Struck by falling object

While adding a steel the steel fell out of the top of the loader. The operator thought they were able to get the steel back into the loader properly. The operator then threaded onto the steel to make the next connection. The steel came free from the rotary head & dropped approx. 2ft onto the drill table & then slid off the table & dropped 3-4ft striking the operator's ankle/foot.

Fall onto or against objects

While descending off of loader, miner lost footing. Miner grabbed with left arm and swung around and hit right shoulder on the counter weight of the loader.

Disassembled the gassing & acid hoses, the acid & gas went leaked through the gloves, 1st & 2nd degree burns on both hands
