EE was clearing material from a blocked chute. After removing wet material, the dry material behind it broke loose and slid down the chute causing some hydrated lime to pass underneath EE's safety glasses and into EE's left eye. After rinsing with eye wash and going to ER, EE was prescribed eye drops for scratch on left eye.

Over-exertion in lifting objects

A temporary EE stated that sharp pain was felt while working with a second EE to install new bags onto cages for a dust collector. The injury was reported at the end of the shift and the EE declined treatment. The next day, the EE stated that the pain was bad. The EE was seen at a doctor for lumbar pain, which resulted in work restrictions.

Employee was trying to unblock a frozen coal chute when a steel access plate jarred loose and slid down onto employee's left foot above the safety toe portion of the boot.

EE was walking outside office area heading to work area and dust entered left eye from outside of respirator when putting it on en route to work area. Eye was flushed but irritation continued visit to doctor resulted in precautionary antibiotic ointment.

At 09:35AM on 30 June employee called HR manager to notify he was not coming to work because he injured his back the day prior while operating a JLG. He made no mention of injury prior to this, so exact information is not known. He stated he went to the ER and received a note from his doctor not to go to work.

Employee was walking on North roadway adjacent Kiln 5 bag house when dust/particulate blew into left eye. Employee was wearing safety glasses at the time. Flushing with saline solution was not effective and employee sought medical attention. Examination revealed minor abrasions to cornea and topical antibiotic was prescribed. Employee returned to duty.

While working on a chain connector, dirt or lime dropped from above and got between the miners face and safety glasses and got in his eye. Repeated flushing did not completely relieve the pain, so miner when to care facility for evaluation. Care facility prescribed antibiotics for cornea abrasion.

Employee was cleaning a over flow from #2 packer bin on the floor at the base of the bin. Employee claims lime dust may have entered his boot at a separation in the laces and tongue. The lime material caused chemical burn to the employees 3rd and 4th toes on the left foot.

Employee removed a lid from a 55 gallon drum. The internal lip from the barrel had a sharp edge sticking out. The employee was moving the barrel and his hand slipped and his thumb was cut on the sharp edge.

Employee was working in 3-4 bag house welding brackets for lockout bar. When he moved to the other side to weld, he picked up welding lead and turned around striking his head on the duct. This caused employee neck Pain. Employee did not seek medical attention until 2/29/12. He was then placed on restrictions.

EE had to reposition himself to complete his job and as he did the lime dust was disturbed. The dust collector was running @ this time. The drafting of the fan inside the wizard and EE disturbing the lime caused the lime particles to be sucked up around and underneath EE's safety glasses and into his right eye.

Employee was manually adjusting the turn heads to back fill the stone bins. The employee failed to open the dampers to air the environment out. It became heavily dusty. The employee was required to climb some stairs and stated that he couldn't breathe with the dust mask on so he removed it a few times. Employee went to doctor and was diagnosed with bronchial spasms.

Employee was walking into a building when he slipped on wet lime and fell on his knees. Employee couldn't get out of his chair 6 hours later. 911 was called. Diagnosis was contusion to knees.

2/6/11 Employee was using an airhammer to unblock material from a hopper when his hand became caught between the roller and the belt. 3 minor cuts on thumb, pinky and index finger. Employee refused medical treatment at the time of incident. 2/9/11 employee requested to see a medical doctor. The diagnosis was 2 small fractures to his left pinky finger.

Injured was carrying a drinking water container up three stairs into the control room when he lost his footing and slipped.Incident occurred on 4/21 but employee did not request medical attention until 5/7/2010.

Employee was kiln raking for several hours when his right forearm felt numb and started to swell. The employee went to the local ER for evaluation and was diagnosed with an arm sprain. Light duty was given to the employee for the next 7 days.

While getting on the ladder of the rail car, the employee's foot slipped off the rung, causing the employee to fall approx 3 feet to the ground. The employee did have 3 points of contact but the fall caused the employee to land on the ground and hit his head against the rail. The rung of the ladder did have frost on it and contributed to the fall.

Employee received a lime in the eye and inhalation injury. The employee was working on a bearing change when the mill bin located above him over flowed and spilled material on top of him. Employee eyes were washed out and sent to the ED for evaluation.

Employee was clearing a clogged chute in the Hydrate plant when material got into his right eye causing irritation. Employee flushed out eye and sought medical attention. Employee was treated at the ED with sulfa drops. No lost time or work restrictions.

Contractor's driver finished unloading glycerol into mine's storage tank. Proceeded to unhook hose from storage tank and due to pressure build up, hose blew off of connection causing glycerol liquid to splash into driver's right eye forcing safety goggles off driver's head. Grit was in the liquid and scratched right eye cornea. Right eye is healable.

Employee was walking down the ramp (compacted stone) of the PPS plant, when he slipped on ice, twisting his right lower leg and ankle. The employee had immediate pain, so he sat down and called for help on the raido. Was placed into the ALPM vehcile and transported off of the ramp to the plant entrence. The ALPM called 911 for transport of the employee to the GSHED for evaluation.

EE was trouble shooting kiln 4 lime skip and was jogging the skip for testing purposes. He left the jog station, proceeded around the back of the skip area towards the north, and hit his left leg on a piece of expanded metal that was propped up against the structure. When this leg caught the expanded metal, the other leg caught the inlet pipe to the air compressor.

Employee pinched his fingers between the steel frame of the multimul and the pry bar the employee was using to clear a product buildup. The prybar got between the belt and return roller of the multimul. Employee suffered lacerations to his finger on both hands requiring 5 sutures to repair wounds.

Fall from ladders

EE was climbing down ladder and thought his foot was on a rung and it was not.EE lost grip on the ladder and fell onto a steel deck approximatly three feet below. Employee reported right knee pain and did not report the injury untill 14:30 hrs. Employee was sent to the Good Samaritian Hospital in Lebanon, PA for evaluation.

Employee was air arcing on the 3rd pier of kiln 4. He was standing on top of the concrete pier 3 feet off of the decking when he lost his balance & fell on to the safety railing. The employee struck his chest on the railing causing 4 fractured ribs. EMS was called for treatment and transport to Hershey Medical Center.

While unjamming the multi-mull, a water hose the employee was using knocked into the multi-mull lid. The lid was already open allowing the hot air and lime dust to escape vertically. Once the lid was moved, the hot air and lime was redirected towards the employee causing a chemical/ thermal burn to his face. The employee suffered 2nd degree burns on his nose and left cheek.
