Struck by flying object

While using a hammer and chisel to knock out bolts after being torched, IE received a sliver of metal in the web of EE's hand between the thumb & index finger. Cause of this injury was awareness of this employee. Compliance with company policies & PPE may be a factor along with miner proficiency. Regulations/mining equip/systems, not a factor.

The employee was taking the draw bar out of an off track lowboy. The motor pulling in bumped the lowboy and caused it to roll over EE's foot before EE could get out of the way. The employee was operating a motor. EE was pulling up to a parked motor when EE bumped into it and claimed to feel discomfort in the lower back. Cause of this injury was not paying attention to task at hand. Filing under protest to comply with a citation from MSHA. An employee was tramming a motor through the South Mains Dip. An unattended motor that was up the hill drifted down and caused the collision. 1 employee was entrapped in the #2 Portal elevator from 8:30am to 10:15am. A capacitor went bad causing the electrical system to shut down. The capacitor was replaced which corrected the problem and the elevator was put back in to service.

Stone fell in the walkway and impeded travel at 42xc of the South 60 Return just inby the #1 Left Side Seal. It did not fall above the anchor point. Stone was in the walkway for 15' and the area was supported with floor to roof support the next shift. Weathering of the roof was determined to be the cause.

The employee was operating a scoop car when it ran over a bolt that was buried in mud. When the tire ran it over, the bolt flipped out of the mud and struck employee in the lip. On 12/22/15 a roof fall was discovered in the 1 Left 3 East air course 1 crosscut outby the #2 seal. The fall was approximately 70' long X 20' wide X 14' in height. The cause of the fall was determined to be weathering.

The employee was carrying a bundle of 5 roof bolts to supply the bolting machine. When he lifted the bolts to place them in the tray, he felt a pain in his left shoulder. The employee was removing stone from the base of a fall area when a stone slid off of the pile of loose stone and struck him in the left hand. This caused a contusion on the miner's hand. On 7-30-15 a report of rock down, but no indication of a fall above the anchorage of the bolts, until 8-10-15 at which time it was reported as a roof fall. Weathering was the cause of the fall. The area was bolted with 8' bolts with resin at the anchorage point. The fall is approx. 35'L X 16'W X 9' in height. It did not obstruct ventilation.

Employee was walking down a set of steps while maintaining 3 points of contact. The miner missed the last step in the set and fell forward striking his side on a hand rail. He took his boot off, no damage was seen, so he put his boot back on and finished shift. After the shift was over, he decided to go to hospital. His toe is not broken but infected.
