Fall down stairs

The employee was walking down a set of stairs and mis-stepped on the final step. After catching self on the handrails, EE landed with EE's knee in an awkward position. This resulted in a sprained right knee.

Struck by falling object

A falling piece of a mill internal struck the employee on EE's right foot. Bruising and swelling was localized to the foot with no fracturing.

Worker using a hoe to scrape mud and strained back. Worker reported injury to management on 03/27/2011. Worker did not leave work or ever miss any time from work related to the injury.

Removing center drive line on 980 wheel loader. Drive line came part way out and caught on frame. EE reached in to move drive line into a different position. Came loose, left hand, middle finger, was in pinch point area between frame and drive line.

A discharge chute backed up; worker went out to inspect the problem. He was going down a four-step ladder facing forward and slipped on spillage from the plugged chute and hit his ear on a bolt sticking out and cut his ear.

The area contained mud & water, an extension cord was draped over a bobcat bucket resulting in a trip hazard, it was raining, the worker was wearing a full body tyvek suit & 12"" rubber pullover boots. He felt the cord against his ankle as he was attempting to step over it & tried to swing his ankle over the cord & slipped & splashed mud & water into his eyes.

Fall from machine

WORKER WAS CLEANING THE TROUGH ON THE KILN ROOF. ROOF GAVE WAY AND THE WORKER FELL APPROXIMATELY 30 FEET TO THE GROUND. WORKER SUSTAINED BROKEN AND DISLOCATED FEMUR AND THREE BROKEN LUMBAR.

EMPLOYEE WAS HAVING TROUBLE WITH SCRAPPER CHAIN FLIGHTS STRIKING THE COVERS. HE CLIMBED UP ON THE KILN TO ADJUST THE COVERS. WHEN EXITING KILN HE JUMPED TO THE FLOOR & INJURED HIS KNEE WHEN HE LANDED ABRUPTLY. THE INJURY WAS DIAGNOSED AS A TORN MENISCUS. EE WENT IN FOR SURGERY ON 6/6/05.

EE WAS WALKING AND WENT TO STEP UP ONTO CONCRETE PAD (26"" OFF GROUND) WHEN HE SLIPPED, TURNING HIS ANKLE. RESULTED IN A HAIRLINE FRACTURE.

WORKER WAS USING A 5'IRON PRY BAR TO BREAK UP FROZEN MATERIAL. HIS FINGER HIT AGAINST THE RAILCAR RESULTING IN A LACERATION & SLIGHT FRACTURE OF THE TIP OF THE LEFT INDEX FINGER.

A MAINTENANCE MECHANIC WAS WORKING IN THE KILN DEPARTMENT. WORKERS WERE BRINGING IN A LARGE I-BEAM. A METAL FLAKE WAS DISLODGED FROM ABOVE. THE WORKER NOTIFIED MANAGEMENT THE NEXT DAY.

Absorption of radiations, caustics, toxic and noxious substances

The worker was working inside in the hydrate warehouse. He was stacking 35# bags of hydrated lime. A bag became caught at a juction where the bag comes off the belt line and onto a metal slide. The bag came open releasing material. The worker used a hand broom to sweep off the slide. The worker flung some hydrate into his eye.

PULLING CABLES FROM DOCK WITH A FORKLIFT. TIE OFF ROPE SNAGGED AND CAME LOOSE. CABLES CAME FORWARD ABRUPTLY AND ROPE TRIPPED WORKER AND HE FELL. WENT TO DOCTOR TO GET CHECKED OUT - NO INJURY F OUND.

EE WAS TURNING A WHEEL 30"" DIA TO TAKE THE SLACK OUT OF A CABLE. THE CABLE TOOK OFF AND WHEN HE TRIED TO PUT HIS FOOT ON THE BRAKE INACCUERTENTLY PUT IT INTO THE WHEEL.

WORKER WAS CLEANING A ROLLER WITH A BAR AND THE BAR WAS CAUGHT IN THE ROLLER AND IT SWUNG TO THE ICE AND HIT HIS LITTLE FINGER AGAINST THE GUARD AND BROKE HIS LITTLE FINGER.
