Employee was removing soft start from a piece of equipment. The soft start slipped in employee's hand and cut ring finger on left hand

Struck against stationary object

Replacing Cutting Edges on 844 John Deere Loader and was taking out bolt and bolt had a spur that caught his glove and the impact gun spun causing the bur to cut through glove into the outer palm of rt hand.

EE sprained his lower back while assisting in the rescue and CPR efforts of an employee immediately reportable accident on 5/23/12.EE reported the incident to Penni Carpenter the evening of June 7, 2012.

Fall from machine

EE was climbing down from the operator cab of a loader when he lost his balance and fell to the ground. Was taken to ER on 11/6/11 for x-rays and they were negative. Continued to have pain and a MRI was ordered on 11/15/11. EE was diagnosed on 11/18/11 with a talus fracture.

Employee was helping replace V-belt on motor under feeder. The hook at the rachet end of the come-a-long came off the beam which allowed the come-a-long to strike employee in the head on the right side of his hard hat. Employee was knocked unconcious. Was taken by air ambulance and then later released.

Employee had been shoveling base material for approx. 10 minutes w/a co-worker. Then they took a 15 minute break and when he went back to shovel he felt a sharp pain in his lower back as soon as he stuck the shovel in the material. Employee was diagnosed with a muscle spasm.
