The victim had traveled inby permanent roof support in the #3 entry right crosscut #4 and a section of draw rock fell in the unsupported area fatally injuring the victim. The continuous miner was cutting into #3 right crosscut from the #4 entry when the draw rock fell out on miner and victim.

This was an extension of a roof fall that was discovered 10/15/08 on 2 South, #3 entry, #4 x-cut. The secondary escapeway was blocked, but it was re-routed to the #3 entry. The fall was approximately 40' x 30' x 10' thick. Area was mined August 2006, bolted with 6' fully grouted roof bolts. Initial fall had been timbered off.
EE did not show at the surface, men were sent to travel his normal weekly exam routes. Two co-workers found his body at approx. 3:30 pm at the #34 x-cut, #7 Entry, 2 South panel. EE was transported to the surface, where LeFlore County EMS awaited the county Coroner, who pronounced him dead. Mine Inspector on property was called at 3:50 pm.
Injured tripped/slipped going through the split in the curtain at the section power center.
Fireboss examining #4 belts, slipped and fell. Caught himself with left hand, landed on left side, dislocating his left shoulder.
Conditions contributing to the Accident;1 West section,#22 x-cut, #2 Entry, 4-way intersection; 25-30 ft. long by 25-30 ft. wide by 7 ft. thick. Appeared to be in a slip area; bolted with 6 ft. fully grouted bolts. area mined in 2004.This area has been barricaded with cribs timbers and boards
Injured was loosening bolts on refuge screen pulley and middle finger of right hand was caught between wrench and frame, smashing the tip of the finger.
2 South Section, Roof Fall occurred #4 entry x-c 42 in a 3 way intersection 25'w x 30'l x 18' hi. A x-c was turned just inby a known fault area. Area bolted with 8' tension rebar full grout bolts.

Lifting a timber to load in scoop bucket, had sharp pain in lower back and fell to floor. Employee put on back board and took to hospital where he was treated and released then returned to work next shift. On Jan 5th, 09, he called in with back hurting again and went to dr and has not returned to work since.
On 11-15-08 Injury reported a injury when going thru mandoor at 2 drive his left knee popped. Employee completed report but did not miss any work or go to dr until 4-2-09.
3 West #1 Room #5 entry just inby intersection x-cut 1, right side rib rolled off behind line curtain hitting miner operator and scoop operator. Miner operator broke 2 toes on left foot
Roof Fall, 3 West, #2entry belt neutral (outby section tail 1-2 xcuts), xc 36, 40'x40'x10' thick Area was good when mined and 2 weeks later started busting up showing signs of fault. Area was bolted with 8' tension rebar, Depth is over 600' of cover.
Roof Fall discovered in the 3 South, 2nd Room, Bleeder, Crosscut 10. The fall was 18' wide by 10-12' thick and unknown length. The 3 South is being pillared at the time of the fall and the fall was located in a fault zone.
Roof Fall, 3 West Panel, #5 entry, x-c 13 intake, area mined around 08/18/07 bolts were 8' tension rebar 3/4"" diameter. area had fault which started showing in 12/2007 and the area was cribbed & dangered off in 12/2007. 6/23/2008 the had additional timbers added around the fall. The escapeway was in the #6 entry. The fall measured 25' x 20' x 12' thick
Roof Fall on 3 South 2nd Rooms outby working section, #4 enry x-c 6. Area mined in 5-5-08 and showed signs of fault bolted with 8' tension bolts then 16' cable bolts. 5-26-08 area was cribbed off and escapeway rerouted prior to fall on 6-9-08. The fall was 20' Long x 70' wide x 8-12' thick.
EE was operating s/car in the #3 South, #2 entry and tire went into a pot hole and hit his head, jamming neck into the canopy.
Mine was idled and the Examiner preshifting for 3rd shift found roof fall during exam at 9:50pm in the 1st Mains #3 entry, xc 6. The fall was in intake travelway in oldest part of mine, mined around Oct. 1996 and measured 25'w x 12'L x 6'-8' hi. The area was supported with 6' fully grouted bolts in a fault area with some cribs.

