Mechanic had parked EE's service truck and was exiting the cab. A gust of wind caught the door and slammed it into employees right side.

Employee was installing a new hose under crusher CR-3. The hose became stuck and the employee pulled on the hose to free it. The hose broke loose and the employee lost his footing and fell back against the side of the crusher. Employee suffered back strain.

4 Employees were tightening nuts on crusher using 20 lb sledge. A metal piece splintered off and struck employee in the lower R abdomen.

Employee was welding wear strips to floor of primary feeder, took shield off and began to chip slag. A piece of slag got behind his safety glasses and into his left eye. It irritated his left eye.

Employee was checking spray bar, slipped on an incline and banged right leg on a bolt that was sticking out which caused a contusion on his right lower leg.
