Employee went inside of robotic palletizer guarded area without de-energizing the control panel. Employee placed a hand in front of bag sensor to remove sand build-up. Robot moved and struck employee in the back/side.

Employee climbed out of track hoe and stepped to the ground. Employee felt a sharp pain in the back and right leg.

EE was stacking 50# sand bags on a pallet. When EE turned to place a bag on the pallet had a sharp pain in the abdomen. Later in the day EE had a lump protruding from the same area. EE can push the lump back in. *On 03/11/2020 EE saw a doctor & was diagnosed with a hernia. EE plans to have the hernia surgically repaired. The doctor issued a Return to Work slip with No Limitations.

EE was trying to break frozen chunk of sand that was lodged in feeder opening. When tried to hit chunk with sledge hammer, hammer struck steel brace, bounced towards EE causing the end of hammer handle to make contact to the EE left leg near his knee. EE was taken to the ER for X-rays. No broken bones were found. EE returned to work. EE saw dr 03/02/15, given restrictions.

Miner was changing the shaft and bearings on the head pulley of the 40/70 bucket elevator. He and another miner were trying to place a pry bar between the belt and the pulley to position the belt. The miner' s finger was pinched between the pry bar and the belt.

Miner was loading 6"" PVC pipe into the back of a plant pick-up truck when his right hand moved over a burr on the pipe. The miner was not wearing gloves and sustained a laceration to his right hand. The miner was treated at Knox County Hospital where he received a tetanus vaccination and 7 stitches to close the would. He was released to full duty.

Employee was positioned incorrectly when he was catching 100# bags off of the slow moving #3 product discharge conveyor belt. He caught his rt. thumb between the conveyor belt & discharge table. EE immediately jerked his thumb out causing a laceration to his thumb. EE was taken to the Dr. & received 12 stitches on his right thumb.

WHILE DRIVING SLOWLY ON A FORK LIFT, A UNKNOWN PIECE OF DIRT/DEBRIS STRUCK LEFT EYE CAUSING AN ALMOST IMMEDIATE IRRITATION. EE WAS WEARING SAFETY GLASSES AT THE TIME. IRRITATION OF LEFT EYE L ASTED ALL DAY & INTO EVENING. EE SOUGHT MEDICAL TREATMENT AT LOCAL ER ROOM

EE WAS HELPING TO CVHANGE OUT A FAILED DOUBLE TRUNNION BEARING ON THE LEACH TUBE. AFTER REMOVING THE TRUNNION, EE APPARENTLY STAYED BEHIND TO CHECK THE TIGHTNESS OF THE PINION BEARING BOLTS. A LL OTHER EES HAD LEFT THE AREA. WHILE APPARENTLY CHECKING THE PINION BOLTS, EE FGELL APPROX. 6', BREAKING HIS RIGHT WRIST.
EE SLIPPED AND FELL WHILE STANDING ON A PIPE. THE PIPE WAS COVERED WITH SAND MAKING IT A SLIPPERY SURFACE. HE FELL APPROX 2' AND FELL AGAINST A STEEL CROSS MEMBER, BRUISING HIS RIGHT SIDE RIB AREA. HE WAS PULLING ON A CABLE HOIST WHEN THIS OCCURRED.
WHEN REPAIRING A WARMAN SLURRY PUMP, THE FRONT COVER OF THE PUMP SLIPPED OUT OF EE S HAND AND FELL AGINST AN ADJACENT TANK, CUTTING HIS RIGHT MIDDLE FINGER. THE CUT TO THE FINGER REQUIRED FIVE SUTURES.
