Employee was stepping from the steps of a haul truck to a manlift platform. Employee's knee buckled and they fell 3' to the bumper.

A Miner suffered a strain to EE's right knee while exiting the cab of a Skid Steer Loader. EE missed the step on the bucket while exiting the cab causing EE to lose EE's balance and fall resulting in EE's injury.

Lumbar strain sustained while carrying used shoes away from the vertical shaft crusher for disposal. Medical attention was requested on 05/18/2020.

***1st day of treatment 10/26/18*****climbing out of the truck, foot caught step & twisted, hurt right leg

Climbing out of the cab, missed the bottom step, fell to the ground, hurt left side of rib cage & left forearm.

Placing a piece of steel in chute it fell striking him on the left calf

Victim scaled fence adjacent to public roadway and jumped to quarry floor below.

Slipped on 990 platform while opening access door. He caught himself jamming right shoulder

Four men were threading bowl of 1560 crusher into mainframe. While pushing on spud bar, the injured employee fell forward, hitting railing and fell backward to decking landing on his back. Unknown if bar slipped or crusher moved.

While using a 6' step ladder, he leaned against the clean burn furnace. The ladder bottom moved out causing employee to fall 3.5' - 4' striking left shoulder/arm on anti-spin device from omni cone crusher which was laying on the shop floor.

MOVING OLD SUPPLY TRAILER & DIRECTING LOADER, STUMBLED OVER BRUSH & FELL ON OLD SHOVEL, STRIKINGRIGHT KNEE. (SHOVEL WAS NOT VISIBLE) DID NOT SEEK MED ATTENTION UNTIL 4/2/001
