Finger caught under crane cable after cable slack was winched taut resulting in a laceration requiring 6 stitches. Returned to full duty; no restrictions.

Fall to the walkway or working surface

Miner exited work truck to begin drilling. Slipped and fell on ice/snow following snowfall.

Employee was moving a section of 12"" hose with the telehandler. They attempted to extend the boom out to place it down while on an uneven surface. The outriggers were not extended and the vehicle rolled onto its cab side. The employee was wearing a seatbelt but injured their leg from the impact of the rollover.

Employee was repositioning PPE when a small piece of metal went into right eye. After flushing eye, employee received medical treatment for a scratch to cornea.

While cleaning material around the access hatch on C1 Conveyor the employee lifted the hatch and twisted their back. Employee felt pain in their back but did not report it to management until 11/1/2021. Employee was taken to occupational medicine for evaluation and was later released back to work with restrictions.

Employee was cleaning between Screen 6 & 7 when employee attempted to move a metal plate on the ground. Employee felt a pop in their groin. Employee was taken to clinic for evaluation.

Employee attempted to remove a stuck safety chain on a crane block. The workers finger got caught in the block and resulted in a injury to EE's left hand.

Employee was welding on a metal strip which was caught in a feed chute of a crusher. The metal was cut free and sprung out hitting the employee and knocking the employee backwards.

Employee received a slight bruise to EE's right leg while moving a metal plate on top of a crusher.

A hydraulic hose on the employee's service truck outrigger got caught. The employee reached in to free the hose. EE began to retract the outrigger to gain slack in the hose and did not realize that a hand was in a pinch point. The outrigger broke several bones in the left hand.

While adjusting crusher setting employee tripped over cable strung between crusher assembly and loader. Employee fell onto walkway causing contusion on left knee.

Employee was attempting to rotate the rotor inside the primary crusher. EE said had heard a pop in back. It got stiff.

Employee was rushing out of the lunchroom to avoid clocking in late and struck head on the corner of the door, causing a laceration and bump.

Employee was climbing up a berm to check water flow and a rock shifted causing employee to lose footing and fell forward with hand extended causing a laceration between pinky and ring finger. Required sutures

EE was assisting on the repair of a broken water pipe under the secondary tower. After they were finished EE was walking under the tower and got foot caught in hoses causing EE to trip and fall resulting in a fracture in ankle of right leg. EE was taken to the ER.

Employee was taking a conveyer pulley off a pallet. EE squatted and bent down to lift one end up and roll it off the pallet. As EE did EE twisted slightly causing EE to have pain in the lower left side of EE's back.

Employee was cleaning a spray nozzle on S-17 while standing on the catwalk alongside screen frame. Employee's foot slipped on the wet surface of the catwalk and wedged between the catwalk and screen deck frame.

While ee was priming the fuel pump at tank 1 EE put fuel into the priming tube of the pump. Started the pump and went up on the truck to see if it primed. It didn't act like it primed so EE went back down turned the pump off and shut valves, when EE removed cap off priming tube diesel fuel shot out getting past EE's safety glasses and into EE's eyes and mouth.

Removing pipe flange from SP-5 rental pump, employee was holding flange so co-worker could get a bolt out when bolt came out flange fell to the ground pinching employee's left index finger between falling flange and flange on the ground.

While attempting to lift a cover plate to expose a broken hydraulic hose on a 980 Cat loader employee felt a sharp pain in his abdomen. He was treated for an abdomen strain & returned to restricted duty. He returned to his regular job as a mechanic. After additional testing it was determined that he has a hernia. He is OOW for hernia surgery as of June 18th.

Absorption of radiations, caustics, toxic and noxious substances

While the IW was inspecting a siphon line on a water pump an insect crawled up his shirt sleeve. The insect bit the IW on the shoulder. This incident was reported by the IW 3 days after it occurred. When the injury was reported it was still swollen and required medical attention due to an infection.

IE was standing on a conveyor catwalk when the conveyor frame broke. Sudden drop of the conveyor caused the employee to fall. As employee was falling he grabbed the handrail and strained his shoulder. Injury has been classified as first aid to this point and will be changed to lost time effective May 26, 2011 We discovered that the employee will require rotator cuff surgery.

Struck by flying object

The injured employee was removing a gear box from a stacking conveyor. As the IE was standing on the conveyor, the conveyor frame broke and the IE was struck in the right arm by a hand shovel that became airborne. The injured employee received 11 stitches to his upper right arm.

Fall from machine

Employee was assisting on blast crew as a driver & laborer. He was attempting to exit the rear of a straight van truck. He was using the rear door and door frame as hand holds & was reaching with foot to use ICC bumper as a step when his foot slipped. He fell to the ground injuring his right knee dan right shoulder.

EE was pushing a wheel barrel and was too close to an Ibeam. He bumped into it and hit his hand.

Employee reported on 3-1-2010 he felt pain in right groin while assisting in lifting a steel plate..he did not request medical treament at the time of report. 12 days later he did seek medical treament and was diagnosed with a hernia and scheduled for surgery on 3-26-2010 and estimated to be off work for 4 weeks.

Employee was climbing down from conveyor head pulley chute and slipped off of a shaft for the flop gate. His leg scraped against the shaft as he fell and scraped his leg causing lacerations.

Pain in the left thumb. Employee went to his own family doctor and then referred to the company's doctor. Doctor says it is trigger thumb from repeating motion by pulling on the handle to loader.

Employee was getting off bobcat when his right foot got caught between the frame of the bobcat and frame of the bucket causing injured ee to fall striking his knee on the top of the bucket, which caused a laceration to his right knee.

EE was helping co-worker with welding a plate for shaker 2 & something went into employee's left eye causing pain. He then went to Occupational Health & they referred him to eye doctor. He has a scratch on his left eye due to a foreign object. Must have flown into his eye. Eye protection was being worn at the time.

EE was washing off a concrete pad with a high pressure water hose, when hose broke and struck EE in the leg, causing injury.

While greasing a drum on a crane, employee slipped and lost his balance causing him to fall on his left leg which went between the dismount steps and the body of the crane while his body bent sideways toward the ground.
