Caught in, under or between running or meshing objects

A process attendant clearing material under the East/West CCDC screw reached into a leak hole, where their finger contacted the moving screw, amputating its tip. They reported the incident immediately, received first aid, and was taken to the ER for a tetanus shot and further treatment.

Accident type, without injuries

11:40pm a fire was discovered on hot belt 3 due to leak-by from the clinker gate. When the material was moved outside, there was no visual verification it was still going to the belt. The area was evacuated, 911 was called, and fire trucks arrived shortly after. Power was shut off and locked out for safety. The fire was out by 12:13am.

Caught in, under or between a moving and a stationary object

While diverting material to another chute, a process attendant manually assisted a gate closure using a rock bar. During the attempt, attendant was using a strong amount of force to hit the gate. Their hand shifted and left pinky was caught between the bar and equipment, causing a laceration and fractured fingertip. Attendant was transferred to the local ER and received 7 stitches.

Struck by concussion

A mechanic was performing adjustments on the coal and was injured after slag from nearby hot work ignited a gas leak in a burner pipe, causing a flashover and backflash through the kiln. The force caused the mechanic to fall back and hit EE's thigh on a nearby handrail resulting in a contusion. The mechanic was given restrictions.

Struck by concussion

During hot work on a coal pipe, slag fell into the burner pipe and ignited a combustible source, causing a flashover into the kiln. A backflash returned to the coal mill, where three other mechanics were working. The three mechanics were taken to the emergency room and were given diagnosis and work restrictions

A maintenance apprentice was assisting with coal mill adjustments was injured after slag from nearby hot work ignited a gas leak in a burner pipe, causing a flashover and backflash through the kiln. The incident led to a diagnosis with lifting restrictions.

Contact with heat

Process attendant showed signs of heat exhaustion, including feeling hot and a headache when trying to unplug the kiln feed elevator with an air lance. They came down to the ground and their symptoms worsened, prompting EMS response. Employee was treated with an IV, diagnosed with heat exhaustion and released the same day.

Utility operator was cutting a piece of vacuum hose towards EE's body and cut self in the upper abdominal area. Employee received approximate 1.5"" long laceration and was taken to the hospital and received 8 staples.

Fall to the walkway or working surface

A mechanic and mechanic apprentice were horse playing in the maintenance shop. When the apprentice went to plant their right knee, EE fell and hit their right knee on the floor and heard a pop. The employee has been diagnosed with a fractured kneecap and will undergo further evaluation.

Over-exertion in wielding or throwing objects

Employee felt a pop in their left elbow while trying to break a chunk with a poke bar and hammer on the bottom of silo. Employee was taken to the clinic for x-rays and were negative. The employee went back to the clinic six days later for another x-ray and was diagnosed with lateral epicondylitis in the left elbow. The employee received a steroid shot and was put on restrictions.

Struck by flying object

Employee was walking outside and got dust in their eye. The eye was immediately flushed out.  The irritation persisted and the employee was taken to a clinic where no foreign objects nor scratches were found. The next day, the employee still felt irritation and returned to clinic where antibiotic eye drops were prescribed to prevent infection. Employee was wearing safety glasses.

Employee stated that EE injured left shoulder when EE slipped on ice exiting the vehicle. Employee was taken to the clinic for evaluation and returned to work. Upon follow-up appointment, employee was referred for MRI. On 02/21/2024, MRI indicated a partial torn rotator cuff & Bankart lesion of the labrum. EE returned to work with restrictions and will be re-evaluated in 4 weeks.

Caught in, under or between running or meshing objects

IE tasked with inspecting mill conveyor, reached through framework of belt to clean pulley failing to LOTO. IE's hand was pulled into pulley. IE stopped the belt by using the stop cord, called for help, coworkers relieved tension on belt, freed employee's hand. IE transferred to the hospital for care. The incident resulted in friction burns to hand, arm and fracture to hand.

