Fall from machine

LUBE MAN WAS GREASING CRUSHER WHEN HE SLIPPED OFF THE DECKING AND FELL OVER THE HANDRAIL LANDING ON HIS BACK APPROXIMATELY 4 FEET BELOW ON THE GROUND

Struck against a moving object

Employee was traveling up a grade when he ran out of fuel stalling the engine. Without maxi brakes that lock up when depletion of air the truck could not start, it started to coast backwards and struck a birm and rolled onto its side. Employee was wearing his seat belt.

Employee was descending down the ladder of an end dump and slipped on the second step from bottom and fell to ground.

EE WAS ATTEMPTING TO REMOVE THE DISCHARGE HOSE FROM A 4 INCH WATER PUMP WHILE THE HOSE WAS STILL UNDER PRESURE. ONCE THE CLAMPS WERE RELEASED, THE METAL END OF THE HOSE SPRUNG BACK AND HIT EE IN THE MOUTH.
