Struck by falling object

Employee was cutting a piece of corroded angle iron with a spear like point while inside of a chute at the wash plant. The employee tapped on the cut piece of angle iron with a hammer causing the entire 8-foot piece to come loose, falling and puncturing the top of EE's left foot a quarter inch above EE's steel toe boot.

Employee was changing screens on wash plant and when he tried to put screen over the rail part of the screen cut his wrist.

EE finished end of day mixer washout and slipped on ice and hard packed. He broke his fall with his right arm outstretched.

Employee was lifting steel C Channel (@75 lbs) up to a saw to cut down for a sign. He strained his back but waited until 8/3/10 to seek medical attention. Possible disc herniation.

Employee was cutting I-beam from primary crusher frame and it dropped and struck him in the neck and chest, bruising him.

Employee was kneeling on stones cleaning around a conveyor, when a stone punctured skin on his right knee. Employee did not want to seek medical treatment at this time. About a week later employees right leg was badly swollen. He went to the doctor and was told he has cellulitis in his right leg due to an infected puncture wound on his right knee.

THE EE SUSTAINED PAIN INHIS RIGHT SHOULDER FROM THE REPETITIVE LIFTING OF A HOSE (~45 LBS.) ON AND OFF WATER TRUCK.

PLANT FOREMAN AND ANOTHER EE WERE WORKING ON A SCREEN TO FIX BEARING. AS FOREMAN WAS REMOVING COUNTERWEIGHT COVERING BEARING, IT SLIPPED OFF PINCHING HIS RIGHT RING AND LITTLE FINGERS.

EE SLIPPED FROM CATWALK OF HAUL TRUCK & CAUGHT HIMSELFT BY HOLDING RIGHT ARM ONTO HANDRAIL. HE REPORTED INCIDENT BUT DID NOT SEEK TREATMENT UNTIL 1/8/01. HE HAS NOT MISSED WORK BUT WILL UNDER GO THERAPY FOR SHOULDER STRAIN & HAS LIFTING RESTRICTIONS FOR APPROXIMATELY 1 MONTH.

EMP WAS KNEELING UNDER DUMP TRK. HE PICKED UP PUMP (APPROX 75 LBS) AND LIFTED OVERHEAD TO INSTALL ON TRK. WEIGHT OF PUMP FORCED EMP TO LOWER AND RE-LIFT. STRAINED BACK DURING RE-LIFT.

EMP DRIVING R35#1 FROM SHOT ROCK AREA. DRIVE SHAFT BROKE ON TRK, CAUSING IT TO PICK UP SPEED. EMP PANICED AND JUMPED FROM MOVING TRUCK.
