Miner was pulling in a dredge cable when a tiny piece of the cable inadvertently punctured and entered the miner's left knee and lodged itself between his knee cap and tibia.

Miner was turning off pump on dredge, turned to walk back to cab and stepped in man hole falling partly in (one leg), hitting his left side.

Struck by flying object

After cleaning rust from steel plate, employee enter cab of his endloader then noticed an irritation to his right eye. Employee was taken to the nearest medical facility where particles of sand and dirt were removed. Employee was wearing safety glasses.

Over-exertion in pulling or pushing objects

EE stated to me om 3/2/06 at 1:30 pm that his back was hurting. He said he injured it the week of 2/20/06 sometime. He was house keeping around the river tunnel for long periods of time. That week he pulled a muscle due to the laboring activities.

GROUND SOFTENED BY HEAVY RAINS GAVE WAY UNDER EMPLOYEE'S WEIGHT CAUSING HIM TO FALL ABOUT 3' TO WORK FLAT BOAT BELOW, BRUISING LOWER PART OF LEFT LEG.
