While changing a tire, the side/flange ring and bead seat band fell off of the wheel striking thumb of the technician.

Three employees were performing maintenance on screen bearing. All 3 were wearing safety gloves. While 1st employee was using a pry bar to pry the bearing up, 2nd employee put in a spacer piece, then 3rd put one in. The opposing side slipped and the bearing, housing and Mag bar fell pinching 3rd employee's left thumb, resulting in a laceration. EE received 6 stitches.

EE was using a truck boom to push down the pusher bar to push bead seat band down. The boom slipped off the pusher bar while EE was reaching under to install the o-ring. The boom slipped pinching EE's finger. Left middle finger laceration received 17 stitches.
On 1/25/18, EE was cleaning feeder tube and twisted knee when EE stepped up onto a flange. This incident became reportable due to lost time as a result of knee surgery performed on 7/5/18.
EE removing transmission on pickup when tool slipped causing EE to hit hand on transmission or framework of truck cutting top right pinky finger. At TOI, EE refused medical treatment, just bandaged cut. EE contacted supervisor at 9:42PM and went to hospital resulting in 3 stitches, shots and a splint.

IE felt pain in lower back during pre-shift inspection when IE bent over. IE informed supervisor at 9:09am and was taken to hospital. Reportable due to restrictions, prescriptions and lost time. IE was transferred to different job based on restrictions and missed 10/27/17 & 10/30/17.
Injured EE and witnesses were changing a bend pulley on 50 conveyor. While performing the repair, the mast on the telehandler appeared to be lodged against the conveyor frame. When injured EE reached inside of conveyor frame to hook chain on pulley shaft, the mast moved, pinching left arm between conveyor frame and telehandler mast causing injury.

Injured employee was on top of the 777C haul truck cutting the bolt with a cutoff wheel to remove mirror. The cutoff wheel caught on the bolt, kicked out and cut left thumb. The wound required sutures to close.
EE was supervising re-installation of floor plate. Plate was lying flat on the floor to slide into position, but it became caught on support structure. EE tried to lift above, but it sprang free & slid toward EE, pinching EE's left index finger. Sutures were required.

EE RUSHING UNDER CONVEYOR TO AVOID GETTING WET & HIT HEAD ON CONVEYOR FRAMEWORK LABELED LOW OVERHEAD. EE REFUSED MEDICAL ATTN AT TIME OF ACCIDENT. EE ON VACATION 8/28-9/2/13. ON 9/3, HE COMPLAINED TO MGR OF STIFF NECK/DIZZINESS. EE SENT TO CLINIC ON 9/5 & INCIDENT BECAME REPORTABLE DUE TO LOST TIME.
EE DETERMINED THAT FINE GRIND MATERIAL WAS FREEZING IN THE BIN SO HE DECIDED TO START THE BIN HEATERS. HE DISMOUNTED HIS LOADER AND TOOK TWO OR THREE STEPS BEFORE SLIPPING ON SNOW COVERED GROUND, FALLING ONTO HIS RIGHT SIDE. ON 2/5/13, X-RAYS REVEALED RIGHT WRIST SPRAIN RESULTING IN A WRIST SPLINT.

EE WAS REMOVING WHEEL CHOCKS W/L HAND OUT FROM UNDER L FRONT WHEEL OF TRUCK. AS HE PULLED ROPE & TWISTED CHOCKS FOR REMOVAL, FELT PAIN IN TOP OF L HAND. EE REFUSED MEDICAL TREATMENT ON 3/3/11. DURING HIS SHIFT ON 3/4/11, EE NOTICED SWELLING & TENDERNESS IN HAND & ASKED TO SEE DOCTOR. EE WEARING SPLINT ON L ARM/WRIST MAKING INJURY REPORTABLE ON 3/4/11.

EE WAS WASHING BETWEEN PLANT SCREENS WHEN NOZZLE ON WATER HOSE SLIPPED AWAY FROM EE'S GRASP AND HYPER-EXTENDED THE EE'S THUMB. EE IS WEARING A SPLINT ON RIGHT HAND UP TO BELOW THE ELBOW.
EE working on conveyor, turned and tripped over guard that was left on the deck. EE tripped/fell landing on left side of body. He refused treatment and declined to fill out investigation report at time of accident. We were informed on 6/9/2010 that MRI was positive for Rotator Cuff tear so this became reportable on that day.
EE WAS WALKING IN 3"" DEEP SNOW AND DOWN A SMALL INCLINE. THERE WAS ICE UNDER SNOW. HE SLIPPED ON ICE CAUSING HIM TO FALL FLAT ON HIS BACK FROM HIS HEAD TO HIS TAILBONE. EE WORKED 2/26 - 3/2 AND WAS ON VACATION FROM 3/3 - 3/9. THIS INCIDENT BECAME REPORTABLE ON 3/10/2010 BECAUSE MGMT HAD TO MODIFY HIS WORK DUE TO RESTRICTIONS.

