Struck against a moving object

Employee was operating an off road articulating truck, the employee backed into the berm to dump the loaded truck, when dumping the front of the cab raised up and articulated causing the cab to strike the ground.

Worker tossed an 8 inch 2x2 angle iron into the scrap bin, and his right hand struck the vice mounted on the back of the truck. He suffered a right hand 5th Metacarpal Fracture.

Using a spud bar to clean out a chute on the S2, reaching in with the bar through an access panel. Bar slipped and right hand was caught between bar and the edge of the access panel. A severe avulsion of the fifth finger from the second knuckle and a small abrasion to the fourth finger on the right hand.

Struck against a moving object

Description...Operator was digging into a frozen #4 aggregate pile when the corner of the bucket stuck in the frozen material. The operator went to curl the bucket and lift material, but due to the frozen material, the back end of the loader came up. The operator pushed controls forward and the machine came down hard, jarring operator's back.