Employee reported injury 12-27-07 but did not miss any work until 4-17-08, still hurting and went to doctor. Employee stepped through a check curtain and twisted right knee.
Roof Fall, 3 South Section, #5 entry x-c 12, area started working and was dangered off, 2hrs before it fell, size was 60'L x 18' w x 13' thick. Area was bolted with 8' tension rebar bolts.
Doing pre-shift in #7 entry and slipped and fell hitting right hand finger on hammer or caplight attached to mining belt.
Employee was operating shuttle car when he came to a 6"" ledge and hit his head on canopy then his neck and back started hurting. Reported to operator on 11-19-07 but never missed any work until 11-26-07 over incident.
Submitted online 10/16/07 submission # 526437 but had mistake filing so resubmit. Roof bolter operator slipped while moving the drill station, right hand was on canopy post and left hand hit canopy lever, dropping canopy and cutting little finger on right hand.

Msha issued neg findings 6-1, event 4260825. Then on 6/18 issued #2 on same event, that there was an ignition concerning scsr. Cont.miner punctured while loading gob onto a scar & caught fire & extinguished within 5-7 min. This is not considered an ignition & disagree with Msha's 2nd paper after closing original out on 6-1. Being reported only to abate 7284820 citation 6/18.

Roof fall found during holidays outby the 2 South Section, #1 entry, x-c19 intersection, area was a fault with water, bolted with 6' resin bolts and additional 8' supplemental support, the fall area has been cribbed and dangered off, stoppings were rerouted around bad area for travel down the return.
The dayshift operator, who works 5:30am - 1pm got off work, went to his personal vehicle and gave the 2nd shift operator a ride to loader. As 2nd shift operator walked away he heard the employee's engine rev up and went back to vehicle where the employee had fallen over from apparent heart attack at 2:08pm.
Injured was replacing offside tram motor on a shuttle car. A chain hoist was used to raise the motor. The motor shifted and the chain rolled from the top of car frame to the side of the frame and came across injured's left index finger, smashing it.
Roof Fall, while pillaring Hatfield Mains #5 entry x-cut #10, 8:40pm 12' of unbolted rock came in on continuous miner cutter head while backing out and wedged miner in x-cut, at 11:05pm the roof fall occured inby in the Hatfield Mains #5 entry from x-cut #10 down to x-cut #11. Areas was cribbed and timbered off and miner pulled out from under rock.
Employee was roof bolting. When he stopped to take wrench out of drill pot it was stuck. He took a hammer and started hitting the roof bolt wrench. A small piece of steel flew into his right eye. #1 entry, 002 section.
Drilling hole for roof bolt, the operator reached out and grabbed while still spinning, twisting left hand, fracturing hand. Note: Empoyee took drug test on 2-6-06 and results came back positive for drugs on 2-10-06 and employee was terminated per company policy.

Roof fall found in 1st right in the #3 entry x-c17 across from the #3 belt in belt neutral. Fall was 60' x 60' x 6-10' thick. The belt access point had already been cribbed and timbered before fall and after fall all other access points were cribbed off. Area mined and bolted around Dec.2001 with 6' full grout resin bolts thru a slip in top. No plans to reclaim.
Employee reported he slipped on steps of a belt crossover falling and landing on hip and lower back and was in pain. Sent to dr. reported bruise
Existing Roof Fall from 1997 fell out again was found on the intake travelway about 10'long x 20' wide x 10' thick by the 3rd shift fire boss at 9pm as he was going in to do preshift exam. Fall was in the #4entry around 320' inside portal. The mines had been idled since 11-23-05.
Roof fall occured in the intake intersection of New 2nd Right Panel, #6 entry at x-cut #9. 800' outby the face. The fall measrued 20' x 20' x 7' thick. The area was known to be bad and had supplemental support and cribs all along the area for 3 x-cuts inby.
Employee bent down to put crib under miner and when he stood back up his knee popped with pain. He finished his shift but next day knee was swelled so he went to doctor and returned to work next day.

