Employee's were attempting to empty a small metal scrap bin into a dumpster. They were using an overhead hoist to raise the bin. Upon turning the bin over to empty it out, one employee got hand pinched between the bin and dumpster, causing a contusion to right hand.

Employee was pulling a conveyor belt with a skid steer when the clamp slipped off the belt and struck EE in the right knee.

The Hired Hauler was tarping his truck after being loaded, the handle broken causing him to lose his balance. The driver fell (same level) bumped his head. This is not an Martin Limestone employee and some of the information is not known.

The EE called his supervisor to complain of chest pains. The supervisor immediately told the EE to shut down the equipment he was operating and that he (the super) was on his way. The EE was finally convinced to go to the hospital after speaking with his supervisor and his physician. He is currently treating for hypertension.
As the EE was stepping off of the service truck he twisted his L ankle on uneven ground.

The EE was exiting the track hoe when the door blew shut causing a minor wound the his R leg just below the knee area. The injury exacerbated an existing condition causing infection. The employee didn't tell the site supervisor until 3/15 and it was not reported until 3/16.

The employee was providing assistance in fabricating a work stand in the Shop. As he was carrying a section of steel grating and attempting to lift it onto the stand, the piece of steel slipped from the employee's grip and caught his finger against the edge of the work stand.

After loading his truck from the overhead bins, the employee noticed stones leaking out from the back of his bin dump truck. He began removing the material that was preventing the tailgate from completely closing with his fingers. As he cleared the material, the tailgate suddenly shut and pinched his fingers against the dump bed.

WHILE OPERATING A WATER TRUCK UP A RAMP IN THE QUARRY, THE EMPLOYEE ""BLACKED-OUT"", CAUSING THE TRUCK TO CLIMB OVER THE BERM, DESCEND A 25-FOOT HIGH EMBANKMENT AND OVERTURN ONTO ITS SIDE ON A H AUL ROAD BELOW.
