Fall onto or against objects

Employee was stepping from the steps of a haul truck to a manlift platform. Employee's knee buckled and they fell 3' to the bumper.

Fall from machine

A Miner suffered a strain to EE's right knee while exiting the cab of a Skid Steer Loader. EE missed the step on the bucket while exiting the cab causing EE to lose EE's balance and fall resulting in EE's injury.

Over-exertion in lifting objects

Lumbar strain sustained while carrying used shoes away from the vertical shaft crusher for disposal. Medical attention was requested on 05/18/2020.

Fall to the walkway or working surface

Four men were threading bowl of 1560 crusher into mainframe. While pushing on spud bar, the injured employee fell forward, hitting railing and fell backward to decking landing on his back. Unknown if bar slipped or crusher moved.

Fall from ladders

While using a 6' step ladder, he leaned against the clean burn furnace. The ladder bottom moved out causing employee to fall 3.5' - 4' striking left shoulder/arm on anti-spin device from omni cone crusher which was laying on the shop floor.