Heat exhaustion and dehydration. Rec'd IV fluids for hydration, blood labs were completed and determined that all kidney & liver functions were normal and could return to work with no restrictions

Employee stated that they injured their right elbow while reaching to mount a ladder. Employee was taken to the clinic for evaluation and returned to work. Upon a follow-up appointment, employee was referred for an MRI. On 5/31/2023, MRI results indicated a torn triceps tendon at right elbow. Employee was referred for surgery, has not yet been scheduled.

Struck by falling object

Foreign object entered employee's left eye from above EE's safety glasses when EE raised EE's face shield. Reported to supervisor, no issues at that time. On 12/20/2021 a metal shaving was removed by a physician and medication prescribed. Employee returned to work with no restrictions.

Struck against stationary object

Employee was repairing closing line cable on overhead bridge crane when fall protection harness lanyard became entangled in drive motor shaft while spotting the drum for repairs, pulling employee from their feet and against the coupling. The employee received broken ribs and bruising.

Employee was using a punch and 3lbs hammer to remove a lock nut from a cement transfer pump. The employee was not using a punch holder, and was holding it with their hand. The hammer struck the employee's index finger, resulting in a fracture and laceration to the tip of the finger.

Employee cleaning out hopper via inspection port. Material broke free and struck employee resulting in fractured nose.

10:07pm one employee in coal silo man elevator when it stopped. MSHA hotline was contacted at 10:13pm. Employee exited the elevator at 10:40pm, immediately reported to MSHA. Elevator was locked out until service technician inspected the elevator. Adjusted over speed switch & tested all functions.

Struck by flying object

Employee approached compressor to unload and shutdown. As they approached, the connection between compressor and hose failed (safety pins, and whip check were in place) and the hose impacted left upper arm, resulting in a puncture wound from a bolt on the coupling clamp. Employee received 6 sutures to close wound. Returned to work on next shift.

The employee entered the elevator on the first floor, closed the door then gate, and pressed the button to ascend to the second floor. The elevator left the first floor as expected but stopped approximately halfway between floors one and two. The elevator technician identified that the governor switch had tripped, locking the brake on the car cable.

Employee was descending a scaffold ladder when EE's foot slipped. EE's fall was controlled by EE's hands on the ladder rail. EE's right ankle was twisted when EE's foot came into contact with the ground.

Employee was working on the Clinker Cooler Bag House when they stepped on a hatch lid handle and twisted ankle resulting in a fracture.

Struck by flying object

Electrician was drilling metal earlier in the shift without incident. Upon task completion, EE was putting tools away when they felt something in their eye. Windy conditions appear to have contributed to the incident. Employee regularly wears prescription foam seal type safety glasses and was wearing them at the time of the incident.

Struck by falling object

IW leaned 36"" pipe wrench against a pipe. IW placed left hand on the floor to support themselves as they repositioned a brace. Pipe wrench fell over striking employee's hand. IW sustained fractures to the left middle and ring fingers. IW finished their shift and received no lost time.

Caught in, under or between a moving and a stationary object

The employee was rolling a piece of steel on a Webb plate bending role in the Maintenance Shop when EE's finger was caught between the plate and roll. The employee's left ring finger was crushed resulting in a laceration and fracture to the end of finger.

Contact with heat

Employee sustained a heat illness while working around #2 Raw Mill. Employee was treated by on-site first responders and local EMS. EMS administered IV fluids and transported employee to hospital for further testing. Employee was cleared to return to work.

Caught in, under or between a moving and a stationary object

Employee was changing the 7/8"" closing cables on material storage building bridge crane. EE's hand was caught between the cable and drum resulting in a fracture to EE's right hand and lacerations to EE's middle and ring fingers on Rt. hand.

Employee was clearing a material blockage from Finish Mill #1 dust collector hopper and came in contact with the rotary feeder resulting in lacerations and crushing to the tips of the 2nd and 3rd digits on EE's right hand.

Employee was clearing a plugged chute and manually removed a tool resulting in a strained lower back.

Struck against stationary object

Employee was switching a manual valve with sledge hammer and struck the protruding metal valve handle with EE's finger while swinging the hammer. Incident treated as a first aid. Employee went to personal physician (on own) and verbally informed the company on 11-19-18 of a fracture and prescription for antibiotic.

