Struck against stationary object

EE was shoving top soil away from the highwall. He backed up over and through an 8 foot berm that had been constructed at the edge of the highwall. The dozer went over the wall rolling over and throwing him out of the cab.

Employee was working on a side auger bulk unit and had hand on a hydraulic ram. When the employee moved the ram it closed completely, catching the employee's hand between the ends giving the employee a fracture and lacerations.

Employee was under a bull dozer changing a hydraulic hose. He was trying to bend it up in place and it slipped out of his hand and hit him in the forehead causing a cut - 4 stitches.

Injured employee was attaching a tow cable to his vehicle and in the process slid his hand down cable receiving a cut which required 6 stitches to close.

Our stripping shovel caught fire from an electrical short down in the tub of the machine. The short caused the electric line to melt and ignite grease that is inside the tub. Major electrical damage.

Employee was walking across a muddy spoil bank to get to a water pump and slipped and twisted his knee

Employee was replacing teeth on a 992 loader, he was carrying one over to the loader and slipped and fell hitting his shoulder on the ground. He has worked with a pain in his shoulder, now he has decided to have it repaired. Doctors have told him that he has a small tear in his rotator cuff.

EE was welding on a 992 bucket and at the end of his shift was picking up his tools and scrap metal and placing it in the back of his mechanics truck, he hit his knee on the bumper and bruised his knee cap. Lost 1 day of work.

Accident type, without injuries

EE was running his D11N dozer and another ee was working beside him. EE noticed smoke coming from the right side of ee dozer then it erupted in fire. EE shut down the dozer and went out the left door away from the fire. No one was hurt. Fire department was disbatched. Extensive Damage due to the fire was under the cab where you cant get at it.

Over-exertion in lifting objects

Employee lifted a 3"" water pump to set it up to pump water and when he turned he injured his back.

Fall from machine

Employee was standing on the stinger of a 657 scrapper power washing. The hose of the power washer become tangled under the tire of the pan and EE pulled on it. When it came loose it caused him to lose his balance and fall backwards about 4'6"".

Struck by flying object

EE was using a 10Lb. sledge to knock a corner protector off a loader bucket, when a piece of steel flew off the protector and hit his right leg. A doctor had to remove it.

EE was operating a D10 heading towards the north west face. EE(Forman)was traveling back to get with EE when he noticed the D10 had backed over a steep spoil pile. When the foreman opened the cab door he saw that EE had hit his head . He received 5 stitches

Injured was lowering a belly pan down on a 475 Komatsu dozer when his pry bar slipped and hit him in the cheek. Had 3 stitches.

Injured was climbing up onto his Front End Loader and didn't see that the engine compartment door had come open and he ran into it causing a cut on his cheek that took stitches.

A Oil line broke down under the cab of a D11R dozer. The oil was ignited by short in a wire in the wiring harness. The fire was put out twice but kept igniting. No was hurt but the Dozer burnt up.

EE was climbing onto a Komatsu 475 trackhoe to start it when he hit his knee on the track. He reported it to us and he said it wasn't bad enough to go home. He never missed a day till 1/14/08. He has went to doctors and they done a minor surgery on it. EE will be off for couple weeks.

EE was putting hose for treatment of water in ditch, when the hose momentarily got caught on pole that holds the hose when not in use. When the end of the hose came loose it shot water into his right eye. Took him to hospital, was told to take 4 days off.

An employee was descending down a front end loader and at the bottom step he turned while stepping to ground, lost his balance, twisted his knee and fell to ground.

An employee was sitting in his rock truck being loaded by an electric shovel when the door of the bucket on the shovel accidently opened while swinging towards the rock truck striking the side of the bed of the truck in a forceful manner.

EMPLOYEE WAS BACKING THE WATER TRUCK DOWN A PIT RAMP IN LOW RANGE WITH THE ENGINE RETARDER ON AND APPLIED THE BRAKES. THE PEDAL FELT SOLID THE FIRST TIME BUT THE SECOND TIME HE APPLIED THE BRAKES AN AIR LINE POPPED OFF AND HE LOST HIS BRAKES AND AIR. TRAVELING AT A HIGHER RATE OF SPEED IN REVERSE, THE BACK DRIVER'S SIDE DUALS CLIMBED UP THE BANK AND FLIPPED OVER.

EMPLOYEE TURNED TO WALK TOWARDS HIS LOADER AFTER BREAK TIME AND TRIPPED OVER HIS FOOT, LANDING ON A ROCK ON HIS SIDE.

EMPLOYEE WAS ADJUSTING THE 6 INCH WATER HOSE BY PULLING ON IT AN FELT A PAIN IN HIS LOWER BACK.

EE WAS REPLACING A LOADER TOOTH AND WHILE STRIKING THE STEEL PIN WITH A HAMMER, A SLIVER OF STEEL STRUCK HIM IN THE LIP AREA.

EE WAS WALKING OVER A HILL IN THE DARK AT 10:30 AT NIGHT TO CHECK THE DISCHARGE HOSE OF A WATER PUMP WHEN HE SLIPPED ON THE ICE AND LANDED ON HIS BACK AND NECK AREA. ***THIS ACCIDENT WAS NOT TURNED IN FROM THE EE UNTIL FEB. 18TH, 2004.***

EE PARKED THE FUEL TRUCK 50 FT AWAY FROM THE ROCK TRUCK DURING LUNCH BREAK. AFTER BREAK, TRUCK OPERATOR BACK UP NOT KNOWING THE FUEL TRUCK WAS THERE. THE ROCK TRUCK SLAMMED INTO THE BACK OF TH E FUEL TRUCK RIPPING THE BUMPER OFF. JUST AT THAT TIME. THE ROCK TRUCK TURNED HIS WHEELS AND PULLED AWAY.

EE WAS DRIVING BY A 6"" WATER PUMP ON A DARK AND BITTER COLD MORNING. AS HE SHINED THE LIGHT HE COULD SEE STEAM COMING FROM THE PUMP. ASSUMING THE PUMP WAS ABOUT TO BURN UP, EE STOPPED THE CAR AND TOOK OFF RUNNING TO SHUT THE PUMP OFF. EE SLIPPED ON THE ICE LANDING ON HIS SHOULDER DAMAGING HIS ROTATOR CUFF.

ACCORDING TO EE, AT APPROX. 12:30PM A LARGE PIECE OF COAL BECAME WEDGED ON THE CONVEYOR, WHILE EE WAS ATTEMPTING TO FREE THE COAL HE EXPERIENCED A PULLING/STINGING SENSATION IN HIS LOWER BACK . HE DISCUSSED THIS WITH ASST. SAFETY DIRECTOR. WHO ADVISED HIM TO LEAVE THE MINE AND SEE HIS DRIMMEDIATELY. EE WORKED THE REMAINDER OF THE WEEKAND REPORTED OFF ON 1-14-02.

WHEN THE STORM MOVED IN THE AREA WAS CLEARED ANDAT 4:20 LIGHTENING STRUCK THE BENCH THAT WAS READY TO BE DETONATED. THE LIGHTENING SET OFF 11 HOLES PREMATURELY.
