Fall onto or against objects

Employee was entering cab of dozer and didn't see that floor board was missing. Employee's foot went in the hole and employee caught right side of rib cage against steering clutches.

A pry bar slipped while attempting to align a pin. The bar hit the employee in the chin and resulted in a minor laceration requiring stitches.

Operator was shoving dirt out with a D11T Bull Dozer and noticed smoke coming from engine compartment, EE jumped out and used the fire extinguisher on the fire but it was out of control so EE got off the machine and walked away.

Employee was getting a fuel hose to fuel a loader and slipped on ice and snow pack, employee stuck hand out to catch self. Employee's hand landed up against the ladder bending index finger back and fracturing it.

Operator noticed that a culvert was plugged up on one end so ee got off machine to go and shovel it out. On ee way ee twisted ankle on uneven ground on the pit floor. Operator worked the rest of the shift but the next morning ankle was swollen so ee called off work and went to the doctor.

Struck against a moving object

Employee was operating a D-6 dozer and was backing up at a 2 to 1 slope and went to shift into forward and the transmission did not catch causing the dozer to free wheel. The operator dropped the blade and this caused EE to lunge forward and rotate from the waist hitting head on the window frame. Operator was wearing seatbelt. Operator received stitches.

Accident type, without injuries

Machine was parked at 1:45am the morning of the 28th and the operator went home, at 3:00am that night a miner that was still there noticed that the equipment was on fire and called 911. No one was around the equipment for over an hour so the origination of the fire is unknown.

Struck against a moving object

Operator said ee hurt back when ee hit a bump in the road. Upon new evidence this has become reportable.

Employee was attempting to put a belly pan on a dozer and a weld broke letting the pan to drop hitting him in the arm and hand.

Employee was climbing into a 16G Cat grader when the wind blew the door around and hit him in the side of his head causing a cut requiring 4 staples.

Employee was driving up out of pit and the sun blinded him and he run his water pumping truck up a bank and laid it over on its side. He worked the rest of shift and the next day and then took 2 days off.

Employee was starting up stairs into parts trailer and grabbed the hand rail. He felt something pop in his shoulder.

Employee was helping mechanics put a pin in a 992 boom when a pin above fell and crushed his pinky finger.

Employee was mounting a D11T dozer. He was on the push arm and reached for the hand hold on the side of dozer and missed. Employee fell and hurt his back and ribs.

Employee was standing on the track of a D-9L dozer and turned to get back into the cab when he lost his footing, he grabbed the hand rail but turned wrong and pulled a muscle in his back.

Employee was walking through water and mud to get to his water pump and stepped on a rock twisting his ankle.

Employee was climbing down the ladder of a 365 Trac-Hoe and lost his footing. He fell approximately 4 feet to the ground injuring his back.

Our Highwall Miner hit an unmapped/ Unknown abandoned mine which was full of water. The water started to fill the pit area that we were in so we retracted our miner a pulled out of the pit area.

EE was using a bar on the pins on a highwall miner and his bar came up and hit him. He received 5 stitches.

EE said his seat in his D11 dozer bottomed out and hurt his back.

Employee was climbing onto his D11 dozer and slipped and fell spraining his left hand.

Struck by flying object

EE had just replaced a drum of grease on his Lube truck and he screwed his grease hose onto it. When he shifted the lever to charge it with air, the end blew off and hit him in the chin. He received stitches.

EE was using a utility knife to cut a water hose. He was cutting away from his body and came to the end and turned his knife back towards himself to cut the remaining inch. He pulled back and stabbed his leg. 3 Stitches were required.

Employee opened the door of his D11 dozer and stepped out onto the deck, he straddled the door to retrieve an air hose that was attached to the cab. The hose had been used for repairs. He went to throw the hose off and lost his grip and fell. He suffered a broken arm.

Employee was climbing onto a D11 Dozer. He was stepping over from push arm to tracks and slipped and fell onto tracks. Went for xrays. Had a bruise on his rib cage.

Employee jumped from his mechanic's truck to put a fire out and when he returned to his truck he noticed his knee was hurting. He went to his doctor and his doctor told him to be off for 2 weeks.

Was removing a transmission filter on a cat 777. Filter was stuck and he put extreme pressure on it. When it let loose he felt a pull in his back. This happened on Monday July 2nd. And he was in pain and went to doctor on Aug. 6th and then reported it to us.

EE WAS CHANGING A GREASE BARREL BAG AND WHILE STANDING ON THE DECK, HE TRIED THROWING THE ALMOST EMPTY BAG OVER HIS HEAD INTO THE BED OF THE TRUCK AND LOST HIS BALANCE AND FELL TO THE GROUND LANDING ON HIS FEET BREAKING HIS LEFT HEEL

WHILE CLIMBING UP THE LADDER ON A CAT 988B LOADER TO START IT UP, EE FELT A SHARP PAIN IN THE LOWER LEFT SIDE OF HIS BACK.

SHOT WAS COMPLETELY LOADED AND WIRED. WHILE RUNNING LEAD WIRES OUT, LIGHTNING STRUCK THE LOADED SHOT. THREE HOLES WERE DETONATED AT APP 1:42 PM. THE AREA HAD ALREADY BEEN CLEARED. THERE WERE N O INJURIES AND NO EQUIPMENT WAS HIT. AUTHORITIES WERE INFORMED.

EE WAS UNDER AN AUGER ONT HE DECK OF A LOW BOY PREPARING FOR TRANSPORTATION. HE HARD HAT FELL OFF. WHEN BENDING DOWN AND RETRIVING THE HARD HAT. HE TURNED SLIGHTLY WHILE COMING BACK UP AND BUM PED HIS HEAD CAUSING A 1 INCH LAERATION. HE WAS TRANSPORTED TO HOSPTIAL WHERE THE DR GLUED THE CUT TOGETHER.

EE AND EE HAD JUST LIFTED A 300 FT SECTION OF COILED WATER HOSE FROM THE BED OF A PICK UP TRUCK AND WAS UNWINDING IT ALONG SIDE AN OLD ROAD WHEN EE NOTICED A DULL PAIN IN HIS LOWER BACK. THE P AIN BECAME INCREASINGLY WORSE AS THE DAY PROGRESSED.

EE ARRIVED AT THE MINE PRIOR TO STARTING TIME IN ORDER TO START A WATER PUMP LOCATED IN PIT 002-0. HE PARKED HIS VEHICLE IN THE PIT AND WAS WALKING TOWARDS THE PUMP ON A LOOSE MATERIAL DIRT RO AD, WHEN HE STEPPED INTO A HOLE TWISTING HIS LEFT KNEE. THERE WERE NO LOST DAYS, HOWEVER THE INJURY IS NOW CAUSING SEVERE PAIN AND THE DR. IS RECOMMENDING SURGERY TO RECONSTRUCT THE LIGAMENTS

A RUBBER TARP, USED FOR HOLDING THE FUEL LINE NOZZLE WHEN THE TRUCK IS IN MOTION, WAS REMOVED AND LYING ON THE TOP STEP AT THE REAR OF THE TRUCK. WHEN HE BEGAN DESCENDING THIS STEP THE METAL HOOK ON THE RUBBER STRAP CAUGHT IN HIS RIGHT PANT LEG CAUSING HIM TO LOSE HIS BALANCE AND FALL. HIS LEFT FOOT STRUCK THE GROUND FRACTURING TWO BONES IN HIS ANKLE. HE FELL 30 INCHES.