Miner was loosening a threaded union from a pipe with a wrench and as miner applied downward pressure the wrench slipped and miner's elbow struck a solid support in the work area resulting in contusion of the left elbow.

Maintenance employee was working on a pump in an area where the site had been cleared of vegetation and was exposed to poison ivy/oak. Resulted in dermatitis of the skin on the arms and abdomen.

Incident originally reported as personal medical. 3/19/2018, IE changed report to work related. IE was sent to company doctor, work comp claim was filed. IE was diagnosed with bilateral paresthesia. Work Comp claim was denied. IE pursued as personal medical. On 8/22/2019, IE refiled with work comp. Incident became reportable on 9/28/2020 due to scheduled surgery.

Miner was indoors removing a nut from a drive resistor box. The wrench slipped off the nut.

Maintenance department tried to move a non-working bridge crane down the rail by manually turning the drive with 48 inch pipe wrench. The event resulted in a strained shoulder to the maintenance employee because of the manual force applied with the wrench.

Fall onto or against objects

Worker tripped on a protruding piece of expanded metal that had been propped against the wall at the working area, near no.1, raw mill separator. Worker fell onto the expanded metal, lacerating the little finger on the right hand, requiring stitches.

Canopy roof collapse, employee made 15 minutes call to MSHA, an EE from the operator was checked for injury/illness. EE released with no symptom immediately. Contractors had two of their employees injured.

Over-exertion in lifting objects

EE changing the gas cylinder on mig welder machine, reported a mild sprain. Treated as first aid at the Clinic and diagnosed until recommended for surgery. Surgery on the 01/25/17

EE slipped off the rung of a fixed ladder and fell off causing fractures to EE's left ankle and contusion to the right ankle. EE will need surgery to the left ankle.

EE walking down the stair in FM 3 building and felt right knee popped. Conservative first aid treatment until surgery for torn meniscus on 11/09/16.

Air supply valves forcefully blew off while removing an airline from a compressor. The air hose had a contact with an employee's left ankle causing contusion, EE got a prescription drug by the doctor.

Employee claimed EE passed out due to heat. First aid treatment, allowed EE to cool off, drank water, provided electrolyte and chill it towel. Employee resumed work thereafter.

Employee right ring finger caught in between the trunnion end cap tapper and the oil stand pipe while installing the trunnion end cap on the SW pier 5 on Kiln 1. The finger sustained six stitches.

EE right middle finger had a contact with brass bearing when installing it into trunnion socket. The finger sustained four stitches.

The lid of the feeder air slide door around the V-valve on Truck Silo B blew releasing dust to an employee working around the area. Oral Steroid treatment administer but release back to regular duties.

Employee was driving 992C loader when he wanted to remove the metal strap on the vent which has an internal fan. His hand made contact with the fan which led to lacerations to his right middle and index fingers.

Muddy materials pushed EE to hand rail causing thigh sprain. EE felt it would get better and continue working until 6/26/2015 when he got scheduled to attend physical therapy 3x/wk * 3wks

Employee crawled under an air slide when he felt a pain in his upper left thigh and knee area. Employee felt it would get better and continued working until 4//27/15 when he went to the doctor who prescribed physical therapy.

The Otis elevator at the coal silos tripped out with an employee stuck in it. Another employee reset the breaker an the elevator continued down. It was determined that the gear box oil was thick due to the low temperatures that night. No mechanical issues were found.

Around 1 p.m. employee entered elevator and traveled about 12 feet up from ground level. Power cable caught and pulled out of control. Outer panels had to be cut so employee could get out. Incorrectly gave MSHA time of 12:15 when i called in to report entrapment. no injuries elevator still being repaired, waiting on report from elevator repair group.

Vac truck had a clogged vac hose from a rock. Employee was trying to relieve the blockage and was using a box knife to cut hose to get rock out. Knife slipped out of hose and cut employee on the right wrist (3/4"" long cut).

Employee was moving a ladder to reach a area that needed to be patched. Vac truck hose was in the floor in the area I was working in. I stepped backwards & tripped over the hose, fell down & hit a rock laying in floor with my left ankle. Employee didn't go to the doctor until later and was advised on 12-4-2012 the ankle was broken and would need to have surgery on 12-7-2012.