Area was located in belt intake neutral where noone travels but weekly fire boss. Area was not in any escape ways. The fall will not be cleaned up.
Changing a head shaft on Stamler Feeder when the shaft became lodged against frame and when shaft became free the weight shifted and the shaft hit employee in the ribs knocking him down.
A big rock was hung in stacker chute & employee climbed up on top of stacker guard around 2' hi and slipped, falling to ground.
EE was making a belt splice and a piece of steel broke off hammer and went through his clothing and penetrated his upper left chest. EE had steel removed on 5-3-05.
Mine Idled working on undercast project outby 1st left #2 belt, The employee did not report any injuries to the Foreman on 2/27/05. Employee came out to mines on 2/28/05 and reported he pulled muscle in his back lifting sunday night and was going to doctor. Foreman stated he did not see how that he couldn't get the guy to lift a finger to help the others that night.

Located in Hatfield Mains in return #3 entry at x-cut 22. Continuation of an old rooffall reported in 6-4-02. Area was already cribbed and dangered off to impead travel. Fell on downhill around 50'x60'x8' hi. Area was bolted with 6' fully grouted resin bolts.
Walking down slope to mine opening and slipped on patch of ice 50 foot from portal and fell to ground spraining right ankle.
Employee was helping to build seals in the Hatfield 1st left and had skin irritation from the B-bond sealant on his chest, stomach, legs and arms.
During a AAA inspection Employee & Inspector were walking return in #2 Left Panel in the #1 entry x-c25 where they found fall outby pillar section. size 8'hi x 70'long x 45' wide located in fault zone. Fall not in way & dangered area off.
Third shift outby rolling up belt. Foreman told men not to climb on top of scoop to pull belt & left to go to another area. Employee climbed up on scoop anyway. Some of guys told him not to as well and when he climbed up to pull belt slipped & fell off scoop hitting ground.
Employee was behind curtain and scoop came by. The floor was slick & downhill grade & scoop slid into curtain. Canopy of scoop pinched employee against rib hitting him in chest & back.

South Central changed to Double-lok bolts april 2004.
Employee was walking down #3 belt line & went through low place & bumped head & stepped on loose rock & lost balance & fell twisting ankle.
Injured was chipping weld slag and a piece went into his eye. Injured was wearing glasses but it entered through the side of his glasses. The injured was treated with cream for his eye and prescriptions for pain and anti-biotics.
The foreman was operating the scoop, pulling feeder cable slack,and the slack became hung under the feeder. The cable suddenly came loose, striking the injured on the left leg.This report was not filed within 10 days because there was a questionwhether or not it was job related. l
Injured was operating a battery powered golf cart and the brakes failed. Injured steered the cart into the left rib and was thrown from the vehicle.He went to St. Edward's hospital where he recieved stitches and prescription for pain.
Injured was welding, using only glasses and a lens for eye protection,and his eyes were burned. Injured went to the ERand was given cream for his eyes and a prescription for pain medication.

FALL IS 20' X 40' X 10' THICK. THIS WAS AN INTAKE TRAVEL WAY AND THE TRAVEL WAY HAS BEEN RE-ROUTED TO #7 ENTRY.
ROOF FALL OCCURRED ON THE #6 BELT AT X-CUT 34, 3 X-CUTS OUTBY THE SECTION. THE FALL WAS IN AN APPARENT SLIP/FAULT AREA. IT WILL NOT BE CLEANED UP. THE SECTION WAS MOVED. 40' - 50' OF BELT AND STRUCTURE WAS DESTROYED.
WHILE PERFORMING ROOF BOLTING OPERATIONS THE OPERATOR WAS FEEDING A RESIN BOLT INTO THE ROOF WHEN THE BOLT GOT STUCK. THIS CAUSED THE BOLT TO BEND MASHING THE OPERATOR'S HAND BETWEEN THE DRILL HEAD AND THE BOLT.
WHILE UNLOADING FROM MANTRIP OUTSIDE AT END OF SHIFT THE RADIATOR CAP BLEW OFF ALLOWING HOT FLUID TO BLOW OUT.

