Employee was walking through plant to perform crusher inspections. While doing so something blew into left eye.

At 4:30pm a supervisor noticed a grass fire at the North end of the mine site. 911 called with fire department arriving at approximately 4:55pm. Fire extinguished by 5:33pm. No injury to miners nor damage to company property or assets. The extent of damage limited to grass and brush.

Minor was moving a 55 gallon drum utilizing a hand dolly designed to do so. Minor slipped and fell onto left side. Minor complained of left shoulder pain and later taken for medical care. This has been first aid with employee being observed and not restricted of any duty. Ultimately on November 4, 2019 employee underwent surgery on left shoulder resulting in loss time from work.

Employee was working on a 922 loader installing bottom bucket pin. The pin was struck by forklift and bushing was forced from opposite side of pin carrier and struck the employee in the face. He was knocked back from machine, lip was cut and tooth was chipped. Employee reported this incident Friday 11-13-15 at 11:15 AM to his supervisor
Employee was cutting a bolt. The employee lost his footing, grabbing for the guard, the jagged edge of the guard cut the left forearm.

Employees were replacing a pin on the stick cylinder on a 345 trackhoe. Injured employee picked up the pin, weight approx. 25 lbs, and handed it to another employee. He took two steps with the pin and lifted it up and felt a sharp pain on the back of his left knee. Later diagnosed with a meniscus tear.
Miner was installing a toggle plate in the jaw. The bar being used to center and seat the plate slipped striking the miner above the left eye causing a minor laceration.

EE was pulling rear drive shaft out for repairs it slipped and turned and he lost control of it. The rear drive shaft glanced off of EE right jaw causing laceration requiring six stitches.

EE was trying to straighten up a bolt under a bearing when the bearing slipped and smashed his left ring finger. Injury required 6 stitches and broke the very tip of his finger.

EE was assisting a CAT field mechanic to line up a final drive. While lifting the final drive up with the forklift, the final drive tipped forward. Our employee had his left hand on the chain that they were using around the final drive. His left index and left ring finger was pinched between the final drive and the chain. This resulted in 3 stitches in each injured finger.

Ee had been stripping using a dozer. While the dozer was being fueled up, another employee noticed that the dozer was missing a pin. The ee walked back to where he had pushed down some timber to look for the pin. He stepped on a rock, it rolled underneath his foot causing him to fall and sprain his right ankle.

Two employees were removing a screen from the top deck of a tower. As they prepared to throw the screen down to the secured area below, one of the employees gloves got caught on the lip of the screen and pushed his left pinky finger in to the handrail of the catwalk. Employee recevied a broken joint on his left pinky and a laceration injury between the pinky and the ring finger.

Ee reported on 12/28/05 at 10:30 am that on 11/16/05 he felt a pain in his neck after removing the wheel chock on the front-end loader that he operates. Ee was sent to the doctor on 12/28/05 and was released to full duty status; however, doctor is sending Ee to PT for rehab exercises to start on 12/30/05.
Ee was working to dig out the 7' cone crusher when he twisted his ankle. Diagnosed with a mild sprain and was released back to work as full duty. Ee continued to complain of pain. Went back to the doctor in December 05, received an MRI in January 06 and on February 8, 2006, he was written a prescription and set up for PT.
Ee stated that he slipped and fell inside the wash plant while attempting to change screens. Ee did not report injury until 5 days later. Ee has a bruised rib.
Ee had been welding at night inside the bin at the rip rap plant. When the ee woke up the next day, he had a flash burn to both of his eyes. Ee reports that he was wearing a hood.
EE was performing regular maintenance at the jaw crusher. While turning a wrench, he felt a pop in his R. index finger. Pain radiated towards his palm. He thought that it would get better but it did not. He reported this incident one week later on 5/9/05. He was taken to the doctor and is being treated for an inflammed tendon.
Ee was lifting a wear plate and hurt his lower back.

Ee had been shoveling and when he started walking around he felt a pain in his groin and stomach area. Was taken to the doctor the following day where he was diagnosed with a hernia. Hernia had been repaired per call to Penni Carpenter on 2-3-05. coder 12.
EE was moving shot rock in the pit area. At the end of his shift he reported to his supervisor that his lower back was sore. He had previously had lower back surgery. EE was treated for a sprain in the lower back.
Employee was drilling holes in to the side of a shaker with a magnetic drill when the drill turned loose. The drill crushed his 5th digit on his left hand in between it and the shaker. The injury required 10 stitches to secure the end of his finger.

Employee was carrying two 5-gallon buckets of oil. Felt pain in his upper back/neck. Reported to supervisor later but refused treatment stating he would be ok. Recently came back to supervisor and said he was still having pain. Doctor identified a sprain on 9-17-04 to his upper back/neck that had not ever properly healed. He was placed on restricted duty at that time.
EE WAS REMOVING A 1"" PIECE OF METAL PLATE FROM INSIDE OF THE SCRAP DUMPSTER. HE HAS POSITIONED IT ON TO THE FORKLIFT, WHEN HIS LEFT FOOT SLIPPED, CAUSING HIM TO COME IN CONTACT WITH THE EDGE O F THE METAL PLATE. IT CAUSED A LACERATION ON HISLEF CALF AREA THAT REQUIRED STITCHES.
EE INVESTIGATING EXCESSIVE SMOKING ON AN 992 CAT WHEEL LOADER. THE ENGINE WAS OFF AND EE S FACE WAS 12 INCHES EXHAUST PIPE WHEN ENGINED BACKFIRED, RESULTING IN FLAMES AND ENGINE SHOOT HITTING AND BURNING FACE AND EYE.
EE WAS PULLING A PIPE UP OUT OF THE WATER/MUD. HIS FOOT SANK INTO THE MUD & HE FELT HIS RIGHT SHOULDER POP. EE WAS TAKEN TO CLINIC & RELEASED TO WORK NORMAL WORK ACTIVITY. HOWEVER, THE CONDITION LATER WORSENED & DR PERFORMED SURGERY ON 3/22/04 RESULTING IN THE INCIDENT TRANSITIONING TO A REPORTABLE LOST TIME.
EE BENT OVER TO ALIGN A WIPER BELT WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. EE WAS DIAGNOSED WITH A BACK SPASM.

JAW OPERATOR WAS BUSTING A ROCK OUT OF GRIZZLY FEEDER WITH A SLEDGE HAMMER. A CHIP OF ROCK FLEW SOME 25' UPWARD, HITTING EE IN THE FOREHEAD.
EE WAS REMOVING THE BOLTS ON A LOADER BUCKET CUTTING EDGE. ONE OF THE BOLTS WAS TURNING IN THE HOLE. HE TRIED TO HOLD THE HEAD OF THE BOLT WITH HIS HAND AS HE LOOSENED THE NOT. THE HEAD WAS WO RN AND HAD A SHARP EDGE THAT CUT HIS FINGER. HE WAS TAKEN TO CHOCTAW MEMORIAL HOSPITAL E.R. WHERE HE RECEIVED SEVEN STITCHES IN HIS RIGHT MIDDLE FINGER.
HE WAS USING A PICK TO CLEAN PACKED DIRT OUT OF THE FEEDER. AS HE CAME DOWN WITH THE PICK HI HITTHE SIDE OF THE FEEDER AND DEFLECTED AND HIT HISLOWER RIGHT LEG.
