Struck by falling object

EE was installing buckets on an elevator when the EE's right hand slipped off the bar causing the bar to fall. When this happened the EE tried to catch the bar as it was falling and it ended up smashing EE's middle finger on EE's right hand. This injury resulted in the EE having to get five stitches in EE's middle finger.

Caught in, under or between running or meshing objects

EE was cleaning out a chute due to material being clogged. After EE unclogged the chute they communicated this to the control operator. When the material began to clog up again EE reached into the chute to clear the material. At this time the control operator closed the gate crushing EE's hand. EE received three stitches in their right thumb.

Over-exertion in lifting objects

Employee was lifting a 5 gallon bucket of hydraulic oil out of the back of a pickup on a cold day when EE turned and twisted causing a muscle strain in back.

Fall onto or against objects

While mounting a front end loader the employee's foot slipped striking EE's knee on ladder way. Employee reported at end of shift. Did not think EE needed any medical attention until 10/17/2019. Resulted in one day modified duty.

EE was using a airless paint sprayer to paint a building. As the EE was cleaning the nozzle, EE accidently pulled the trigger while EE's right index finger was in front of the nozzle, causing a laceration and getting paint in the wound.

Struck by flying object

EE was observing an EE removing an anvil from the VSI table during rebuild with a sledge hammer. As the anvil was struck, a sliver of metal broke loose and struck EE who was standing 10-15 feet away in the left leg above the knee, causing a small cut and puncture wound which required stitches. No Lost Time or Modified Duty.

Caught in, under or between a moving and a stationary object

While cleaning the threads of a conveyor belt adjustment bolt with a wire brush, the employees finger was pinched between the wire brush handle and the adjustment bolt cover causing a cut that required stitches.

Fall from machine

EE was standing on the platform of a Komatsu FEL when he was attacked by a wasp. EE was holding onto the handhold swatting at the wasp when he lost his balance and fell ~6' to the ground suffering contusions to his hip and back and arm strain from holding onto the handhold. The post-accident drug test results revealed that the EE tested positive for THC and opiates.

Struck by falling object

Checking for a certain screen cloth size. The screens were standing on edge. The screens started to fall. Employee tried to catch the screen. The edge of the screen caught the tip of the ring finger on left hand. Finger was cut. It required 3 stitches to close the cut.

Employee was removing an inspection door from a bucket elevator. The door slipped pinching the end of his left ring finger between the door and elevator frame, requiring stiches. No modified duty was required. Employee returned to work with no lost time.

Employee was cleaning a wet scrubber drain using a rotary drain cleaner when the cutting edge encountered hard mud causing the cleaning coil to twist striking the empolyee on the tip of his left ring finger, fracturing the end digit.