THE FALL OCCURR ED IN A FAULT AREA THAT WAS ABANDONED 3-02. THE AREA WAS MINED 3-02. NO PERSONS OR EQUIPMENT WERE INVOLVED AND THE FALL HAS BEEN DANGERED OFF.
WHILE INSTALLING A ROOF BOLT THE BOLT WRENCH BROKE AND STRUCK THE OPERATER IN THE FACE. IT CUT HIS LIP AND BROKE 2 TEETH.

WHILE POSITIONING FEEDER ONTO TAILPIECE, THE FEEDER STRUCK TAILPIECE KNOCKING IT ONTO HIS FOOT.

PIECE OF METAL FLEW OFF MINER CONVEYOR CHAIN WHILE 2 REPAIRMEN WERE WORKING ON IT AND INJURED WAS WALKING TOWARDS MINER WHEN A PIECE OF METAL HIT HIM IN THE EYE.
MINER OPERATOR WAS SETTING MINER UP IN 3 FACE TOTURN RIGHT HAND X-CUT AND APPARENTLY GOT BETWEENMINER & RIB & WAS PINCHED. EMPLOYEE WAS INJURED IN WAIST AREA.

ROOF FALL IN #4 ENTRY X-CUT 42. AREA WAS KNOWN TO MANAGEMENT SINCE 5/15/00. HAD TIMBERS & SUPPLEMENTAL SUPPORT & WAS MONITORED. 5/18 CRIBS WEREADDED. 5/31 MORE CRIBBS WERE ADDED & AREA DANGER ED OFF & RUNAROUND CREATED. 20'W X 22'L X 10'H.
HE WAS OPERATING THE #3 MBC, SETTING IN DECK WITH CONVEYOR RUNNING & TRAMMING FORWARD. PAPER TOWEL KEPT BLOWING IN HIS FACE & HE KEPT SWATTING IT AWAY AND IT LANDED ON THE SIDE OF THE CONVEYOR REDUCER AND HE REACHED TO KNOCK IT OFF, WHEN HIS FINGER GOT CAUGHT IN THE REDUCER, CUTTING HIS FINGER.
RIDING IN MANTRIP INTO THE MINE, THE MANTRIP WAS TURNING TO THE LEFT AND THE REAR HALF OF THE MANTRIP BRUSHED UP AGAINST THE RIB CORNER. EE WAS HOLDING ONTO THE VERTICAL POST WITH HIS FINGERS ON THE OUTSIDE OF THE MANTRIP. HIS FINGERS GOT CAUGHT BETWEEN THE RIB AND THE MANTRIP.
EE WAS SHOVELING IN THE INBY RIB OF A CROSSCUT BETWEEN THE #3 & #4 ENTRY. THE DIESEL SCOOP CAME FROM THE #3 ENTRY ENGINE END FIRST, HE HAD 5 TO 6 FEET CLEARANCE BETWEEN THE SCOOP AND RIB. TH E SCOOP PAST BY AND TURNED LEFT GOING DOWN HILL. IT STOPPED AN DSLID DOWN HILL NEXT TO INBY RIB. EE WAS MOVING OUT OF THE WAY BUT THE SCOOP BUCKET PINCHED HIS LEFT LEG AGAINST RIB.
DRIVING THE MANTRIP IN AND OUT OF THE MINE, HIT A HOLE AN DHIT HIS HEAD ON THE ROOF OF THE MANTRIP.
EE WAS CROSSING OVER THE CONVEYOR BELT ON THE SECTION'S REM (METAL) FROM THE RETURN SIDE OF THE BELTLINE. THE BELT WAS OFF AND THEN SOMEONE TURNED THE BELT ON CAUSING THE EE TO FALL DOWN ONTO HIS RIGHT SHOULDER. EE HAS BEEN COUNSELED ON THE BELTS. IT IS AGAINST COMPANY POLICY TO CROSS BELT.
