Employee was dismounting a ladder. As they descended, they missed the 2nd step catching themselves with their right wrist.

The employee was removing a guard and as employee twisted felt a pop in elbow.

Employee was at work, sitting in a truck, when they started having chest pains. The supervisor felt they needed to get checked out and drove employee to the doctor. After examination, medical staff decided employee need further care and sent them to the hospital. No injury attributed to the condition.

Employee was tasked with climbing into a screen deck to perform maintenance. When the employee climbed in EE complained that EE hurt shoulder. Employee was taken to the doctor and was treated as a first aid case. On 11/2/2016 the case became reportable due to a second doctor's opinion saying that the employee needed surgery on the shoulder.

Employee was dragging a piece of 8"" PVC tubing attempting to attach it to the water pump on a barge and stepped backwards into a gap between the barge and catwalk. He fell down and struck the backside of his left upper arm on the corner of the handrail which caused a laceration that required five stitches.

End dump driver was backing under pit loader, pit loader said his bucket dropped, hitting bed of truck and causing haul truck to come off ground. When pit loader raised bucket up, haul truck came down, causing it to jump forward causing employee's neck to move suddenly. Haul truck driver was wearing seat belt.

Shoveling rock and strained back.

Walking around the plant to start servicing, he stepped on rock on ground and twisted ankle.

While driving, He said the airride seat lost air pressure, and he hit a bump in road and jarred his back.

Using a wedge to move rock from jaw of crusher, wedge and rock gave way and employee fell against the safety railing.

PICKING UP TOOLS OFF THE GROUND, SET 2 1/2 LB HAMMER ON #1 SCREEN. HAMMER FELL OFF SCREEN BOUNCED OFF THE CAT WALK AND STRUCK HIM ON THE RIGHT SHOULDER BLADE

CLEANING UP JUNK IRON PILES, LIFTING JUNK IRON HE SPRAINED OR STRAINED HIS BACK.

Rolling belts on V-16 and got his finger between the belt and sheave.

SPRAIN OR STRAIN. PUSHING SCREEN WITH TOP OF HEAD.

JUMPED OFF END DUMP AND LANDED WRONG. MEDICAL RECORDS RECEIVED 8/20/02, PRESCRIPTION RECEIVED FOR INJURY.

WHEN TIGHTENING BOLTS WITH WRENCH, SHARP PAIN IN RIGHT SHOULDER. 07/30/02-NO MEDICAL TREATMENT REQUESTED-NO LOST TIME. 08/07/02-MEDICAL TREATMENT REQUESTED.

DRIVING END DUMP ON HAUL ROAD, HIT BUMP IN ROAD.

LIFTING BELT GUARD OFF OF #2 SCREEN & HIT ELBOW ON CORNER OF BELT GUARD

PULLING BOXES OFF SHELF, BOTTOM BOX FELL OUT AND HIT HIS FOOT.

PUTTING METAL WEAR PLATE IN CRUSHER. IN A TWISTING MOTION, LIFTED A 100PD METAL WEAR PLATE TO PUT IN PLACE

STEPPED OUT OF TRUCK AND TWISTED HIS ANKLE. *WAS NOT NOTIFIED UNTI 8-24-00 THAT IT WAS AN ON THE JOB INJURY. PER EE ON 8-24-00, THE INJURY HAPPENED AT THE END OF THE WORK SHIFT. HE CALLED IN M ONDAY, 8-14-00 ABOUT INJURY TO ANKLE; WOULD NOT BE TO WORK. NO MENT THAT IT WAS WORK RELATED. RETURNED TO WORK FOR 12 1/4 HOURS 8-17-00, AND HAS NOT WORKED SINCE.

STEPPING OUT OF SCREEN BOX TO CATWALK, TWISTED LEFT HIP JOINT.

WHILE BACKING LOADER DOWN THE RAMP, THE LOADER CAME UPON AN UNEXPECTED ROUGH SPOT PRODUCING UNEXPECTED MOTION. HE WAS LOOKING BACKWARD AND AS A RESULT OF THE UNEXPECTED MOVEMENT, HE HIT HIS MO UTH ON THE WINDOW LATCH AND BROKE A TOOTH.

HE WAS UNDER THE LOADER, TIGHTENING A U-JOINT AND SOME DUST OR DIRT FELL IN HIS EYE. HE WAS WEARING SAFETY GLASSES WHEN THIS OCCURRED.

LOST HIS FOOTING ON A SCREEN & FELL

PULLING ON ELECTRICAL CABLE AND FELT A GRINDING IN LOWER BACK.
