While unclipping the chain when another employee batched the boom up wedging the employee's thumb between the chain and the shaft.
Struck against stationary object
While moving wood pallet, and it dropped to the ground, which caused the pallet to break. While picking up the pallet again, EE's right foot stepped on several protruding nails.
