Drowning

The employee was performing work activities on a sand dredge to free a clam shell bucket from material in a lake when the dredge capsized.

Struck by flying object

Employees were realigning the tracks on a CAT 235C excavator. As the operator jogged the machine forward, a small piece of metal broke of the sprocket and struck an employee in the right forearm.

Employee walked to haul unit upon start of shift, climbed up steps of truck and sat down in seat to begin preshift inspection. Another employee noticed that employee was in distress, ERP was initiated and employee was eventually taken via ambulance to ER where he was pronounced dead. Causes of death on coroner's report are Coronary Atherosclerosis and Adenocurcinoma of Lung.

The employee radioed the plant operator shortly after beginning his shift to complain of chest pains. He was operating a haul truck and was under no stress at the time. He was transported to the hospital by EMS (as directed by the conpany's emergency response plan). Doctors later diagnosed a heart attack. This event was in no way work related.