EE WAS LIFTING A 60-70# BUCKET OF AGGREGATE TO DUMP IT INTO THE SAMPLE SPLITTER. BUCKET WAS SITTING ON THE FLOOR AND NEEDED TO BE DUMPED IN SPLITTER (~39"" HIGH). EE REPORTED THAT HE FELT SOM ETHING ""PULL"" IN HIS LOWER BACK OR RIGHT HIP WHILE ATTEMPTING TO LIFT. THIS BECAME REPORTABLE ON 9/22/09 AFTER EE WAS MOVED TO DIFFERENT POSITION DUE TO ADDITIONAL RESTRICTIONS.
EE WAS TIGHTENING THREADED DOOR LATCHES ON EAST ACCESS DOOR OF WEST HAZEMAG CRUSHER USING AN 1 1/2"" COMBINATION WRENCH. IN ORDER TO TIGHTEN TOP LATCH EE USED A HAMMER TO STRIKE THE WRENCH. WRENCH SLIPPED OFF THE HEX NUT ON LATCH PINCHING THE EE'S FINGER BETWEEN THE WRENCH AND LOWER LATCH.
RAINY, SLIPPERY & MUDDY. EE STEPPED DOWN OFF OF LADDER IN TIGHT QUARTERS ONTO UNEVEN FLOOR, TURNING ANKLE UNDER RIGHT LEG.

EE assisting 2nd EE in lowering lever to close safety gate. The lever slipped from grasp of 2nd EE causing full weight to force injured EE to floor impacting & twisting his right knee. EE refused medical attention at TOI, but later went to Hospital. On 12/30/08, we were notified verbally that MRI was performed & surgeon had recommended surgery making this injury reportable.

Employee was bringing a sample bucket off the sample pile. He slipped in the wet material, fell and caught himself on the tailgate of the pickup truck. Initially sought medical attention on 9/10/07. On a followup visit, doctor applied a hard cast on left wrist, which was brought to our attention on 11/12/07. This accident became MSHA reportable at that time.
Employee turned to walk over to the welding truck and stepped into a 5 1/2"" drill hole twisting his left knee. On April 12, 2007, employee informed management that he had received results on April 10, 2007 of an MRI that was positive for ligament damage in the knee. This accident was not reportable until we received the positive results from the MRI on April 12, 2007.

Injured employee was holding wrench on nut to tighten. Another employee was using impact wrench on bolt and when he hit the impact wrench, the injured employee's wrench spun around and pinched his finger between the frame of the crusher and the wrench.
Employee was splicing #1 conveyor. He was attempting to cut belt when his arm hit a nail sticking up when he was sliding the Flexco cutter back and forth on the track. The wound required sutures (stitches).
EE was working in an elevated manlift. A passing loader struck the tire of the manlift, causing the boom basket of the manlift to shake vigorously. EE strained his back, arm & leg resisting the motion.
Employee brought in Physician's Report of Work Ability to GM on 4/20/05 at 4p. Report stated that EE was totally disabled from work from 4/21/05 to 5/8/05 w/ no DOI listed, but claim #02-816744. This claim relates back to 4/3/02 & 10/22/04. When the GM asked employee what EE did to hurt back, EE replied, ""nothing EE could recall.""

Employee woke up Friday morning with back pain and brought a doctor's slip in excusing him from work from 10/22/04 through 10/31/04. This injury was originally reported on 4/2/2002 and this injury report is being refiled per Bob LeMasters per 11/2/04 phone conversation.
While driving stock truck, employee hit a bump and hurt back.
EMPLOYEE SLIPPED IN MUD AND TWISTED KNEE. DID NOT SEEK MEDICAL ATTENTION UNTIL 12/1/2004. AT THAT TIME, X-RAYS WERE POSITIVE FOR INJURY.

EXITING SHOP THROUGH ENTRANCE DOOR. EE TOPPLED FORWARD & TO THE LEFT, FELL DIRECTLY TO THE GROUND & LANDED ON LEFT SIDE. AFTER HE CONTACTED HE BEGAN TO CONVULSE (APPROX 10-20 SECONDS). DID NOT APPEAR TO LOSE CONCIOUSNESS & WAS ALERT & AWAREOF HIS SURROUNDINGS AFTER THE CONVULSIONS STOPPED. EE HAS A HISTORY OF KIDNEY DISORDER & WE FEEL THAT MAY HAVE CONTRIBUTED TO THE DIZZINESS.
EE WAS TRACKING 9 CONVEYOR BY BEATING ROLLERS WITH A HAMMER. HE MISSED THE ROLLER & SMASHED HIS FINGER, WHICH REQUIRED STITCHES.
EMPLOYEE WAS CLIMBING DOWN LADDER FROM LOADER & CAUGHT HIS RING ON RUNG OF LADDER CUTTING FINGER OPEN. STITCHES WERE NEEDED.
EMPLOYEE WAS SHOVELING MATERIAL IN WASH PLANT WHEN HE COMPLAINED OF A BURNING IN HIS STOMACH AREA. EMPLOYEE WS PUT ON RESTRICTED DUTY FOR 1 DAY.
EE WAS TRYING TO REMOVE STEEL CAP FROM USED OIL TANK. UPON HITTING THE CAP WITH A HAMMER, A PIECE OF SOMETHING WENT IN HIS EYE. IT WAS REMOVED SURGICALLY AND EYE DROPS WERE PRESCRIBED.

CRAWLED INTO BIN, PUT HIS HAND ON SHARP AUGER, CUTTING 5TH FINGER ON RIGHT HAND REQUIRING 3 STITCHES.
EE WAS WALKING DOWN STEPS AT XOVER WHEN HE SLIPPED ON STEP CAUSING HIM TO FALL. WHEN HE FELL HE COULD FEEL PINCH ON BACK SIDE OF LEG. CONTINUED TO WORK THEN PAIN BECAME UNBEARABLE, SO HE WAS T AKEN TO HOSPITAL WHERE THEY FOUND LACERATION ON OF LEG.
