Absorption of radiations, caustics, toxic and noxious substances

Employee had a hand on a cluster of hydraulic lines, troubleshooting a leak. Employee grabbed a hose, which was lying up against the frame of the truck and pulled it away, when oil shot out and penetrated the right middle finger. Employee was wearing gloves and had two rags wrapped around the hose.

Employee was operating a Cat D400E Articulating Truck constructing a stream channel when the employee became stuck. While a dozer was assisting the employee, the tractor tipped over. Employee received a small cut on left arm requiring 2 stitches. Operator was wearing seat belt.

Accident type, without injuries

Operator had just dumped the load and heard a loud thud and then fire came up around the cab, operator put machine in park and applied the parking brake and exited the machine. Fire was too big to fight so EE got away from it.

Employee was back dragging a drill bench when he backed over an embankment and roll his dozer over onto its side.

Fall from machine

Employee was stepping off a D-6 push arm and slipped on ice causing him to fall about 2 feet to the ground. Employee suffered a hairline fracture in his wrist.

Employee was getting into his dozer and slipped causing the door to slam shut on the very tip of his middle finger breaking it.

Fall from machine

Employee was changing oil filters on his loader and slipped and fell injuring his arm. He fell 38 inches to the ground.

Employee was driving a pin into the track segment and a piece of metal came off and hit another employees arm causing a cut that required 5 stitches.

Over-exertion in lifting objects

Employee picked up a drive shaft out of a D-9, he lifted it up about 2 feet and said he hurt his neck and shoulder.

Employee was running a loader and was thrown from his seat into the upper part of cab hitting his head on a radio causing a cut requiring 4 stitches.

Employee was ripping rock with a dozer and when he went to back up the left side track dropped in a 3 foot hole causing a jolt to the operator.

Employee was operating a 777 Cat rock truck and was being loaded by a 992 cat loader. He claims a rock slid over and hit the inside of the bed and jarred him. Went to Twin City Chiropractic center and they gave him 3 days off.

EE was helping our mechanic change a ride cylinder on a 777 Rock Truck. EE was holding a B&O and the mechanic was using a sledge to drive the pin out of the cylinder. The sledge hit the B&O and bounced off and hit EE's hand, causing a cut that required 5 stitches.

EE was chaining a 992 bucket to the loader and stepped on the spair chain that was on the ground, he turned his ankle over and when he did it pulled a muscle in his back.

Injury was laying perferated plastic pipe for drainage. The pipes are in 10 foot sections and fit very thightly. He was turning the pipe to make sure the drain holes were lined up when the pipe was shoved together, His right index finger was cut off right behind his finger nail.

EE was helping install a Cylinder Yoke Bushing in a D11R. The bushing did not want to move, so they hooked up a battering ram from the machanics boom to force it in. The ram hit the back of EE's hand and took skin off up his arm. EE is off work for fear of infection.

EE was putting a swing motor into a PC750 Trac Hoe. The gear wedged itself in the mounting hole and when he tried to straighten it up it slipped and pinched his thumb between the gear and mounting bracket. Went and had 5 stitches. Worked the next day.

While helping to install belly chain drive shaft, employee injured his left thumb when a guard swung down and hit his thumb while he had hold of the shaft. He was treated at a Medical facility. Had to have stitches.

Suction hose on a 4 inch water pump become covered with dirt, causing it to have some pressure on it. When EE unhooked hose from pump, it flew up and hit him in the nose. EE had to have 4 stitches.

Was helping lube man change oil in dozer,needed a tool that was on hood of dozer ,climbed up to reach tool and slipped, on the way down hit his arm on a door that was open . went to hospital , he required 7 stiches

Was leaning down under a radiator putting bolts back in the supports, when trying to get back out he pulled on an air hose which in turn pulled air gun down on his head.

While employee was climbing up equipment at start of shift, his footing slipped. He felt a pull in his shoulder and neck while still having ahold of the hand grab bar.

Employee was climbing down off of the platform of a Cat 5130 Excavator attempting to put his foot on the first rung of the ladder when his footing slipped off the rung and the employee fell to the ground landing on his feet.

Leaving at quitting time - EE was walking up a grade to his car at a fast pace to get out of the rain when he slipped in some mud and fell.

EMPLOYEE HAD MUD ON HIS FEET AND SLIPPED ON THE PUSH BEAM OF THE HIGH WALL MINER AND FELL TO THE GROUND CATCHING HIMSELF WITH HIS LEFT HAND AND INJURING THREE FINGERS.

EMPLOYEE WAS TRAVELING DOWN A SLIGHT GRADE WITH A TURN TO THE RIGHT AT THE BOTTOM WHEN HIS FRONT TIRE SLID AND THE BED OF THE TRUCK TO JACK KNIFE SIDEWAYS CAUSING THE FRONT END TO STAND UP ON ITS NOSE THROWING HIM FOREWARD IN THE CAB. SEAT BELT WAS WORN.

EMPLOYEE BACKED A 150 TON CATERPILLAR 785 ROCK TRUCK INTO THE FRONT OF A 85 TON KOMATSU 785-3 ROCK TRUCK KNOCKING INJURED TO THE GROUND. per call on 11-16-04, there were multiple serious injuries. Broken bones in head, neck, back, blood on brain, bruised lung, head lacerations- coder 12.

EMPLOYEE WAS REMOVING THE DISCHARGE HOSE FROM THE WATER PUMP AT THE QUICK CONNECTION WITH APPROX. 900 FEET OF WATER THAT WAS UPHILL BEHIND THE CONNECTOR WHEN ALL AT ONCE, THE ALUMINUM CONNECTOR FLEW OUT OF HIS HANDS AND HIT HIM IN THE HEAD AND KNOCKED HIM TO THE GROUND.

EE WAS BUILDING A ROAD PUSHING TREES OVER IN AN OPEN CAB JOHN DEERE 550 DOZER WHEN A TREE BRANCH ENTERED THE CAB AND PINCHED HIS LEFT HAND AGAINST THE CONTROL LEVER.

WHILE EMPLOYEE WAS DRIVING A WELDING TRUCK TO WORKSITE IN THE RAIN ON A MUDDY HAULAGE ROAD, HIS TRUCK SLID OFF THE ROAD AND HIT AN EMBANKMENT. AT THAT TIME A KOMATSU 785 ROCKTRUCK WAS FOLLOWING AND PUT ON HIS BRAKES, CAUSING HIM TO SLIDE INTO THE WELDING TRUCK WHOSE DRIVER BUMPED HIS HEAD.

EE WAS LUBRICATING STEERING CLUTCH LINKAGE ON DOZER. CAN CAME INTO CONTACT WITH HOT WIRE FROM BATTERY TO PANEL WIRE HARNESS CAUSING A SHORT WHICH BURNED A HOLE IN THE CAN & CONTENTS ESCAPED UN DER PRESSURE & CAUGHT FIRE. SECOND DEGREE BURN TO EE'S RIGHT HAND.
