BACKHOE OEPRATOR WAS DIGGING A DITCH THE LOADER OPERATOR WAS STANDING NEXT TOTHE BACKHOE. AS THE BUCKET WAS SWINGING, THE FOOT PEDAL STUCK CAUSING THE BUCKET TO SWING TOWARDS THE VICTIM (LOAD ER OEPRATOR). VICTIM BACKED AWAY AND PUT OTU HANDS AND PUSHED AGAINST THE BUCKET, VICTIM FELL BACK STRIKING HIS HEAD ON THE GROUND.

Struck against stationary object

Completed screen change. Moved down to change gate configuration. Another employee attempted to close from the below the chute, unsuccessfully. EE began clearing material from the outside of chute with a 6' breaker bar. When the material loosened and sloughed off, EE was still throwing the bar. Which resulted in EE's left arm overextending and striking the divider wall.

EE initially said it was related to haul road and it is rough. EE later clarified that on 06/24 EE was driving the road and it was bumpy and the air seat bottomed out. EE stated was slammed to the bottom causing back pain. EE noted was worried about a compression type injury. EE did not seek treatment or report until 8/27/2020.

Was doing maintenance on the crusher. Taking the drive chain off the hopper and the guard tipped over and hit the top of head and hurt neck. EE was wearing a hard hat.

Fell 10 feet while trying to apply pressure to a come along and the handle came off. Left wrist/shoulder sprain and back sprain.

Cutting a piece of wood with a pocket knife and cut right thumb. Received sutures at Silverton Hospital.

Hit back of head while operating a Terex Haul Truck after it stopped on unstable ground and the back end sunk down and truck tipped back. Didn't seek or receive treatment until 8/01/17 after having head aches and neck pain. Treatment was received at Back to Health Clinic in Keizer

Was using a grinder on tailgate and the grinder got caught and jerked back into nose. Nostril was cut and received sutures at Salem Clinic Urgent Care.

Right little finger was pinched between a chain and backhoe bucket while trying to release the come along from the chain that was securing a concrete block in the bucket. The block tipped forward tightening the chain and pinching the finger.

EE was backing down a ramp in a loader and ran over wet material that had slid across his road. The impact jarred his back and that caused a back strain.

Employee was replacing track pad on the excavator and when he went to put it on it slipped down and fell on both feet. He broke his left foot and a toe on his right foot.

Worker was welding in a chute, positioned on his side. A piece of hot slag entered his helmet and hit him in the right eye. It burned his eye lid and his eye.

EE WAS LIFTING A SCRAPPER FROM A SAND JUMP BELT WHEN HE PULLED SOME MUSCLES IN HIS BACK. HE DID NOT FEEL THE PULL AND WORKED THE REMAINDER OF HIS SHIFT. HIS BACK TIGHTENED UP THAT NIGHT AND TH AT WAS WHEN HE WAS AWARE THAT HE HAD HURT IT.

AFTER CHECKING OUT MACHINE, THE EE SHUT THE ENGINE COVER DOOR, BUT IT DID NOT SHUT TIGHTLY. SO HE OPENED IT AND SHUT IT AGAIN, BUT DID NOT GET HIS LT INDEX FINGER OUT OF THE WAY OF THE DOOR, & IT SHUT ON IT BREAKING THE END OF IT.
