While helping another 18 wheel over the road haul truck driver check to see if the tail gate was closing tight, the injured got his thumb caught between the tailgate and bed when the driver closed the latch before being signaled to do so.

While gathering and burning brush, the injured's leg became entangled in some vines. When he attempted to pull it free, he immediately felt pain in the leg from a strain.
Employee is a Long Haul Truck Driver and was off-loading product. As he was descending ladder from top of tanker, he missed the last rung causing him to fall to the ground and strike head and neck against the ground, resulting in contusion to head and neck.

No medical or time lost until 2/12/08, when doctor gave the injured two prescriptioins. One was an anit-inflamitory and the second a pain pill. While breaking coal chunks in the crusher box, the injured raised a metal bar and strained a muscle on his arm. He has continued to work.

Company was not notified until 8/22/07 of alleged injury. No explanation other than a slip & fall 16 days earlier. Last day of work 8/18/07. Company notified after calling injured after six days of no show that he claims injury of his lower back muscle, hips & tailbone. No injury report on his written timesheet for the 7th through his last day of work on the 16th.

While putting the dragline walk lock on the walk shelp, the lock fell on his hand and cut it requiring stitches.
While helping remove & unload a dragline swing motor, the motor shifted & pinched the injured's foot, bruising/straining it. As a precautionary measure, x-rays were taken & the injured was given a prescriptiong for pain medication, should he need it.
While working on the dragline, the injured hit his finger with the handle of a hammer, smashing or bursting it open requiring 6 stitches to close it.
While operating the water wagon in the pit, the injured lost control and ran into a pile of shot material, straining his body and hitting his head. No injuries other than strain and headache.
Notified company 4/26/06 that he received a prescription that he did not fill or use. While helping the mechanic pull a rear drive out of a water wagon, the differential moved suddenly & pinched his hand causing swelling. Diagnostic X-rays on date of injury and release to return to work on his next regular shift 4/15/06. No lost time due to injury, only the prescription.

While hanging a strap attached to the dragline powercord to a hook on the shoe to help move the cord, the injured got his finger caught between the strap and the shoe causing a broken finger.
Installing bearing into a radiator guard on D9L. A piece of bearing broke off and embedded itself in EE's finger on right hand, between 1st and 2nd knuckle.

WHILE COMPLETING A 200 HOUR SERVICE ON A 637 SCRAPER, EMPLOYEE WAS STEPPING OFF THE FRONT ENGINE USING THE STEPS MOUNTED TO THE MACHINE. HE LOST HIS FOOTING WHILE DISMOUNTING AND STUMBLED BACKWARDS FALLING INTO THE TOOLBOX DOOR OF HIS SERVICE TRUCK. HE CAME INTO CONTACT WITH THE DOOR IN HIS BUTTOCK AREA, CUTTING HIS BUTTOCK WHICH REQUIRED STITCHES.
WHILE CUTTING HOSE TO BE USED ON A REPAIR, THE INJURED SLIPPED AND CUT HIS HAND WITH THE UTILITY KNIFE HE WAS USING RESULTING IN 17 STITCHES.
WHILE CUTTING BRACE LOOSE DURING REPAIR PROJECT, GREASE IN BULL GEARBOX WAS IGNITED. BECAUSE OF DIFFICULTY GETTING TO SOURCE OF FIRE, EXTINGUISHERS COULDN'T EXTINGUISH COMPLETE FIRE & FOAM FROM FIRE DEPT UNITS WAS USED. FIRE CONFINED TO GEARBOX INTERIOR. DAMAGE WAS PREDOMINATELY SMOKE & A FEW NEARBY WIRES WERE REPLACED DUE TO HEAT. CLEANUP/REPAIR ADDED UNDER 1/2 DAY TO PROJECT.

