BACKHOE OEPRATOR WAS DIGGING A DITCH THE LOADER OPERATOR WAS STANDING NEXT TOTHE BACKHOE. AS THE BUCKET WAS SWINGING, THE FOOT PEDAL STUCK CAUSING THE BUCKET TO SWING TOWARDS THE VICTIM (LOAD ER OEPRATOR). VICTIM BACKED AWAY AND PUT OTU HANDS AND PUSHED AGAINST THE BUCKET, VICTIM FELL BACK STRIKING HIS HEAD ON THE GROUND.
Stepping onto the steps to the cab of the haul truck and felt a pop in the groin.
While changing screens on the second finish screen, EE rolled out of the bottom duck and when EE applied pressure to stand up EE felt a pop in EE's knee followed by pain.
Does not know what happened, EE's foot just started to hurt sometime in the afternoon.
Employee was putting in troughing rolls, return rollers, and holding up the belt when they felt discomfort.
Employee was wrenching on a tail pulley when the wrench got stuck. They decided to use more body weight resulting in the wrench falling and hitting their shoulder.
Designated parking place had ice and was uneven
Little tiny holes in lungs and air pipe - PNEUMOMEDIASTINUM
Pushing belt clamp to set it over roll and felt a strain in their back.
Completed screen change. Moved down to change gate configuration. Another employee attempted to close from the below the chute, unsuccessfully. EE began clearing material from the outside of chute with a 6' breaker bar. When the material loosened and sloughed off, EE was still throwing the bar. Which resulted in EE's left arm overextending and striking the divider wall.
EE initially said it was related to haul road and it is rough. EE later clarified that on 06/24 EE was driving the road and it was bumpy and the air seat bottomed out. EE stated was slammed to the bottom causing back pain. EE noted was worried about a compression type injury. EE did not seek treatment or report until 8/27/2020.
Was doing maintenance on the crusher. Taking the drive chain off the hopper and the guard tipped over and hit the top of head and hurt neck. EE was wearing a hard hat.
Fell 10 feet while trying to apply pressure to a come along and the handle came off. Left wrist/shoulder sprain and back sprain.
At the end of the day went to get out of truck and foot missed the step and fell all the way to the ground and landed on back.
Cutting a piece of wood with a pocket knife and cut right thumb. Received sutures at Silverton Hospital.
Hit back of head while operating a Terex Haul Truck after it stopped on unstable ground and the back end sunk down and truck tipped back. Didn't seek or receive treatment until 8/01/17 after having head aches and neck pain. Treatment was received at Back to Health Clinic in Keizer
Was using a grinder on tailgate and the grinder got caught and jerked back into nose. Nostril was cut and received sutures at Salem Clinic Urgent Care.
Was exiting the loader and foot slipped off step and ring on finger got caught on the step while falling. Went to Urgent Care in Keizer, OR.
Was checking the tailgate on truck and it closed and smashed finger. Went to Salem Hospital.
Pulled muscle after removing tight bolts all day with ratchets and wrenches.
Right little finger was pinched between a chain and backhoe bucket while trying to release the come along from the chain that was securing a concrete block in the bucket. The block tipped forward tightening the chain and pinching the finger.
Strain to back while pulling and lifting pipe onto pump float.
EE was backing down a ramp in a loader and ran over wet material that had slid across his road. The impact jarred his back and that caused a back strain.
Employee was replacing track pad on the excavator and when he went to put it on it slipped down and fell on both feet. He broke his left foot and a toe on his right foot.
Employee was starting brush fire and gas fumes ignited.
Employee is allergic to bee stings and was stung in the right arm.
Standing on track of machine and slipped off and fell on left shoulder.
Driving off road truck, the truck jerked several time and twisted his back.
Changing impellers, twisted back while lifting and turning.
Worker was welding in a chute, positioned on his side. A piece of hot slag entered his helmet and hit him in the right eye. It burned his eye lid and his eye.
Employee was adjusting the position of an iron chute on a pair of sawhorses when the sawhorse moved and the chute fell against the employee's knee. The weight caused his knee to give out.
Worker was cutting metal with a cutting torch when the hot metal popped and went into his left ear. Worker wiped his ear and continued to work.
EE WAS LIFTING A SCRAPPER FROM A SAND JUMP BELT WHEN HE PULLED SOME MUSCLES IN HIS BACK. HE DID NOT FEEL THE PULL AND WORKED THE REMAINDER OF HIS SHIFT. HIS BACK TIGHTENED UP THAT NIGHT AND TH AT WAS WHEN HE WAS AWARE THAT HE HAD HURT IT.
THE WORKER WAS CARRYING A PIECE OF STEEL UP A LADDER WHEN HE FELT A MUSCLE PULL IN HIS BACK.
AFTER CHECKING OUT MACHINE, THE EE SHUT THE ENGINE COVER DOOR, BUT IT DID NOT SHUT TIGHTLY. SO HE OPENED IT AND SHUT IT AGAIN, BUT DID NOT GET HIS LT INDEX FINGER OUT OF THE WAY OF THE DOOR, & IT SHUT ON IT BREAKING THE END OF IT.
THE EE WAS WASHING ROCKS OFF OF THE TOP OF THE CAB OF HIS PIT TRUCK WHEN HE SLIPPED & FELL ON THE HOOD, LANDING ON HIS ELBOW (LEFT).
EE WAS CUTTING PIECE OF RUBBER WHEN UTILITY KNIFE WENT THROUGH THE RUBBER, WENT INTO EE'S LEG.
