Employee was in Pit #3 of Plymouth Meeting Quarry to take pictures. EE was walking back to EE's vehicle and stepped on a rock while crossing 6' wide mud wash. The rock gave way, and the employee fell sideways. EE struck the left side of EE's head on a rock. Left ear was lacerated requiring stitches.

The employee was assigned to clean the magnet off one of the belts. Employee was using the JLG manlift to complete the task. Once the magnet was cleaned off, they lowered boom to ground and exited the basket. They were carrying bucket of tramp steel as they stepped out, they twisted their left ankle. Resulting in un-displaced fracture of left leg fibula.
Emp strapped a starter on a flatbed p/up & felt the need to climb onto flatbed to secure strap. Emp did this task with a co-worker. When done w/tie-down, emp went to the rear & went down forward on a step bumper, w/out handholds and missed the step and fell on side with right elbow, forearm and hip taking the impact

Shop mechanic was changing oil filters on an Cat 775f in the shop. in an effort to loosen the filter with a ratchet wrench, they were pulling on the wrench when it slip off the filter and came toward them hitting them in the right side of the forehead.

Checking the coolant level on the 982M. To do this the hood is raised on the machine and the employee climbs onto the rear step and visually inspects the coolant tank. EE did so and while climbing down from the step EE slipped, landing on the ground on their feet. EE then moved backwards, tripping over a wheel chock on the ground and landed on top of the wheel chock on their back.
EE was washing out under the secondary crusher with a hose. As EE was doing this, a large rock came off the secondary screen and struck EE in the right arm causing a fracture to the shaft of the right radius.
Employee was bolting wear plates into a chute. When they stood up they felt a pop in right knee. EE told supervisor they would be fine. Came in next night and worked until about 11 pm, and told supervisor they wanted to get the knee looked at because it was still swollen. EE went clinic to be seen.
While cutting I-beams, 1 emp. cutting had IE hold the beam as EE cut the bolts & letting the beam fall away. The beam came loose, IE pushed the beam & lost balance falling w/ the beam. They cut the beam loose, & partner said, LET IT GO. Losing balance, fell w/ the beam. IE hit knee on the tail support for the tail pulley conveyor. Laceration of R Knee.

employee was removing the head nut from a cylinder as the nut came loose, it dropped on the workbench pinching the employees distal phalanx of the left middle finger between the bench and the head nut resulting in a minor fracture of the finger tip.

employee went to get a tool while working with others on screen performing maintenance. EE didn't return quickly and co-workers went to look for EE. EE was found on the catwalk disoriented, not knowing what happened and not knowing EE's name. an ambulance was called.
Twisted quickly while carrying 50 lb. bagged ANFO resulting in sprained spine.
On February 9th at approximately 10 AM, Employee was plugging in the hydraulic press in the shop at Plymouth in order to press out the ""dog bone bushings"" from a CAT 740 Articulated Truck. When the employee turned the plug into the outlet they received a shock. Individual was taken to urgent to be checked out.

attempting to reach a float on the slurry tank, the emp. stretched across 2 beams w/out a harness & not able to hook into an anchor point. In the act lost his grip & fell to the ground, sprained his back & a few contusions due to the fall. Taken to the clinic, released on light duty & given percocet for pain. X-rays were taken. the employee will report to work tomorrow morning.
Employee was putting a cutting edge on a loader bucket. Cutting edge slipped off floor jack, and pinched employee's fingers between cutting edge and the floor. Employee has a broken middle finger on left hand.
Employee was washing out the tail pulley of the secondary discharge belt when a 6"" rock fell off the secondary screen and hit his left thigh. Employee waited till 5 PM to tell his supervisor what had happened. Employee was sent to a medical faculty for evaluation. He was told that he had a contusion on his left thigh.

Employee was cleaning debris from catwalk when he felt a pop in back. Then mentioned he may have caused this injury previously on June 21st at 9:00 AM employee was moving parts and supplies into the new shop addition when he was walking backwards and tripped over a pallet, falling on his back. He did not report at the time. Employee returned to work with restrictions.

Employee was hammering shaft to remove bearing. Sliver of metal broke off shaft and struck employee on the wrist. Sliver embedded 1/8 inch into skin. Sliver was removed by a doctor.
Employee was removing the components of a flop gate at the discharge of a secondary screen. Supporting pin moved causing the flop gate to drop 6 to 8 inches. Employee attempted to catch the piece pinching it between the gate and side of a chute.
Miner was cleaning up a pile of build-up at a catwalk next to primary jaw crusher using a shovel. Employee felt twinge in his back that continued to worsen as shift progressed. Employee reported at next scheduled shift with severe pain. Employee suffered strained back, was given medication and therapy with lifting restrictions. Employee continued to work with restrictions.

Scalping screen overloaded causing blockage/spillage. Spillage of stone (1-2 inches, 1-2 lbs.) fell approx. 40 feet on to miner's left shoulder causing a contusion. Miner was at ground level. Employee was taken out of work by medical professionals.
Cold, icy conditions. Work areas were being de-iced. Employee was going to his haul truck when he slipped on a patch of ice and struck the back of his head. Employee was transported to hospital where he was diagnosed with a mild to moderate concussion.
Employee was coming into work walking on company parking lot. Conditions were dark and icy. Employee slipped on dark ice. Felt pain. Employee was taken to hospital for treatment. Employee was diagnosed with tear of lateral meniscus.

Employee is a quarry QA/QC technician. Employee was shoveling and picking up buckets of stone when he began suffering chest pains. Employee thought it was heart attack and left site to report to ER. Was diagnosed instead with a strain of the intracostal muscles of the right ribs. Employee missed next scheduled shift due to injury and later placed on modified duty.
Employee was cutting conveyor to size for installation using straight edge and utility knife. Utility knife slipped off straight edge and cut employee along right index finger of ungloved hand. Cut required 9 stitches to close.

Employee was moving pumps from a lower level of the quarry to a higher level due to heavy rain falls. Employee was wading in 2' of water to disconnect pump hoses and to rig pumps for the lift. Employee slipped on an unseen rock below the water. Fell to his right side. Attempted to stop his fall, his right arm suffering a right shoulder contusion.

He exited cab and saw a bulge forming in tire, turned to get away when tire burst. Was 10 to 15 feet away when tire burst forcing him to the ground breaking lower left leg. Another employee in area when tire blew suffered a ruptured eardrum.

EMPLOYEE WAS OFF LOADING 5 GALLON CONTAINERA FROM PICK-UP TRUCK. SLIPPED OFF TAILGATE AND FELL 2 1/2 FEET.
While installing R.F. mounted tire on wheel, the tire rim shifted and caught employee's finger between wheel and assembled tire.
Employee was picking up a crane outrigger pad. As he threw it up onto the crane deck, he slipped in the mud. The combination of slipping while throwing the outrigger pad strained his back muscles. Employee could barely move his back the next day and needed medical treatment.

EE WAS SAID TO HAVE HAD A MILD HEART ATTACK CAUSING HIM TO BLACK OUT. DURING THE BLACK OUT PERIOD, THE EE DROVE THE HAUL TRUCK THROUGH A ROCK BERM AND FLIPPING OVER 20 FEET BELOW THE HAUL ROAD . THE EE SUFFERED 2 FRACTURED NECK VERTABRA AND THREE BROKEN RIBS.
