Fall from machine

A miner fell from a height of approximately 9' from a fixed conveyor catwalk while clearing a tail guard. The miner had no fall protection while using a pry bar measuring 70"" and 17 lbs while standing on a fiberglass ladder and the fixed mid-rail. The miner was taken to the ER and received 8 staples to a laceration on EE's head, was given a sling for a strain to EE's shoulder.

Struck by falling object

The 2 employees were attempting to reposition the blow bars in the horizonal crusher, while moving the blow bars it moved and struck the employees little finger on the left hand.

Approx. 10:30am local authorities discovered a man committed suicide by jumping from a public bridge which landed approx. 90ft below in our open pit mining site. MSHA was notified immediately upon discovering the body and an inspector was on our site within a couple hours to conduct a thorough investigation which resulted in no changeability. The police report # is 13-0373.

Employee states while watering roadways, he hit unseen water filled pothole. This caused his back sprain/injury.

Employee was cleaning out the #7 bin on the secondary plant. The inspection door had been removed and a water hose was being used to spray out material. Employee's left hand was resting on the mounting frame of the door, when material broke free striking his hand.

Employee was preparing to cleaning spilled material under 304 conveyor tail pulley with water hose. When he turn on the water hose, water presure caused him to lose his balance and fall backwards against the steps leading to the screen tower and then to the ground landing on his right hand.

Employee was reinstalling a lid on a horizonal crusher after repairs. Employee was hitting the lid with a hammer to align the bolt holes, employee missed the edge of the lid with the hammer, swung back where his other hand was resting, striking the little finger of his left hand.

After employee finished his preshift plant inspection, employee went to the port-a-john. After exiting, employee walked 4-5 feet and tripped over a rock, and landed on his right knee, causing the dislocation.

Employee was replacing toggle plate in jaw crusher, releasing come-along to reseat toggle plate. Employee had his hand on chain of come-along when toggle plate popped up, tightening chain and pinching finger between chain and jaw crusher, causing a laceration on left middle finger.

The person injured was firing a blast, when the connecting wire used to hook in the shot was caught by the wind and hurled into high tension power lines, resulting in electric shock.

EE WAS WALKING TO STEEL RACK, STEPPED OVER 2' WALL, RT. LEG FIRST THEN HIS LEFT LEG, WHEN THE WEIGHT WAS PUT ON HIS LEFT LEG EE FELT PAIN IN HIS LEFT KNEE.

AFTER LOCKING OUT CONVEYOR BELT, EE BEGAN CLEANING SNOW FROM THE RETURN SIDE OF BELT WITH A SHOVEL. EE STATES THAT SHOVEL GOT TANGLED IN CONVEYOR FRAME. WHILE TRYING TO FREE IT, EE LOST HIS BA LANCE, STEPPED BACK ONE STEP AND FELL OFF OF CATWALK.

EMPLOYEE WAS STEPPING DOWN FROM SERVICE TRUCK & STEPPED INTO SNOW COVERED RUT CAUSING RIGHT ANKLE TO GIVE OUT.

EE WAS GREASING A ROCK TRUCK. HE CLIMBED INTO THE CAB TO GET A TUBE OF GREASE. WHEN HE BEGAN CLIMBING DOWN FROM THE CAB HE MISTAKENLY GRABBED THE WINDSHIELD WIPER ARM ISNTEAD OF THE GRAB HANDL E LOCATED BELOW THE WIPER ARM. THE WIPER ARM PULLED AWAY FROM THE TRUCK THROWING HIM OFF BALANCE, HE SLIPPED AND SLID DOWN THE TRUCK'S LADDER LANDING FORCEFULLY ON HIS FEET.

EE WAS BLOCKING THE LIME & SCREENING BIN DOORS OPEN WITH 4""X8""X16"" WOOD BLOCKS TO PREVENT THE DOORS FROM BECOMING FROZEN SHUT OVER THE WEEKENDHE PLACED 1 BLOCK IN THE #8 BIN DOOR & ANOTHER E E CLOSED THE DOOR ON THE BLOCK. HE THEN PLACED 1 BLOCK IN THE #7 BIN DOOR, AND THE OTHER EE CLOSED THE DOOR ON THE BLOCK. WHEN THE #7 BIN DOOR CAME SHUT IT PUSHED THE BLOCK TOWARD THE BLOC
