Employee dismounted from cab of hauler and planted left foot at an angle resulting in foot strain.

Employee was helping to put counterweight on screen, weight slipped and lacerated palm of hand.

Finger caught between drill bit & drivers sub while changing bit. Required stitches & may have slight fracture. Treated at local hospital ER & released. Middle index finger required 7 stitches.

Driller was in process of cutting air hose to splice, grinding wheel on cutter flew apart and sliced into chest of operator. Cut deep but apparently no penetration.

Employee was cutting hinge on engine excavator cover and fell off

EE was using a wrench to loosening a bolt on a screen guard. He pulled a shoulder muscle while pulling on the wrench.

Miner was dismounting from cab of drill when the grab bar broke loose and he fell backward approximately 4 ft onto his bottom, back and head. His head struck the ground and causing a laceration and bruise.

Employee was determined to have a recordable hearing loss discovered during a routine audiometric exam.

Employee was trying to open a clogged chute and hit his right index finger on a piece of sharp metal, cutting finger.

Worker was spraying belt dressing on #67 belt. During the process of this operation the worker dropped the spray can. As a reflex he reached for the can as it was falling and pinched his right pinky finger between the belt and roller causing an injury to the pinky finger that required a skin graft to repair.

Employee was exiting a company pick-up truck when he twisted wrong and felt pain in his back. He strained his lower back.

EMPLOYEE WAS SPRAYING CONVEYER BELT WHEN SPILAGEON CATWALK SHIFTED UNDER HIS FOOT CAUSING HIM TO GET TOO CLOSE TO HEADWHEEL, LEFT INDEX FINGER GOT CAUGHT BETWEEN HEADWHEEL AND CONVEYER BELT, E E PULLED HIS FINGER.

CHECKING ANTIFREEZE IN THE ENGINE OF A FRONT ENDLOADER WHILE ENGINE WAS HOT, IT BOILED OVER AND BURNED LT. FOREARM (2ND DEGREE BURN).

WHILE DOING MAINTENANCE IN FEEDER EMPLOYEE CLIMBED DOWN THE METAL FRAME OF THE FEEDER AND TORE ROTATOR CUFF OF LEFT SHOULDER.

OPERATING WATER TRUCK; COMPLAINED OF NUMBNESS INRIGHT WRIST & HAND GETTING PROGRESSIVELY WORSE.

EMP PERFORMING REGULAR MAINTENANCE TO PLANT. DROPPED WRENCH INTO BIN FULL OF COARSE AGGREGATE. TWISTED KNEE ON MATERIAL AFTER GOING TO RETRIEVE.

EMP WAS CLIMBING ONTO TEMP CATWALK, LOST BALANCE AND FELL ONTO KNEE.
