Caught in, under or between a moving and a stationary object

Employee was using a personal knife to cut off a piece of tread from their boot. When the employee attempted to close the knife, they closed it on their left middle finger.

Fall onto or against objects

The employee was working on junction box. EE proceeded to step up and out of the hole leading with EE's left leg. EE's foot slipped out from under EE and EE's leg contacted the lip of the hole and the ground. Employee did not seek medical attention 6/29/25. Employee suffered a possible left fibular fracture.

EE foot became tangled in a power cord and EE lost balance and fell. EE caught self with left hand on the table and left ring finger bent backwards against the pulverizer machine. EE then fell to knees after EE's hand took the brunt of the fall. Did not seek medical treatment until 12/15/2024

Over-exertion in lifting objects

Employee was in the process of moving a 6' x 4' wire screen. Employee lifted the first screen to move it out of the way to get to the next screen. While lifting the screen and turning employee felt pain in left arm.

Struck by flying object

IW was grinding on W13 drip pan on 8/14/23. At some point between 9:00AM and 12:00PM they felt something in their right eye. They washed it out on the night of 8/14/23. Employee did not seek medical treatment until 8/16/2023.

While cleaning under the conveyor belt, the employee's arm and shoulder contacted the conveyor frame, causing a skin tear on the left elbow and acute pain in left shoulder.

Employee was dragging an empty fire hose across the plant floor when EE felt a sharp stinging pain in EE's abdominal area. EE was able to work full duty until surgery 2/22/2022.

Employee was using a fixed blade razor knife to cut and prepare conveyor belt for attachment to pulling cable. The blade slipped from contact with the belt and cut the employees inside right forearm requiring stitches.

EE was flipping a mantle with a bridge crane and strap when the strap broke causing the mantle to fall to the floor injuring employee.

Two employees were replacing the end plate of W-13 Conveyor Discharge Chute by lifting it into place. While putting plate into place, they lost conrol of the plate and it fell striking injured on the right index finger causing laceration. (Plate wears approximately 150 lbs.)

INDEPENDENT HAULER WAS REMOVING BOWS FROM HIS TRUCK BED IN PREPARATION OF LOADING. HE CLIMBED STEPS ON THE SIDE OF THE TRAILER & ATTEMPTED TO SWING TRAILER BOW WHEN HE SLIPPED & FELL TO THE GROUND.

Employee was cleaning spilled material (-1/4"" size) from tail pulley area of W-18 Conveyor. He had been shoveling approximately 20 minutes when the accident occurred. He was working at a normal pace and was not overexerting himself. Lower back strain - lumbar root syndrome

EE WAS TROUBLESHOOTING AN ELECTRICAL PROBLEM WITH A CONVEYOR'S TAIL PULLEY. A CO-WORKER WAS STANDING ON THE TAIL PULLEY'S FRAME & HAD STARTED TO REMOVE BOLTS SECURING THE SWITCH COVER. INJURED POSITIONED AN 8' STEPLADDER AGAINST THE TAIL PULLEY GUARD & STARTED TO ASCEND THE LADDER. THE BOTTOM OF STEPLADDER KICKED OUT & HE FELL ONTO THE UPPER FRAME OF THE GUARD, FRACTURING RIBS.

EE WAS TAKING A SAMPLE FROM W-10 CONVEYOR SAMPLETAKER. EE STATED HE PULLED HARD DUE TO THE SCOOPSOMETIMES STICKING. WHEN SCOOP CAME OUT OF HOLDER, THE WEIGHT OF THE SAMPLE DROPPED AT WHICH TIM E THE EE GRABBED TO CATCH THE SAMPLE CAUSING PAIN IN LOWER BACK. SAMPLE WEIGHT FULL SCOOP IS 45 LBS.

EE WAS ORGANIZING STEEL BARS ON STORAGE RACK. ASHE WAS PUSHING BARS BACK INTO THE RACK HE LOST HIS FOOTING DUE TO THE BANK HE WAS STANDING ON. WHILE TRYING TO REGAIN HIS BALANCE HIS RIGHT FOOT LANDED ON AN INCLINED PLANE CAUSING HIS CALF MUSCLE TO BE STRAINED.

EE WAS ASSISTING ANOTHER EE INSTALL A TROUGHING ROLLER. WHILE DOING SO EE PINCHED HIS SECOND FINGER ON HIS RIGHT HAND BETWEEN TWO ROLLERS. AT THAT TIME EE LOST CONCIOUSNESS FOR APPROX 30 TO 45 SECONDS. EE SUSTAINED A BRUISE TO BACK OF HIS HEAD AND WAS TRANSPORTED TO HOSPITAL. EE WAS RELEASED AFTER BRIEF OBSERVATION ADN RETURNED TO WORK THE NEXT DAY AS SCHEDULED.

EMPLOYEE WAS TAKING INVENTORY. HE USED HIS FOOT TO PUSH A HEAVY DRAWER BACK INTO PLACE. AT THAT TIME HE EXPERIENCED PAIN IN HIS STOMACH AREA. HEREMAINED AT WORK BUT HAS EXPERIENCED DISCOMFORT SINCE THEN. UPON HIS VISIT TO THE DOCTOR IT WAS DISCOVERED THAT HE HAD A DOUBLE HERNIA & IS DUE TO HAVE SURGERY ON 10/6 & IS EXPECTED TO BE OUT OF WORK FOR 6 WEEKS.

EMPLOYEE AND COWORKER WERE EMPTYING METAL BINS IN SHOP. COWORKER WAS USING A PRYBAR TO UNWEDGE BIN HANDLE WHEN IT SLIPPED AND STRUCK INJURED EMPLOYEE IN THE ARM.