EMPLOYEE WAS RUNNING AWAY FROM ANOTHER EMPLOYEE AND HE FELL DOWN LANDING ON HIS RIGHT SIDE. THIS INJURY WAS A RESULT OF HORSEPLAY.
WHEN STEPPING OFF OF THE LOW LOW, HE STEPPED ON A CHUNK OF COAL THAT CRUMBLED BENEATH HIS FEET, CAUSING HIM TO TWIST HIS ANKLE.
WHILE LEVELING THE BELT LINE, HE WAS MANUALLY RAISING THE STRUCTURE CHANNELS TO THE PROPER LEVEL. HE FELT A STRAIN IN HIS LOWER BACK.
CLAIMED THAT HIS NECK WAS INJURED ON MANTRIP. THIS REPORT IS BEING DISPUTED WITH WORKMANS COMPEMPLOYEE DIDN'T NOTIFY ABOUT THE ACCIDENT UNTIL 2 MONTHS LATER.
WHILE RIDING IN MANTRIP TRAVELING INTO MINE, THE MANTRIP HIT A POT HOLE IN ROADWAY IN MAINS ENTRY AT 3XC. THIS CAUSED THE EMPLOYEE TO BOUNCE OUT OF SEAT, HITTING HIS HEAD AGAINST THE ROOF OF T HE MANTRIP. EMPLOYEE WAS WEARING HIS HARD HAT ATTHAT TIME. DID NOT MISS WORK UNTIL 3-31-00.
CARRYING A 12' CRIB TIE, HE SLIPPED AND WHEN HE CAUGHT HIS FOOTING, HE HAD A SHARP PAIN IN HIS LOWER BACK.
ROOF FALL IN MAINS #2 ENTRY FROM #7 TO C6. C7 TO C6 WAS ALREADY CRIBBED OFF, XC7 WAS INACCESSIBLE. XC6 WAS PRIMARY ESCAPEWAY LEADING TO 1ST LEFT PANEL XC6 FALL AREA WAS 18' LONG BY 20' WIDE BY 2-7' HIGH FROM GROUND, XC7 TO XC6 42' LONG 2' TO 8' HIGH 18' WIDE. IT WILL BE RESUPPORTED AND TRAVEL OPENED BACK UP. AREAS WILL NOT BE CRIBBED AND DANGERED OFF.
WHILE BENDING DOWN, HIS RIGHT KNEE TWISTED.
ROOF FALL LOCATED AT X-CUT 21 IN THE #2 AND #3 INTERSECTION, 2-3 X-CUT. #2 ENTRY IS ALTERNATE ESCAPEWAY #3 IN BLET ENTRY 90' WIDE 21' LONG 6-15' HIGH. AREA BEING DANGERED OFF. A PLAN WILL BE WROTE UP AND FALL AREA REHABILITATED.
CLAIMS SORE WRISTS AND ARMS. THIS CLAIM IS BEINGDISBUTED; CLAIMED TO HAVE HAD THIS PROBLEM FROM A PRIOR JOB AND WAS TAKING A PERSONAL DAY OFF, THEN CAME BACK AND FILED WORKMANS COMP.
BASED ON OUR INVESTIGATION THE EMPLOYEE WAS TRAVELING THE #2 CONVEYOR. THE NEXT TIME THE EMPLOYEE WAS PHYSICALLY SITED, HE WAS ON THE #2 CONVEYOR. THE EMPLOYEE ENTERED AN AREA TOO LOW FOR HIM TO PASS AND WAS CAUGHT IN BETWEEN THE MINE ROOF AND THE #2 CONVEYOR. ESTIMATED INJURIES TO APPROXIMATELY 50% OF THE BODY.
EE GOT SMASHED BETWEEN RIBS AND MBC.
REPORTED TO SUPERVISOR 1/7/00. HIT HIS HEAD ON ROOF BOLT.
