Shuttle car operator stopped to talk to the shuttle car operator in the crosscut behind him. Scoop operator in crosscut in front proceeded to cross in front of shuttle car. Shuttle car operator proceeded with the buggy towards the feeder running over the scoop operator in front of him.

He was pulling rocks off of the miner when a piece of rock slid off lacerating his right forearm. He received 8 stitches.

Stone struck foot while prying fallen rock off of continuous miner with 5 ft long stone bar Transported to ER by Mine foreman and treated for bone chip. Xray later confirmed fracture on 11-21-2011

Employee was setting up sights on section. Noticed that the roof was flaking, so he started to crawl out. That is when a rock fell from the roof and landed on the employees foot.

Employee was trying to move a johnson(mantrip) trailer out of the way of a scoop when the scoop tire caught the edge of the trailer and smashed down on his leg twisting his ankle.

Roof fell on 23 break on 4 belt. It was on the left hand side of an intersection towards #2. It was 17 feet wide, 6 feet high, and 25 feet long.

OPERATING REAR MOBILE ON #1 SECTION-HAD RIGHT HAND ON THE OUTSIDE OF THE KITCHEN-SMASHED THE TIP OF HIS RIGHT MIDDLE MIDDLE FINGER BETWEEN THE MOBILE AND THE LOW TAIL.

Accident type, without injuries

Roof fall in #2 entry of the mine mouth. The fall measured 4' high, 4'wide, and 15' long. The fall opened a gap between the highwall and fan housing. No equipment was damaged or injuries occured b/c of the fall.

Over-exertion in lifting objects

Employee was in the underground parts hole trying to lift a miner shear jack onto the two man trip. He felt a sharp pain in his lower back and lower extremities. Employee did not go to the doctor that day. He went on January 13th, 2009.

Roof fall in #3 heading of 4 left in the 1 E Butt. Fall was 30' wide, 50' long, and 12-15' high. Miner operator was turning crosscut 3-4 when a roof fall occurred. Operator dove under boom for protection, however some rock fell on his right leg.

Employee was going outside to get supplies. He was pulling mantrip into crossuct and front tire hit the rib spinning the steering wheel. Steering ball on the wheel went between the middle and index fingers.

Employee was operating roof bolter. He was holding drill steel in top. Swung canopy and hit hand pinching it between the drill steel and canopy.

Employee was operating a twin boom roof bolter. Hand was on the drill steel top of pot. Employee started the rotation before removing hand from drill steel. Corner of the pot caught his glove wrapping his hand around the drill steel in the pot. All employees were instructed about placement of hands while drilling.

During a belt and power move, two employees were setting a feeder by using 4' x 8' headers. While doing this, employing felt a pull in his back.

Employee was assisting in moving the miner cable and felt a pull in his low back while lacing the miner cable along the rib. Employee continued to work. First day missed was 2/18/05.

Employee was setting a beam on the canopy of the roof bolter to support it to the roof. The beam slipped off the other side of the canopy causing the side the employee was on to raise into the top smashing his thumb between the beam and roof.

While getting out of the deck of the roof bolter and crawling towards the side controls, employee hit his head on a low hanging roof bolt.

A roof fall occurred at no. 6 crosscut of ""0"" entry at a 4-way intersection. This intersection had been re-bolted with 8' cable bolts; however the fall occurred in a horseback above the anchorage point of two of the cable bolts. The remainder of the area is intact. The area that fell was approximately 12' x 12'. Contributing cause was the ongoing squeeze.

A roof fall occurred at No. 7 entry at No. 7 crosscut. This was a continuation of the previously reported fall at No. 6 crosscut. Area was bolted with 5' fully grouted bolts. The ongoing squeeze broke off against the solid. The fall was 18' wide by 50' long.

Employee was backing the miner out of a cut and went inby the last row of roof bolts. A slab of stone (horseback in roof) fell from roof and landed on employee. Off work from time of injury to Oct 25th; restricted duty from Oct 25th to Jan 17th per call to company. coder 12.

WHILE UNLOADING 10"" STEEL CHANNEL SUPPLIES FROM SCOOP CAR TO BOLTER, THEY SLID FORWARD. EE'S HAND CAUGHT BETWEEN THE STEEL AND THE LIGHT GUARD ON BOLTER.
