FELL FEEDING ROCK BOX. THE ROCK BOX WAS PLUGGED W/ A LARGE BOULDER & 3 EMPLOYEE'S ASSESSED THE PROBLEM. LOCKOUT CONVEYOR LINE, CO-WORKER AT JAW TOWER AND FATALY INJURED EE TRIED USING A PRY BAR TO FREE PLUG & HE CRAWLED INTO THE ROCK BOX TWO TIMES, ON THE SECOND TIME MATERIAL ENGULFED HIM.

A miner was using a pneumatic die-grinder to prepare the steel surface on a dump truck for welding when the miner felt something go into eye. The miner flushed eyes several times & finished shift. The next day the miner experienced discomfort in the eye & sought medical attention. A foreign object was removed from eye & miner was released to work with no restrictions.

Miner was positioning a 55-gallon drum of Hydraulic Oil when EE's hand slipped off and EE caught it on the lip of the drum, causing EE's left thumb to bend back and resulting in a sprain.
Employee was utilizing pliers to remove shims from shim-kit when the pliers slipped and the shim made contact with the employees right index finger, resulting in a laceration.
The maintenance crew was moving and placing a piece of HDPE pipe. In the process of setting the pipe, which had apparently caught on something, the pipe released, which catapulted the pipe into the employee, striking employee in the chest and pinning employee against a conveyor frame. Employee was diagnosed with fractured left clavicle/shoulder.

Miner was standing outside of the heavy equipment shop going through task training on a front end loader and was drinking a soda. A bee stung the employee on the lower lip. The miner had an adverse reaction to the bee sting and was transported to a local clinic where employee was treated for an allergic reaction to the sting.
Employee pulled a transmission which had been lowered and was resting on a jack with casters out from underneath an on-highway dump truck. Employee woke the next day stating EE was sore and sneezed at which time EE felt a ""pop"" in the left side of EE's abdomen. Employee sought medical attention and has a strain / possible minor rib fracture.
Employee was grinding on a conveyor belt splice. Employee finished EE's shift, and the next day reported pain in EE's right eye. Subsequently the employee was taken to the doctor, and a piece of metal shaving was removed from EE's right eye. Employee was wearing safety glasses, but was not wearing a face shield.

Employee was striking a guard with a sledge hammer and felt pain in his lower abdomen. Employee was diagnosed with a permumbical hernia. No medical treatment was given, and employee was released to full duty with the doctor noting the hernia would not need surgery.

Employee was throwing away trash from service truck. Employee stepped up onto ladder of dumpster and put right hand behind top rung of ladder. Employee threw away trash and stepped down with hand still behind top rung. Employee felt a pop in hand. Upon medical examination employee fractured ulna styloid and metacarpal on right hand.

EE was dismounting a CAT D9 Dozer after fueling and felt a burning in right shoulder.
EE was installing a gear on a motor shaft for a crusher, while tapping the gear with a hammer to align gear with keyway on shaft, a 3 millimeter fragment of the gear chipped off and hit EE in neck. EE received puncture wound to neck.

EE reported 10/31/13 that a personal injury was a work related injury. EE believed while helping lift 4X8 wire screen on LS1 screen that he strained his back/neck. The EE cannot attribute any instance during this task which caused the injury. EE stated he was sore the next morning, however he did not attribute his soreness to his current injury which has worsened.
The injury to the employee is a muscle strain in the right arm. The exact cause of the strain is unknown. The employee believes that it may be from operating the steering wheel on the Cat D-400 haul truck. The employee has been diagnosed with a muscle strain.

The incident took place in the pit at the dragline. Employees were upending drag cable. employee was setting socket by pulling cable through the beckets. While he was pulling he felt a pop in his right arm. Employee suffered torn right bicep.
Employee was pulling a 2"" water hose up onto a screen deck approximately 17 feet off the ground utilizing a tag line. As the employee was pulling the last 10 feet of hose up ee felt pain in his lower back which was diagnosed as a lumbar strain.

Stringing a conveyor belt onto a conveyor for vulcanization. Reached end of belt and while attempting to remove belting from the wood core the belt recoiled which caused a nail that was hammered through the belting to cut employee's forearm. Affected area was right forearm below the elbow 5"" cut.
It is not entirely clear how employee hurt his left knee. Several hours after working underneath the dragline, employee says his knee began to hurt and didn't think much about it. Two days later the employee reported to his supervisor that his knee was swollen and sore and wanted to seek medical attention.
EE was placing wear liner on conveyor belt and pinched the tip of his Rt. hand ring finger between the wear liner and troughing roller, which caused a small laceration and fracture on the tip of EE's finger.
EE pinched his rt. middle finger in between the top of a center pull bearing and the top of its slide frame while installing new bearing. Appropriate PPE was being worn including gloves. No safety rules were violated, modified duty was in place for two days (no forceful repetitive gripping, maintain bandages). Slight fracture to finger tip, and subungual hematoma.

The mobile crane had set the empty spreader bar on the trailer-on its edge. The crane operator was then given the sign to lower the hook, in order to disconnect the top rigging. As the hook was being lowered the spreader fell over and hit the right ankle - soft tissue injury (Note: incident occurred during the de-mobilization of the crane)
While unrolling a discharge hose, a retainer wire struck the worker's finger causing a laceration and bleeding.

Turned on blue pump about 1/2 throttle, then turned on water valve about 3/4 pressure. Hosed for about 10 minutes when nozzle separated from hose; the band on the hose slid down employee's hand cutting left hand between thumb and index finger.
Contusion to left knee climbing down into the engine well. Foot slipped and hit knee on metal part in well. Fell about 1 foot.
Standing on loose material so lost footing when pressure from water hose caused loss of balance. Dropped hose on foot hitting above steel toe of rubber boot.

Employee was installing screens in a confined area and struck his left arm with a 3# hammer.
Employee was trying to bend a 3/8"" piece of plate that had been heated in the middle by jumping up and down on it. The next morning the employee had some pain in his leg/knee but thought it would go away.
EE was replacing a return roller. When he lifted the roller he hit his head on the conveyor frame. He was not aware he was so close to the conveyor frame.
While repairing a guard on a conveyor, the socket slipped off the bolt and the ratchet struck employee's upper lip pushing tooth back.

THEN CRANE PULLED UP ON CHAIN LACERATING FINGERS.

EE WAS IN THE PROCESS OF REPAIRING A SCREEN ON CRUSHER. HE WENT DOWN A SET OF STAIRS TO CHECK THE OIL AND AS HE PROCEEDED BACK UP THE STAIRS HE SLIPPED AND FELL INJURING HIS KNEE/ANKLE. EE STA TED THAT HE HAD EXCESS GREASE ON HIS SHOES WHICHCONTRIBUTED.
EE WAS ATTEMPTING TO BLEED THE OMNI CRUSHER WHEN HE OPENED TH PLUG THE CRUSHER STILL HAD PRESSURE AND BLEW HYDRAULIC FLUID INTO HIS FACE AFFECTING HIS EYES.
