Over-exertion (Not Elsewhere Classified)

EE drove a stake body truck down to level 12. EE exited the vehicle and then reached back in to turn off a strobe light. While reaching, EE stepped in a 6"" hole and tweaked EE's back. Appears to of strained a muscle. Upon clinic visit, EE was deemed unable to currently work. Missed workdays and likely light duty ahead.

Caught in, under or between a moving and a stationary object

Worker was positioning 10-inch plastic pipe into a 412 Fusion machine to be fused to another piece of pipe. On doing so, EE pinched EE's left pinky finger between the pipe causing a fracture to EE's knuckle.

Miner was injured on 10/7/2019 and continued working until 12/18/2019 when miner's doctor put miner off of work. Miner was climbing down a ladder of a haul truck when miner stepped on a rock and hurt knee.

WHILE TWO EMPLOYEES WERE WORKING OUT OF THE MAN BASKET EE'S HAND BECAME PINCHED BETWEEN THE CONVEYOR SLED AND THE MAN LIFT HANDRAIL. THIS MOTION REMOVED THE SKIN FROM THE TOP OF EE'S HAND TO EE'S FINGERS. EE WAS TRANSPORTED BY AMBULANCE TO CROZER HOSPITAL WHERE EE HAD SURGERY ON THE HAND

Employee was under shaker holding the nuts for another Employee using an impact gun to tighten when pieces of dirt fell and went into the right eye causing it to irritate the eye

Employee went to sit down on a guard and it gave out causing employee to hit their head and elbow on the wall behind them.

Employee climbing up machine put fuel nozzle in and began fueling the 988f. EE climbed back down to check machine over it auto shut off engaged when full. EE climbed back up to top fuel off as EE was topping off, EE slipped off step landed directly on feet and felt right knee pop.

Employee states EE slipped on ice on EE's right elbow near restrooms across from garage resulting in pain in EE's right elbow

Employee twisted knee exiting conveyor head box. Employee was treated at Riddle Hospital where he was diagnosed with a right knee sprain and discharged with instructions of ""No weight bearing on right leg until released"".

While descending the stairs in the tertiary plant one of the treads broke free causing the ee to sprain his right knee. The employee was treated and returned to restricted duty.

Over-exertion in lifting objects

IE was assisting a fellow EE in the removal of a water pump. It was being lifted with the aid of a come-a-long. It appears there was a mechanical failure in the lifting device that caused the pump to drop suddenly. When the pump dropped it jerked his arm as he was holding onto the chain guiding the pump. This caused a strain to his right arm. He returned to modified duty.

Employee was exiting the welding truck. As he stepped on the running board the bracket broke. The sudden drop caused him to slip and jerk his lower back. He was treated for a lower back strain and returned to restricted duty.

The injured employee was struck by a horizontal support beam mounted on the metal decking as it was being removed at the primary crusher. The metal decking was initial struck and broke free shifting the weight.

EE was walking out of the electric house for the plant slipped after taking step to the outside.EE was diag. with a lower back sprain and told to stay out of work for a few days by the treating phys.

While installing speed reducer on head pulley conveyor 26, EE pushed the speed reducer onto shaft. Injured had his hand on back side wiggling the reducer for it to slide onto shaft. When it shot on, EE's right hand fifth finger got smashed between the speed reducer and a piece of angle.

After cutting bushing off of shaft of speed reducer, he leaned over to look at the work, and while holding torch in left hand inadvertently moved torch over his lower right arm causing burn.

EMPLOYEE WAS ROLLING 55 GALLON DRUMS OF OIL OUT OF THE WAY. THE FIRST THREE DRUMS WERE HALF FULL AND MOVED EASILY. THE FOURTH DRUM WAS COMPLETELY FULL, AND WHEN HE TRIED TO MOVE IT HE FELT A PAIN IN HIS GROIN AREA.
