Employee had breaker bar propped against jaw die trying to hold back up to take bolts off of the jaws toe protector. The breaker bar slipped off the jaw die and swung around and hit EE in the arm.

Employee was shoveling rock from the walkway and twisted EE's back
EE was running fuel and water to drill 569. EE was pulling pump out of water trailer when EE fell backwards and hit the ground. Slipped and fell off water trailer. Hurt right shoulder.
