Employee was swinging a hammer and hurt back. The initial reporting of the injury was employee stood up and took a deep breath and back hurt. This injury was found compensable 9/9/2016.

Employee was operating a loader when he hit a bump, jarred him hard, and hit his elbow on the rollover cage.
Employee was helping another employee when he was holding the bolts on the cutting edge of a bucket. One of the bolts dropped and cut his hand.
Employee was grinding a piece of angle iron on a bench grinder when the piece slipped into the grinder, taking his right thumb with it. He was wearing all his PPE and guards were in place at the time.
Employee was cleaning off a screen when he stated having shortness of breath and chest pains. An ambulance was called and went to the hospital. It was determined that he had an infection and severe dehydration the stemmed from that infection.

Employee was hammering in a wedge on the screen when he smashed his thumb with the hammer.
Employee was cleaning up under the conveyor when he felt a pop in his neck. He tried to continue to work but his fingers started to go numb and he got a headache. First doctors visit required no med treatment and was given no restrictions. Second visit gave restrictions and since he is summer help, it will be lost time.
Employee was working on the S-1 conveyor to splice it. He had been working on his knees and his knees had been rubbing on the conveyor belt. He received and abrasion and subsequent infection.

Employee thinks that he injured his shoulder on 10/1/12 while trying to dislodge a rock from the head pulley on R-1. He did not seek medical attention until 10/3/12.

Employee was using a hoist/come-a-long to help place a sprocket on a shaft. The hoist became dislodged catching his finger in between the sprocket and the shaft.
EE was climbing into loader. He was on bottom step of ladder when he turned to talk to someone. He lost his balance and fell off ladder, injuring his left shoulder and ribs.

EE was using a wrench and cheater pipe on a transmission bolt. The wrench slipped and struck him in the face/mouth/teeth.
EE injured his right shoulder while shoveling in silo. He didn't seek medical attention and begin losing time until 5 May 2010.

Injury used a power washer wand to wash grease off his boots. Water went inside his boot and burned his right ankle.
EE was washing the mast on the drill. He was walking along the mast washing it when he slipped. His legs slipped out from underneath him and as he fell, he sprayed himself with the power washer, which was equipped with a stripping nozzle. Took a chunk of meat out of his leg.
Injury received a spider bite on his left hand while cleaning trash under the crusher.
The 7-foot crusher would not start. Employee took a meter to check the fuses. While he was checking a fuse, one of the other leads got close to another phase, causing a short. The meter was too small for this voltage and burned his hands and face.

He was cleaning mud off block at CR-1 conveyor. When piece of chipping hammer broke off and punctured his right hand.
Employee was checking fuses in control panel on portable crusher run plant. He leaned against panel door and got right thumb against back of a amp guage, shocking him and leaving two burns on right thumb.
EE was using a come-a-long attached to a welded pad eye. It came loose and struck him in the face. No lost time, 6 stitches were required.
Employee was walking up stairs and twisted his ankle (right ankle sprain).
Employee was cutting rubber with a knife. The knife slipped and cut his left middle finger. No lost time or light duty but stitches were required.

EE was working with other employees to put the crusher back together. He got three fingers caught under the thrust washer. Finger contusion
Employee was working with another employee to load a 55-gallon oil drum in a pickup truck bed. They decided it was too heavy and were trying to set it back down when the other employee's hand slipped off the drum. The drum cut the first employee's left index finger. Eight stitches were required.
Employee was working on the rear brakes of a welding pickup truck. When he lowered the jack, the pickup truck ran over his ankle. His ankle was bruised.
Employee suffered a hernia while changing screens on the #4 8x20.

Truck driver assigned to Weatherford Batch Plant was at quarry for a load of rock. He drove up to the wet down rack so he could push the rock down & wet it before leaving. When he entered the trailer from the wet down rack, he lost his balance & fell out of the trailer. He is still losing time with fractured two ribs & bruised head, back & leg.
Employee was welding on a loader bucket. A chain hoist was attached to the bucket. The nylon strap broke and the chain host swung down and struck employee's left hip. His hip was bruised. Injured is losing time.
Employee was drilling holes in a piece of metal. He was wearing gloves and the drill caught the glove and pulled his hand. His left wrist was strained. He missed one day of work and is working restricted duty.

EE WAS ON A UNILOADER CLEANING THE CHIP PLANT. SOMETHING GOT INTO HIS EYE, EVEN THROUGH HE WAS WEARING SAFETY GLASSES.
EE WAS REPLACING THE CHAIN ON THE CR2 & SMASHED HIS THUMB BETWEEN THE CHAIN & A GEAR.
