Employee lost balance and fell while moving steel drill rod on bench. Drill rod fell on EE.

Employee struck by piece of angle iron causing a laceration to the scalp requiring medical treatment offsite for wound closure.

While hammering a bucket guard on a loader a small piece of metal broke off and went into the employee's leg.

Employee was working on power steering box on a Mack haul truck; the box swung down and pinched his left wrist/arm, causing a laceration.

The injured worker was assisting in the removal of a snubber pulley from a conveyor frame. As the snubber pulley was being lifted the IW's hand became momentarily pinched between the pulley and the conveyor frame. The IW suffered a broken finger and severe laceration. The IW returned to work with restrictions.

EE started shift @ 6:30 am & was working w/co-worker. At about 6:50 am, EE carried a come-a-long weighing about 20#. Immediately afterwards, EE commented he may have pulled a muscle in his back. He continued working, incl. walking, carrying tools & picking up 80# angle iron (@ 8:30 am). When he returned to co-worker & handed him 15# bearing he said he felt sharp pain in his back

Employee was exiting his personal vehicle, standing by car door, reached in to grab lunch box from car seat and his right knee popped. resulting in a right knee acl tear.

Pocket knife in hand, cutting tie strap, knife slipped cutting left hand between thumb and index finger.

Lightening strike caused fire to office building.

EE WAS USING A SHOVEL TO CHIP AWAY AT A PIECE OF PVC PIPE STANDING OUT OF THE BOREHOLE. WHEN EE STRUCK THE PIPE, A SHARP FRAGMENT HIT INJURED EE IN THE EYE.

TWO EMPLOYEES WERE REMOVING A 4'X 8' 1/16"" THICKSTEEL PLATE. ONE WAS SUPPORTING A NUMBER OF OTHER SIMILAR PLATES, BALANCING THESE PLATES BUT THEWEIGHT OF THE PLATES OVERCAME HIS STRENGTH TO SU PPORT THEM. THEY WENT OVER THE CENTER PUSHING HIM BACKWARDS. THE PLATES FELL ON HIS KNEES AND LEGS.

WHILE TRYING REPOSITION ONE OF THE SPRING LOADED RAMS WE USED TO ADJUST OUR GYRO CRUSHER, MY RIGHT HAND SLIPPED OFF THE RAM & I COULD NOT HOLD THE RAM WITH JUST MY LEFT HAND. SUBSEQUENTLY, THE FORCE DROP MY HAND, PINCHING IT BETWEEN THE RAM & THE CRUSHER ADJUSTING RING. THE FORCE FORCE OF WHICH BROKE MY FINGER.
