Carrying a boom lock for 824L wheel loader tripped on something on ground & dropped boom lock on right hand pinky finger
A customer truck pulled into the plant. While waiting on instruction to be loaded a fire started under the cab of the truck. An employee used a fire extinguisher to try and put out the incipient fire. The wind blew smoke & dry fire extinguisher powder toward the employee which the employee inhaled. A local fire department put out the fire which burned for approximately 40 minutes.

Operator was preparing the fuel truck for the work shift. While using 3 points of contact descending from the ladder, operator stepped onto a rock lying on the ground and twisted right ankle. Operator was taken to Urgent Care Facility for observation.

The employee stepped on the edge of a concrete pad. The sand at the edge of the pad was soft and sank down as part of the employees foot was on the pad and part off. The compressing of the sand created an uneven surface causing EE's left ankle to roll; spraining it.

Company X-rays revealed an anomaly in the employee's lungs. After medical test and examinations a doctor gave a diagnosis of Silicosis

Employee emptied two 5-gallon buckets of sand. While carrying the empty buckets back to the lab the employee felt a sharp pain in her right shoulder.

Employee was releasing the brake on a rail car. When he pulled the brake release lever his forearm contacted a metal burr on the brake wheel causing an approximate 2.5 inch laceration to his forearm.

Employee was re-hanging a power line onto an electrical pole. The employee raised the line approximately 2 inches to fasten the hanger on the power line to the insulator on the pole. As the employee raised the line a butt splice failed allowing the line to move pulling the employee into the pole fracturing his left collar bone.
Employee had his annual audiogram and the results showed a change in hearing acuity for the worse, relative to the employee's baseline audiogram.

The employee parked the backhoe in the barn at the quarry and let down the outriggers. Due to some sawhorses between the outrigger and the barn wall there was no room to walk around the backhoe. He chose to step over the outrigger rather than move the sawhorse. While stepping over the outrigger he tripped and fell fracturing his left elbow and bruising his knees and shins.

The employee was walking to open a valve. A recent winter storm had resulted in sleet and ice. The employee slipped and fell. He tried to break his fall with his left hand, which caused the small finger on his left hand to break.

The employee, assisted by another employee, was moving a railcar using a railcar puller. As the employees were in the process of removing the car puller hook from the railcar, the injured employee's left hand/finger became trapped, causing a fracture and lacerations.

Employee was assisting another ee working on a pump.The boomer they were using pull the pump housing together slipped and struck ee on his left thigh. As the ee moved to avoid being stuck, he twisted his knee. EE was released to full duties but will require further treatment for his knee. Althought the incident occurred on 12/21/04.

AFTER THE EMPLOYEE DISMOUNTED THE BOTTOM STEP OF A RAIL CAR, THE EMPLOYEE TOOK A STEP AND STRAINED HIS LEFT CALF MUSCLE. THE RAIL CAR LADDER WAS IN GOOD CONDITION. THE TRAVELWAY WAS A CLEAN, L EVEL CONCRETE WALKWAY. THE EMPLOYEE WAS NOT CARRYING ANY OBJECTS AND DID NOT SLIP OR FALL.

EE WAS OPENING AND CLOSING BOTTOM DOORS ON RAILCARS WHEN HE FELT A SLIGHT PAIN IN LOWER BACK. EE REPORTED THE INCIDENT ON 5-7-02 BUT DID NOT THINK MEDICAL TREATMENT WAS NEEDED. EE WAS SENT TO PHYSICIAN ON 5-20-02 AFTER COMPLAINING OF PAIN AND DIAGNOSED WITH A MILD STRAIN AND PUT ON LIGHT WORK RESTRICTION. EE WAS RELEASED TO FULL DUTIES AFTER 3 RESTRICTED WORK DAYS. EE WAS MOVING A RAILCAR. WHEN HE APPLIED THE HAND BRAKE TO STOP THE RAIL CAR HE FELT A PULL IN HIS ABDOMENAL MUSCLES. EE WAS ASKED TO GO TO DR BUT HE DECLINED UNTIL 3/13/01 WHERE HE WAS DIAGNO SED WITH A POSSIBLE MILD HERNIA REQUIRING MODIFIED DUTIES. FIRST DAY OF MODIFIED DUTY WAS 3/14/01 AT WHICH TIME THE INJURY BECAME REPORTABLE.