Employee stepped off of platform and bumped left knee on elevator guard. Did not seek medical attention until 2/2/2012. Returned to doctor on 2/7/2012 and was prescribed pain medication and therapy.

The injured employee was adjusting the tail pulley of the #1 secondary crusher pan feeder. The employee had completed the adjustment of the North side and was attempting to adjust the South side. While adjusting the South side the employee was pulling up on a wrench and experienced pain in his abdomen.

While repairing air leaks at the #2 Bag house, employee was hooking up an air hose with a crowfoot fitting to the building plumbing. The air pressure was off with no bleed through at the valve. The pipe was disturbed and dust and debris fell on him. His right eye was irritated and he rubbed it sticking the dust to his eyeball.

Employee exited overhead crane cab at end of shift. While traversing walkway to leave, an AC unit cooling fan engaged creating a draft. Employee was wearing safety glasses but some dust particulate entered his left behind them.

Employee was cutting the sidewall of a tire and as he cut, the knife slipped off the tire and punctured his thigh. Employee was driven to clinic by on-site supervisor where he received three stitches to close the wound.

Employee was removing liner plates from ball mill and hit a plate with hammer and slipped and it splintered and a piece of steel went into his left leg about 3"" deep about the size of a 22 shot close to an artery. He had another little piece in his right thigh.

The tipping valves (airlocks) feedling limestone from the conveyor belt above the kiln hood failed in the open position allowing heat from the kiln hood to travel up the chute to the conveyor causing the conveyor belt to catch fire.

Toolbelt and harness snagged under rung when EE was ascending the ladder, causing him to slip off rung. EE tried to hold his weight with right arm on handrail; this injured his shoulder.

Employee was trying to open tipping valve slide gate on LSI system to #1 Kiln. Gate broke loose and closed pinching right thumb between lip of slide and tipping valve housing.

Employee was assisting contractor with a plasma cutter. Contractor was turning equipment on and off in an attempt to make it work. Employee was holding torch in his hand when it fired unexpectedly.

Employee stepped out of a company pick-up truck and slipped on ice causing the employee to fall on to the door seal of the vehicle and then to the concrete landing on his tailbone.

Employee was welding on 3rd pier---welding stop block against tire. When pulling lead across catwalk, lead grounded against steel causing flash.

Employee hit the concrete ramp west of the fuel station (by the Kerosene tank) with a Ford pick-up. He was pulling into the fueling station while two clinker trucks were driving by the area just south that area. To avoid the clinker trucks, the employee turned hard into the spot, hitting the frame of the truck on the ramp edge.

Employee was working on a pump. Employee removed grease plug from a bearing under pressure. Hot grease shot out of bearing covering employees face and right hand. Employee sustained first degree burns to face, neck, head and right hand.

On 6-12-2006 employee reported a work related injury to his neck. On 4-19-2006 while running #2 crane, after several hours of working noticed neck pain.

Employee was placing scrap metal into a recycle bin. He noticed a length of conduit sticking out over the top of the container. He reached up and started to push it into the container. His left forearm grazed a sharp pipe that was also sticking up resulting in a cut to the left forearm.

Employee was trying to remove counterweight guide from bottom coal elevator. He used a torch to be able to stand out of the way while bolt was burned out. When bolt was freed, the guide came out and hit employee in forehead.

Changing valve and belt, the switched valve blew out stirring up dust and rusty water which sprayed into both eyes

There was loose material on the floor and it had snowed that morning. The employee slipped on the material and fell on his back and sprained his back.

MECHANIC WAS LOOSENING BOLTS ON DRIVE COUPLING WITH BRAKEOVER. PUT BRAKEOVER AGAINST CHEST AND PUSHED TO LOOSEN BOLTS WITH CHEST WHEN MECHANIC FELT SOMETHING POP IN CHEST. IMMEDIATE PAIN IN CHEST. REPORTED INJURY TO SUPERVISOR. WORKED REST OF DAY THINKING MUSCLE WAS STRAINED AND WOULD GET BETTER.
