EE kicked frozen outrigger pad with right leg. EE initially felt no pain but 8-10 hours later, noticed some pain and swelling of the right knee. EE was accompanied by safety to an orthopedic clinic for exam the following day where they were prescribed steroids and given no lifting/pushing/pulling restrictions pending follow up.

EE states they were grinding a handrail by VSI crusher when a piece of metal went into eye. EE stated they were wearing proper PPE for the task. EE sought medical treatment the following night where the foreign body was removed from left eye. EE did not report the initial incident or subsequent medical treatment to the employer.
EE was driving haul truck and began to experience lower back pain. EE was seen the following Monday on 9/22/2025 and provided a prescription anti-inflammatory and muscle relaxer. EE did not miss work or receive any restrictions.
EE states EE's hand was smashed by an unknown object when assisting with removal of welder from truck bed. EE stated on 8/7 and 8/8 that EE did not wish to seek medical treatment and no signs of injury were present. EE also stated that EE had previous injury to the same area of EE's hand. EE left site and sought treatment 8/9/25. EE resigned 8/20.

EE was tightening hammer bolts. EE was trying to get torque gun out of the crusher and dropped it on EE's pinky finger. EE went to local urgent care and a fracture on EE's small finger was identified.

Centrifugal pump moved approx. 5 inches where 2 Mideast Workers were working. They were welding box tubes on the side of pontoons. IE was on platform and other EE was on the boat welding. IE was lowering box tubes down to EE with a rope and EE would weld them . EE heard a loud bang.
Injured employee reported a ware plate fell and hit employee's knee. Employee was released to return to work on 5/13/2022.
EE was grinding rust from the primary sidewall under the crusher. EE finished grinding and proceeded to switch from clear glasses to burning glasses. In the process, grinding material fell into EE's burning glasses. When EE put the burning glasses on, material fell into EE's left eye.

Supervisor was using a wash down hose to clear material off a conveyor. When they turned the hose on, the pressure knocked them off balance. They stepped back and stepped down a 6 1/2"" step to a lower platform. This shifted all the weight to left leg injuring it. Fractured the tibia and fibula bones in left leg.

Employee was operating a pit loader in the South Pit. While loading a haul truck, a rock came off the bucket & struck the cab/windshield. The windshield broke & sent small particles of glass into the operator's eye. The glass was not embedded in eyes. The particles were flushed out of the operator's eyes at the ER & operator was prescribed antibiotics as a preventative measure.

A hydraulic cylinder was being replaced on the pedestal rock breaker. The cylinder was supported with a crane and a lever chain hoist. An employee's hand was pinched in a pivot point at the rod end of the cylinder. A laceration to the employee's right hand required 9 stitches to close the wound.

Employee was walking the plant. While descending the stairs from the 4 1/4 crusher to the floor below EE slipped and fell onto the steps. Employee struck left elbow on the edge of a step causing a laceration to the elbow.
Employee was operating a Cat 740B haul truck engaged in stripping activities. While traveling West, down a grade, empty, the truck veered off the right edge of the roadway causing it to slide and then overturn. The employee had severe lacerations above the left eye and behind the left ear. The truck sustained severe damage to the left side.
Employees were removing chute work below the #7 screen. While supporting the chute with a boom truck the hardware was removed from flanges to split the chute at a joint. After removing the hardware a hammer was used to strike the chute to dislodge it. When the chute came apart it shifted to one side breaking through a hand rail and striking the employee on the right foot.

Employee was operating a John Deere 644K wheel loader. Employee was using the loader's bucket to scrape the floor in front of the face when the bucket struck a toe area in the floor. This caused a jolt that suddenly rocked the machine from side to side. Immediately after the jolt the employee felt pain in his lower back, left hip, and left leg.

I was fusing 12"" HDPE pipe. While working the hydraulics, I got my left hand in the wrong spot pinching it between a block of wood and clamp on the fusing machine. Broke my index finger on left hand.

Hot ember from cutting torch smoldered on a urethane screen panel. Decks caught on fire in #4 screen doing damage to the screens and chute work. No injuries to any employees.
2 fire watchers armed with extinguishers didn't see signs of fire during hot work. after saw flame 30' below on rubber screen, attempted to trigger the process water to the plant - not turned on, tried extinguishers - no help. called fire department. 2 screens, motor & electric wires burned.
EE was removing a sheave from an electric motor. The pusher bolts had broken off, While ee hit the sheave with a hammer, He misjudged the placement of his left hand and got his left thumb struck between the sheave and the hammer. The Dr. made a hole in his thumb nail to relieve the pressure.
4/2/13 1st aid now a reportable. UG utility miner hurt while repairing a pump in underground mine. Rock in rib wall by the pump tumbled and hit the employee in his right rib cage. Made a 2"" bruise. Reported the injury to mine manager & was asked if he needed treatment and he said he was OK. 8/12/13 still affected by the injury, employee taken to doctor. X-ray showed broken rib.

EE was walking from the plant to the shop to get on a piece of equipment.When he walked by our #4 screen,Something bounced from the screen and got in his eye.EE reported the incident to management at the end of his shift.he did have a scratch on his lens.He was released to work immediately with NO restrictions by dr's.

EE attempted to climb into the bed of his sterling dump truck when he lost his balance and fell to the ground. EE suffered a broken left arm (upper). EE was transported to Mt. Carmel west hospital.