DURING A SEVERE STORM A DIRECT LIGHTENING STRIKE HIT THE COMPANY'S SUBSTATION CAUSING A FIRE THAT BURNED UNTIL APPROXIMATELY 3:30 P.M. WHEN THE FIRE DEPARTMENT GOT IT OUT. FIRE CONFINED TO THE OCB UNIT. NO LIGHTENING ARRESTOR DAMAGE ON EITHER SIDE OF SUBSTATION. THERE WERE NO INJURIES FROM THE FIRE.
WHILE HELPING TO REMOVE THE HOIST MOTOR FROM THE DRAGLINE, THE INJURED LOST HIS FOOTING AND FELL INTO THE DRAG TROUGH AND STRAINED HIS BACK.
EE WAS REMOVING HIS 5 GALLON WATER CAN TO ADD ICE. WATER CAN HAD APPROX 2 GALLONS OF WATER IN ITTHE CAN WAS 7FT OFF THE GROUND WHERE HE HAD IT MOUNTED. EE UNSTRAPPED THE CAN & LIFTED IT FROM THE RACK. HE PULLED IT TO HIMSELF, TURNED RIGHT TO LOWER THE CAN DOWN & FELT SHARP PAIN IN LOWER BACK
WHILE CLIMBING DOWN A LADDER INTO A DRAGLINE TUB DURING A SHUTDOWN, THE INJURED SLIPPED OFF THE NEXT TO THE LAST RUNG OF THE LADDER AND FELL APPROXIMATELY 2 FOOT TO THE TUB, CAUSING A STRAIN T O HIS BODY.
WHILE CLEANING THE FILTERS ON THE HIGHWALL DRILL, THE INJURED FELL FROM THE DRILL PLATFORM TO THE GROUND CAUSING SCRATCHES, CUTS, AND SORENESS TO THE RIGHT SIDE OF HIS BODY.
WHILE LIFTING BUCKETS ON 2/10/02 THE INJURED THOUGHT HE MAY HAVE PULLED SOMETHING IN HIS ABDOMEN BUT HAD NO ADDITIONAL PROBLEMS UNTIL 2/16/02. WHILE PULLING HARD ON A PIPE WRENCH AS HE WAS WO RKING TO REPAIR AN AIR LINE, THE INJURED FELT A SHARP PAIN IN HIS ABDOMEN. HE SUFFERED A HERNIA THAT WAS REPAIRED SURGICALLY ON THE 19TH OF FEBRUARY.
WHILE DISMOUNTING FROM THE FRONT AREA OF A LOADER AFTER MAKING A REPAIR, THE INJURED SLIPPED ON THE BOTTOM RUNG OF THE LADDER AND FELL BACKWARD LESS THAN TWO FEET AND HIT HIS TAIL BONE ON THE Z-BAR OF THE LOADER BUCKET. HE FRACTURED HIS TAIL BONE. INCLIMENT WEATHER CONTRIBUTED TO SLIPPERY CONDITIONS.

THE INJURED WAS HELPING PRY A PINION GEAR ONTO ASHAFT WHEN THE BAR SLIPPED, ALLOWING HIM TO SLIPAND FALL TO THE FLLOR, STRAINING HIS BACK.
WHILE HELPING REMOVE A PIN ON THE DRAGLINE BUCKET SPREADER BAR A PIECE CHIPPED OFF THE PIN AND STUCK IN HIS LEG, A DOCTOR REMOVED IT AND GAVE HIM A PRESCRIPTION FOR ANTIBIOTIC AND SENT HIM BAC K TO WORK.

WHILE BACKING HIS DOZER, THE INJURED BACKED OVER A RISE AT THE REAR CAUSING HIM TO HIT HIS HEAD ON THE BACK OF THE DOZER AND HIS CHEST. HE RECEIVED STITCHES IN HIS HEAD AND BRUISED HIS CHEST.
WHILE DISMOUNTING FROM A LOADER, THE INJURED SLIPPED OFF THE BOTTOM STEP AND STRAINED HIS BACK WHILE CATCHING HIMSELF. EMPLOYER WAS NOT NOTIFIED UNTIL 10-12-00.
WHILE HELPING RE-SOCKET THE DRAGLINE HOIST CABLE, THE INJURED PULLED A MUSCLE IN HIS BACK. HE DID NOT SEEK MEDICAL HELP. TWO AND A HALF MONTHS LATER DOCTOR GAVE HIM A PRESCRIPTION AND SENT HIM BACK TO WORK.
WHILE WIPING SWEAT FROM HIS FOREHEAD WITH A RAG, A PIECE OF METAL SHAVING STUCK IN THE RAG SCRATCHED HIS EYE.
WHILE CLEANING COAL WITH HIS LOADER BUCKET, THE INJURED MADE ONE MANEUVER THAT ALLOWED HIM TO BACK INTO THE HIGHWALL STRIKING IT HARD WITH THE REAR BUMPER CAUSING A WHIPLASH (SPRAIN) INJURY TO HIS NECK.
AFTER CLEANING A SUCTION SCREEN, EMPLOYEE FELT PAIN IN HIS BACK. NO SPECIAL CIRCUMSTANCES CAUSED THIS INJURY.
WHILE HELPING TO MOVE POWER CABLE, THE INJURED GOT HIS FOOT CAUGHT IN A LOOP OF CABLE BEING PULLED BY A BULLDOZER CAUSING A SPRAIN TO HIS RIGHTANKLE.
