Mechanic had parked EE's service truck and was exiting the cab. A gust of wind caught the door and slammed it into employees right side.

Employee fainted while spraying under plant with water hose. Employee was diagnosed with personal medical issues

Employee had water from spray bar drip on face and felt debris in eye.

Employee was walking from Primary tower and slipped on a rock and twisted knee.

Employee was installing a new hose under crusher CR-3. The hose became stuck and the employee pulled on the hose to free it. The hose broke loose and the employee lost his footing and fell back against the side of the crusher. Employee suffered back strain.

Employee was bending over using a torch, when he stood up he felt a pull in his back.

Employee was cutting off old liner plate from feeder when something popped from the welding flame and went behind his glasses embedding in his eye.

4 Employees were tightening nuts on crusher using 20 lb sledge. A metal piece splintered off and struck employee in the lower R abdomen.

Employee was welding wear strips to floor of primary feeder, took shield off and began to chip slag. A piece of slag got behind his safety glasses and into his left eye. It irritated his left eye.

Employee was checking spray bar, slipped on an incline and banged right leg on a bolt that was sticking out which caused a contusion on his right lower leg.

He was using a come-along to put steel plate in chute and developed pain in his right elbow.

Was removing a nitrogen cylinder for the ElJay crusher. Oil under pressure burst from the cylinder into his eye. Eye protection was being worn at the time.

Employee was changing the front tire of a water truck. While lifting the tire he felt a pain in his left bicep.

Employee bent over to get equipment out of gang box; wind blew lid onto his head. He sustained a laceration to the top of his head.
