Roof Fall 5th South Panel entry 3 xc3. Maintenance EE found the fall around 1249am and reported to Management and they reported to the MSHA Immediate Accident number then went to get roof fall secured with timbers and dangered off and started investigation of fall. Fall was 18'x25'x10' hi bolted with 8' tr bolts, cable bolts, screened. Fall is secured.
Was working just inby, heard lots of noise. Boss went to see what all the noise was as soon as EE got there EE seen two rocks fall and the intersection fell at xc 18 entry 3 in the 5th South panel
Operator was driving a Kubota down gradient into the mine along the mine's #1 intake line. Per the operator the unit came out of gear to neutral causing the inability to make the turn, resulting in the Kubota impacting the forward embankment causing an injury to the passenger.

EMPLOYEE WAS REINSTALLING THE RUB RAIL ON THE MINER. RESTED THE RAIL ON THE FRAME IT WAS NOT SECURED AND SLIPPED OFF THE FRAME STRIKING THE EMPLOYEE ON THE TOP OF FOOT BREAKING 2 METATARSAL BONES ON RIGHT FOOT.

Employees were cribbing up a shuttle car to change a tire. The tire slid off the hub of the shuttle car, hit the ground and fell over on the employee breaking their leg in two places below the knee.
Employee was pushing wet coal into the shaker screens. A rock bounced off the shaker screen striking employee on right foot.
Employee was putting full plaster buckets through a man door at a stopping when ee left a pain in neck.
Employee was operating a shuttle car, pulled into the face behind the miner to get loaded, when a plastic pogo came flying off the conveyor of the miner striking a rock at the top of the pile in the car sending the rock into the cab of the shuttle car striking the employee in the ribs.
a roof fall occurred at approx. 0240 on 10-24-17 on the 4th south panel in the intersection of 17 xc and 2 entry. The fall measured approx. 50' long by 18' wide ranging from 5' to 10' in height

Ventilation was not affected, No injuries and no equipment involved.
A Roof Fall occurred in the intersection at 8 entry XC 11 the roof fall was approximately from 3-10 ft. tall above the mine roof horizon. The area had 8 ft. roof bolts and 12ft. cable bolts installed. All entries around the fall have two rows of timbers and are barricaded. Mine ventilation was not affected. No injuries and no equipment was involved.

Employee fell backwards to the ground while using channel locks to pull a crease out of belt. Employee tried to break the fall injuring employee's left wrist.
Employee turned around to walk to the back of the bolter when they heard the rock above break. Employee moved to get out of the way and stuck a hand up to push the rock away loosing balance and falling to the floor causing a laceration to the neck area.
A Shop Mechanic was trouble shooting the airflow through the radiator fan when employee's right hand fifth finger came in contact with the fan blade. Resulting in a laceration requiring seven stitches.
Location 5th West Mains, Entry 1, x-cut 7. Employee inspected area, scaled rib-line and roof area. EE pulled bolter in entry and started the first row of bolts. After installing the 2nd bolt the rib started to slough off for a distance of approximately 35'. A rock hit the employee's leg, causing ee to be pinned against the roof bolter, resulting in a fracture to left leg.

