Absorption of radiations, caustics, toxic and noxious substances

Employee was performing security guard duties when a bug went into EE's right ear and bit EE. EE experienced tingling in EE's ear and had difficulty breathing. EE was taken by ambulance to a local hospital for treatment and was later released.

Loading coal samples in truck, lifted sample weights of approximately 30-40 lbs, began to feel some lower back pain upon returning to vehicle and driving.

Employee was pre-oping and climbing off the #10 dozer D9L as his foot hit the track he fell fracturing a rib. The track had ice and mud on it.

EE UNTIED STERN LINE FROM KAVEL ON STERN DECK OFHARBOR TUG BOAT. WHILE WALKING SIDEWAYS, CARRYING STERNLINE, HE INADVERTENTLY STEPPED OFF STERN DECK AND FELL AGAINST STERN SIDE GUARD RAIL. THE STERNDECK IS ELEVATED 2 FEET ABOVE TUG FLOOR.
