Fall from ladders

Miner alleged he was climbing ladder on floating conveyor. Ladder measured about 5.5feet high. He allegedly lost footing and fell. Upon investigation miner does not remember any more information and this incident was not witnessed. Diagnosis was a concussion and lumbar contusion.

While walking thru the plant area and pointing and giving direction to another employee who was operating a piece of equipment, IW tripped falling on his left knee.

Employee was holding a spud bar while another employee was striking the bar with a sledge hammer to break a rock from crusher. The hammer missed the spud bar and struck the employee's left wrist.

EMPLOYEE WAS CLIMBING INTO THE CAB OF TRUCK WHENHE PULLED A MUSCLE IN HIS BACK.