A Roof fall in 3rd South Panel entry 6 xc30 intersection area was bolted with 8 foot TR and 12 foot cable bolts and straps. Area had a slip in it and top was adverse condition, ventilation was not affected, stoppings were intact and area is dangered off on all sides. Roof fall will not be cleaned up.
A roof fall occurred on 3rd south panel, entry 4, xc 21. The fall occurred in a fault zone. The fall area was rebolted and cleaned up using the miner and scar. Additional cable bolts were installed on all sides of the fall as well as the placement of cribs and timbers. Mine ventilation was not effected. The fall was approx. 20' high and involved the entire intersection.
While hanging structure, the employees fifth digit (pinky finger) on left hand got caught between the belt and belt chain, causing a laceration requiring stitches. Employee was not wearing gloves at the time of incident.
Employee was bent down making a cable splice with back turned to the rib. A rock fell hitting the employee in the back, resulting in a fracture to back. The size of the rock of 2' x 3' x 9""-14"" thick. Incident occurred in the 3rd South Panel Entry 5 Crosscut 13.
Roof fall 5th main west entry 3 XC5 in the neutral beside belt entry. Area was bolted with 8ft TR and 12 foot cable bolts and had some supplemental support extra bolts, straps, Area was in a fault zone, ventilation was not affected. Stoppings were intact, area is dangered off on all sides and will not be cleaned up. Size 20ftX30FtX10Ft high.
Employee slipped and fell while carrying a roller frame. The roller frame fell on top of the employee causing a fracture to left rib.
Employee was assisting to flip over a piece of belt to roll up when the employee's channel locks slipped off belt, employee felt a strain to the right shoulder.
Employee let drill pot down and drill steel fractured finger.

The employee stepped near scoop to unload material, the employee did not realize that the scoop operator had not lowered the scoop bucket completely. The scoop bucket when lowered landed on employee's foot causing injury.
Roof fall occurred 4th east rooms in entry 3 crosscut 2 no equipment or people involved no damage to return stopping there were no timbers or cribs in this area there was extra support straps and bolts. The fall was 45X35 and 15 feet high. This Area was originally bolted with 8' TR bolts and 12' cable bolts area was dangered off and timbers set.
2nd S.Panel#2 Entry x-c9, roof fall was found intersection. Around 45ft x 25ft x 8-10ft hi in return airway where no one works or travels. This is same area last fall at x-c8 happened. The area was considered bad 9/1/2015 and lifeline rerouted extra support installed. Area originally bolted 8ft full grout rebar bolts & cable bolts in intersection. The ventilation not affected.
While employee was roof bolting, his glove caught the moving drill steel, causing wrist to twist.
Employee was cleaning out flap on hurricane of Fletcher Roof Bolter. Employee did not notify Roof Bolter partner prior to cleaning. Roof Bolter Partner started bolter, causing the flap to close on injured employees hand.

No injuries, airways not affected. Depth of cover was around 685ft, area bolted with 8ft resin bolts and 5 12ft cable bolts & screened. The fall was around 10-15ft high x 25ft long x 30ft wide
Employee was walking and stumbled. Employee tried to catch himself when falling using his hand and landed on a rock, causing his right ring finger to dislocate.

Employee was roof bolting on offside at #2 face, he put up outside bolt was swinging over to put up 2nd bolt when rock fell from top and landed on his foot. Employee was wearing rubber steel toed boot. Jagged point of rock punctured through the upper portion of boot and punctured his foot.

11/7 employee reported a shoulder strain while roof bolting. When he arrived at ER, his eye was hurting, but nothing was seen in his eye. 11/8 Employee returned to ER with his eye irritated. A fleck of metal was removed from his eye.

Normal mining conditions. Employee caught middle left finger between a roof bolt plate and the mine roof, crushing the tip of his finger.
The area was wet and slick, employee fell and landed on his back.

Employee was helping install a pot on a Joy miner. While going to install the conveyor motor, it slid down, catching the employees finger between the motor and pot, smashing his middle finger. A small piece of loose rock was observed hanging from roof. Bolter operator pulled it down with a claw hammer and it bounced off of rib and hit him on top of his left foot.

Employee hammering on a shuttle car reel chain and a piece of metal flew off the hammer striking him in the right upper leg.
Miner was in scoop bucket feeding duster, scoop lost brakes and slid under belt line structure with him in bucket. Miner had cuts, bruises and contusions.

Park brake on shuttle car locked up causing tires to lock up and car began to slide down toward the miner quickly. Operator got the brakes released and attempted to regain control of the machine to steer it into the rib. The slick conditions didn't allow traction, the machine slammed into corner rib, causing the operator to be thrown around inside the cab, which caused the injury.
