Running loader, feeding plant bouncing in loader repeatedly. Sought treatment for claim on 3/28.

Employee was experiencing shortness of breath due to a preexisting health condition. After arriving on site EE had increased symptoms. EMS was called and EE was taken to the hospital for treatment.

Employee was washing out under feeder when falling debris hit hard hat resulting in bump on head.

Employee was picking up tickets 200ft from the scale house when employee slipped on ice, braced fall with right arm and broke right wrist.

Recordable Standard Threshold Shift.

Employee jumped from a bank to ground below and strained hip.

Employee broke through rotten wood truck deck and received laceration/contusion to right knee. Stitches required to close wound.
