVED.

EMPLOYEE LIFTED A 55-GALLON OFF A STAND TO EMPTY IT INTO A HOLDING DRUM. THE DRUM SLIPPED OUT OF THE EMPLOYEE'S GRASP, CAUSING IT TO FALL ONTO THE FRONT OF THE STAND PINCHING HIS FINGER BETWE EN DRUM STAND AND 55-GALLON DRUM.
