Sledgehammer striking injured miner's left index finger

While cutting pipe to put in dumpster the pipe unexpectedly rolled in an unexpected direction, onto the miner's ankle

While walking miner felt a pull in the left calf area.

While moving rock box to align bolt holes cut left hand palm.

Lacerated left hand index finger while handling 6x2 worn feeder plate.

Employees were using Lull to lift rollout box when it became hung. Employee had Lull operator set it back down and tilt the Lull to the side to try and free it up. Employee tried to look to see why it was still hung up and the box came loose striking the employee in the leg/knee. Fracturing the back of the left knee.

Cutting conveyor belt with utility knife. Knife slipped cutting pinky finger & ring finger.

Employee was traveling roadway; says loader tire hit pothole and seat bottomed out and hurt his back.

We were made aware on 3-16-05 per dr that ee now has reportable injury sustained contusion on right elbow. Hit elbow on floor plate on side wall when handing a welding helmet to witness.

WEATHER CONDITIONS WERE WINDY AND CAUSED PONTOON TO MOVE AND PIN EMPLOYEE'S HAND BETWEEN PONTOON CAP AND CABLE.

EE WAS TIGHTENING HOSE CLAMP WITH SCREW DRIVER. CLAMP SLIPPED OFF HOSE CUTTING HIS RIGHT RING FINGER.

EE WAS WALKING DOWN SURGE BELT CATWALK WHEN HE SLIPPED ON CATWALK & CAUGHT HIMSELF WITH HIS ARMS & STRAINED HIS LOWER BACK.

EMPLOYEE WAS ATTEMPTING TO SHOW A NEW EMPLOYEE HOW TO INSTALL A FLANGE ON A PIECE OF PIPE. THE FLANGE SLIPPED STRIKING HIS LEFT THUMB.

CHAIN WAS HUNG UP ON COMEALONG, EE WAS PULLING ON CHAIN, TRYING TO GET IT UNHUNG.

STEPPING FROM ONE PONTOON TO ANOTHER WHILE CHANGING A PIPE. A FOGGY MORNING, DEW ON PONTOONS. SLIPPED & FELL INTO LAKE.

SNOW ON GROUND EE WAS WALKING DOWN METAL STAIR, SLIPPED & FELL STRAINING THIGH

COLD WEATHER EE WAS GRINDING ON SCREEN TUBE. GOGGLES FOGGED UP. EE RAISED GOGGLES TO INSPECT WORK. SOMETHING FELL INTO LEFT EYE.

EE WAS CUTTING FLANGE OFF PIPE WHEN HOSE BURST, CATCHING EE'S GLOVE ON FIRE.
