Employee was cleaning up the bone yard and dragging a piece of angle iron, tripping over a pipe and injuring his left shoulder. He sought medical treatment on the same day and was released to regular duty. 10/7/15, we find that he went to the clinic again and was given restricted duty w/o the knowledge or approval of his case manager. Still has not contacted CM.

Employee was unloaded a piece of conveyor when he rolled his ankle, falling and hitting his cheek on the truck and hurting his back. On 4/24/14 he notified his manager that he had gone to the ER because he was having pain in his ankle/foot. It is fractured.

Employee was sitting in a Cat 988 with the window open. When he went to shut the window, the wind caught it, slammed shut, and cut the tip of his right little finger off.

Approx. 3:20, a grass wildfire was discovered on mine property. It had started on another property and spread onto mine property. Fire Department extinguished fire. No mine structures or people were endangered by the fire. MSHA inspector was already at the site when the fire was discovered.

Employee was using a wrench to replace a hoist cylinder on a Caterpillar 980F loader. The wrench slipped, smashing finger between cylinder and wrench.

Injury was cleaning the catwalk and steps. He slipped and fell, catching his hand on handrail. His little finger on left hand was fractured.

Time started shift is unknown. An employee found victim with RK Trucking parked at front gate. He said he had caught his ring on his truck & was waiting for wife to come get him. EE was positioning a piece of beam for welding. His left index finger was smashed between beam and saw horse. The tip of his finger was amputated and re-attached.

Crew shut down because of bad weather. They had waited 45 min for storm to pass but it showed no indication of stopping so the helper got out of work truck and went to the rig & turned off the master disconnect. As he was getting back into the work truck, lightning struck the work truck. Helper's right arm was hurt along with burn blisters on both feet.

EE WAS WORKING WITH OTHER EMPLOYEES TO STACK RAILROAD TIES ON A RETAINING WALL. HIS THUMB WAS SMASHED BETWEEN A RAILROAD TIE AND THE SUPPORT BRACE ON THE WALL. HE RECEIVED APPROXIMATELY 25 STI TCHES IN HIS THUMB. THERE WAS NO LOST TIME DUE TO THE INJURY.
