Employee's were attempting to empty a small metal scrap bin into a dumpster. They were using an overhead hoist to raise the bin. Upon turning the bin over to empty it out, one employee got hand pinched between the bin and dumpster, causing a contusion to right hand.
Employee was pulling a conveyor belt with a skid steer when the clamp slipped off the belt and struck EE in the right knee.
The Hired Hauler was tarping his truck after being loaded, the handle broken causing him to lose his balance. The driver fell (same level) bumped his head. This is not an Martin Limestone employee and some of the information is not known.
The EE called his supervisor to complain of chest pains. The supervisor immediately told the EE to shut down the equipment he was operating and that he (the super) was on his way. The EE was finally convinced to go to the hospital after speaking with his supervisor and his physician. He is currently treating for hypertension.
As the EE was stepping off of the service truck he twisted his L ankle on uneven ground.
The EE was exiting the track hoe when the door blew shut causing a minor wound the his R leg just below the knee area. The injury exacerbated an existing condition causing infection. The employee didn't tell the site supervisor until 3/15 and it was not reported until 3/16.
While exiting the skid loader that the ee was operating he slipped on the bucket and sprained his lower back. Although the injury occurred on 1/27/11 it did not become reportable until I received the DR's paperwork on 1/31/11.
ANKLE - SPRAIN - EMPLOYEE WAS GETTING OUT OF HIS WORK VAN, STEPPED ON A LARGE STONE AND TURNED HIS ANKLE OVER.
Employee was changing a bearing on a screen and while pulling hard to break a bolt loose strained his abdomin.
The employee was providing assistance in fabricating a work stand in the Shop. As he was carrying a section of steel grating and attempting to lift it onto the stand, the piece of steel slipped from the employee's grip and caught his finger against the edge of the work stand.
After loading his truck from the overhead bins, the employee noticed stones leaking out from the back of his bin dump truck. He began removing the material that was preventing the tailgate from completely closing with his fingers. As he cleared the material, the tailgate suddenly shut and pinched his fingers against the dump bed.
As the employee picked up a piece of scrap steel plate from the ground at the Upper Plant, a 4-foot section of 2-inch angle iron slid off the plate and fell on top of his right foot.
AS THE EMPLOYEE WAS KNOCKING OFF DUST AND STONES FROM A CONVEYOR CATWALK WITH A METAL BAR, HIS RIGHT INDEX FINGER BECAME PINCHED BETWEEN THE METAL AND THE SIDE OF A CONVEYOR, CAUSING A LACERATION.
WHILE TAPPING ON THE ENGINE TIMING COVER ON A FRONT END LOADER, A PIECE OF PAINT CHIPPED OFF & STRUCK THE EE'S RIGHT EYE.
THE EMPLOYEE DEVELOPED BACK PAIN AFTER FITTING A SCREEN INTO THE BOTTOM DECK OF A POWER SCREEN.
WHILE THE EEE WAS ATTEMPTING TO UPRIGHT AN APPROXIMATELY 200 POUND STEEL SHAFT FROM GROUND LEVEL TO A VERTICAL POSITION, THE SHAFT SLIPPED FROM HIS HANDS AND CRUSHED THE TIPS OF HIS FINGERS AG AINST THE FLOOR.
WHILE OPERATING A WATER TRUCK UP A RAMP IN THE QUARRY, THE EMPLOYEE ""BLACKED-OUT"", CAUSING THE TRUCK TO CLIMB OVER THE BERM, DESCEND A 25-FOOT HIGH EMBANKMENT AND OVERTURN ONTO ITS SIDE ON A H AUL ROAD BELOW.
WHILE STANDING ON THE PLATFORM STAIRWAY AT THE WATER SHANTY, THE EE TURNED TO TALK TO A CO WORKER AND SLIPPED AND FELL DOWNT HE STAIRS.
WHILE ATTEMPTING TO GREASE #8 MILL, THE STEP THAT THE EMPLOYEE WAS STANDING ON TILTED, CAUSING HIM TO FALL ONTO HIS BACK.
WHILE ASSISTING IN PULLING A SIDE PLATE FROM THE OUTSIDE OF THE PRIMARY CRUSHER, A CO-WORKER WHO WAS POSITIONED ON THE OTHER SIDE OF THE PLATE, SWUNG A SLEDGE HAMMER TO LOOSED THE PLATE AND ST RUCK THE EMPLOYEE'S FINGER.
AS THE EMPLOYEE WAS SLIDING A STEEL PLATE FROM THE TOP OF TWO BEAMS ON A 100 TON PRESS WITH ONE HAND, THE PLATE FELL BETWEEN THE BEAMS PINCHING HIS FINGER IN BETWEEN THE HANDLE ON THE PLATE AND ONE OF THE BEAMS.
WHILE DISMOUNTING A FORKLIFT THAT WAS PARKED OUTSIDE OF THE SHOP IN A DIM VIEW AREA, THE EE STEPPED ONTO A 4"" X 6"" WOODEN BLOCK THAT WAS STICKING OUT FROM UNDERNEATH A STORAGE TRAILER AND TWIS TED HIS FOOT.
