While working at the primary crusher doing maintenance employee stepped on a bolt and rolled their right ankle.

Over-exertion in lifting objects

The employee and another employee were carrying a new screen up to the building. When they got to the top of the catwalk and stopped for a break employee noticed an uncomfortable feeling in employee's abdomen.

EE was attempting to get a piece of steel from the steel rack. EE had moved 2 pcs of channel to get to the pc of steel EE wanted & when EE moved the 2nd pc of channel the 1st pc of channel came back on finger in turn lacerating ring finger on left hand which required 7 stitches.

Fall to the walkway or working surface

EE tripped on piece of angle iron sticking out from a pallet of parts by the old scale house after he returned a shove. He fell face down catching himself on his hands and rolled on the ground injuring his right arm. Wrist hurt a little and he shook it off. After about an hour he had pain and could not pick anything up. Took him for X-rays, small compound fracture around elbow.

Employee had greased drill rod threads, then reached in to remove dirt from table as Driller was moving drill rod carousel to drill rod catching employees right hand and forearm between drill rod and drill carousel causing bruising of forearm and laceration of right hand.

Struck against a moving object

Injured employee was operating a quarry haul truck. For a reason not yet known the truck engine stalled on an incline. The truck rolled backwards onto a roadside berm and overturned on the passenger side. As a result the injured worker suffered a dislocated shoulder.

Employee was shoveling waste material under a conveyor when he felt pain in his right hip as he worked. chiropractic treatment was sought at first by didn't help. lost time began after treating with a physician who took EE out of work on October 27.

Over-exertion in lifting objects

EE was lifting an empty oxygen bottle off of the welder truck when he felt pain in his right shoulder. Initially this was reported as first aid however after testing was performed it was determined that surgery was required. EE surgery is scheduled for 12/15/2010.
